ANU CENTRE FOR HEALTH
STEWARDSHIP
AUSTRALIAN NATIONAL UNIVERSITY
CENTRE FOR HEALTH STEWARDSHIP
BRIEF SUMMARY
The Centre for Health Stewardship (the Centre)
was established in 2004. It is part of the Australian
National University College of Medicine, Biology
and Environment. The Centre’s mission is to conduct
research and teaching to improve our understanding
of how the health systems are governed, and the
relations between this governance and the health
of the community. The Centre conducts research
and teaching about how health departments are
organised and operate to fulfil their roles within the
national health system. The centre also examines the
areas of clinical governance and the legal basis of
health stewardship.

Qualitative study of the experiences of dealing with
disaster among adults affected by the 2003 Canberra
bushfires. Dr Theresia Citraningtyas. Adults affected
by the 2003 Canbera bushfire were interviewed to
develop an explanatory framework of the processes
of dealing by disasters from the perspective of the
people affected. This is also the first part of a crosscultural research project to identify how language,
cultural norms, and social constructions determine
perceptions and expressions of experiences of
dealing with disaster.

Long term outcomes of group Interpersonal
Psychotherapy for postnatal depression. Rebecca
Reay, Prof Cathy Owen, Prof Beverley Raphael and
Dr Bruce Shadbolt. This study investigated whether
postnatally depressed mothers would respond to a
short term group treatment based on Interpersonal
Psychotherapy. At two years post-intervention,
treated mothers had more favourable outcomes
that mothers who received usual care. A number of
publications and presentations have resulted from
the study including a treatment manual.
Experiences of mothers with 2-year old infants: an
investigation of their emotional health, perceived
stressors and access to treatment and support.
Rebecca Reay, Adjunct A/Prof Stephen Matthey and
Prof David Ellwood. The research team investigated
the long term outcomes of ACT mothers who
participated in an early detection program for
perinatal depression. There have been a number
of publications in peer-reviewed journals and
presentations at scientific meetings.

Singapore Study of Successful Aging [SSOSA], A
collaborative project between NUS and UCSD,
USA: Dr Rajeev Kumar. This study evaluates the
prevalence and determinants of successful aging in
65 years and older subjects living in the community.
About 500 subjects with diverse ethnic background
(Chinese, Malay and Indians living in Singapore) are
completed the interview. Study completed and data
currently being analysed.
Exploring Risks for Child and Adolescent Mental
Health Problems, Dr Virginia McAndrew. Literature
review to find reports of the prevalence of trauma
and loss in children, particularly in clinical
populations.
Risk perceptions and health beliefs of people with a
mental illness, Dr Paul Maguire. This study evaluated
the risk perceptions and health beliefs of people
(aged between 18 and 65) with a mental illness and
their willingness to comply with health protective
behaviours. It is important that they receive relevant,
evidenced-based health information as they have
been shown to suffer from high rates of comorbid
medical disorders and reduced longevity.

Respiratory Gating for Radiotherapy Treatment

Establishing the Factors that Contribute to a Shortfall
in Evidence-based Radiotherapy, G Delaney.
During acquisition of CT scans for planning and
during treatment of cancer patients, tumours in
the chest wall and thoracic cavity move with the
breathing movement of the patients. The respiratory
movement causes an uncertainty in the location of
the tumour. New CT and treatment machines are
provided with a respiratory control linkage which
allows CT reconstruction and radiation treatment to
occur only during chosen phases of the breathing
cycle. The department is collaborating with the
Radiation Oncology department to research and
develop protocols to use this facility and carry out
measurements to determine the efficacy of the new

technique and improvement in radiation dosage.
Development of a Three-Dimensional Gel Dosimetry
System, Dr Helen Gustafsson in association with
Radiation Oncology, Capital Region Cancer
Service. A gel dosimetry system that can be used
for verification of complex, dynamic radiotherapy
treatment has been successfully implemented at TCH.
The system can be used as part of the introduction
of novel techniques such as Intensity Modulated
Radiation Therapy (IMRT), gated therapy and
stereotactic radiotherapy.
Testing of PET/CT Quality Control Baselines, Mr
Farshid Salehzahi in association with Medical
Imaging, Critical Care and Diagnostics. The Canberra
Hospital has recently installed a state-of-the-art PET/
CT. In order to utilise the imaging available from
a PET/CT in a quantitative manner, the calibration
of the PET/CT needs to be maintained. During the
acceptance, installation and commissioning of the
PET/CT, baseline measurements of the characteristics
of the PET/CT were acquired. This project involves
testing these quality control baseline measurements
and establishing a routine testing regime for future
use.
Development of a Monte Carlo Benchmark for
Clinical Electron Beams, Dr Sean Geoghegan, Mr Ben
Cooper and Mr Michael Smith in association with
Radiation Oncology, Capital Region Cancer Service.
The ability to perform accurate and reliable Monte
Carlo particle transport has become indispensable in
large radiotherapy departments. The department is
particularly interested in modelling dose distributions
due to electron beams at oblique incidence. Areas of
specific clinical interest are skin treatments and tinfoil bolus treatments of the nose. The project aims to
build and verify a Monte Carlo phase space model for
(at least) one clinical electron beam energy. BEAMnrc
software and commercial linear accelerator designs
are used for the purpose of this research.
The Assessment of New Radiation Oncology
Treatments and Technologies (ANROTAT), Dr Sean
Geoghegan and Mr Ben Cooper in association with
Radiation Oncology, Capital Region Cancer Service
and the Trans-Tasman Radiation Oncology Group
(TROG). There is little clinical evidence available for
the efficacy of advanced radiation oncology treatment
techniques. TROG has been commissioned to
undertake project work by the Department of Health
and Aging (DoHA) to develop and pilot an evaluation
framework for the assessment of new radiation
oncology technologies and treatments. This project is
being undertaken in response to a recognised need
for the Medicare Benefits Scheme (MBS) to support
appropriate new radiation oncology technologies
and treatments as they become available, to ensure
optimal patient care. A research framework is being
developed and piloted to assess the safety, clinical
efficacy and cost effectiveness of IMRT and Image
Guided Radiation Therary (IGRT) in four tumour sitespecific projects:
1.Nasopharynx (IMRT)
2.Anal Canal (IMRT)
3.Post-Prostatectomy (IMRT)
4.Intact Prostate. Volumetric Imaging versus the use
of fiducial markers (IGRT)
Respiratory Modulated Four-Dimensional Cone
Beam Computed Tomography, Mr Ben Cooper in
association with Prof Paul J Keall, University of
Sydney. This 5+ year research project started at the
end of 2010 and will constitute Mr Ben Cooper’s PhD
which he is studying part time. Benefits have already
been realised by the Radiation Oncology department
as this project has expedited the clinical cone beam
CT implementation project. Furthermore, ACT Health
will be participating in leading edge research that
directly benefits patients receiving radiation therapy.
Tin Foil Modified Electron Beams for Superficial
Cancers, Dr Sean Geoghegan in association with
Radiation Oncology, Capital Region Cancer Service.
Non-melanoma skin cancer (NMSC) is common in
the Australian elderly population with a long history
of chronic sun exposure. Radiotherapy for skin cancer
of the nose has been technically difficult due to the
surface curvature. We are researching a radiation
technique using tin foil electron for the nose to allow
homogenous cover of the superficial curved surface.
We aim to maintain the high cure rate of NMSC and
good cosmesis with a new radiation technique.

Calibration of diode probes and MOSFET detectors
for in-vivo dosimetry in HDR brachytherapy
This is ongoing work being carried out by Dean
Wilkinson. The outcome of this work is that patients
undergoing brachytherapy, a form a radiation
therapy for cancer treatment, will benefit from a
more accurate dose monitoring system whilst they
undergoing the actual treatment. This will provide
the radiation oncologist with more information that
will facilitate better patient management.
A Study into dosimetry of the Shielded Vaginal
Cylinders in HDR Brachytherapy. This project
was undertaken by Simran Gill. The main aim
of this study was to verify the dose distribution
of the standard vaginal plans prescribed by the
Brachytherapy Planning System. The transmission
factors were measured using a combination of 90
and 180 degree tungsten shields. An estimate of
the error introduced by the planning system due to
absence of heterogeneity corrections was also made.

Dementia Medications: How are they Managed?
PracNet members have raised concerns in relation
to the lack of clinical guidelines for ceasing these
medications. The aim of this project is to identify
patterns of use for cholinesterase inhibitors and
Memantine in the treatment of Alzheimer ’s disease
patients. The hypothesis is: patients started on
cholinesterase inhibitors or Memantine by a specialist
rarely have the medication ceased by their GP.
