AMBULANCE GREEMENT ETT coRPoRATlON. AND DALEY'S MEDICAL TRANSPORT INC'. d/b/a BUD'9 AMzulaNaE SERVICE, medical WHEREAS, the VILLAGE OF DIXMOOR desires to contract for emergency services; and provider of advanced life WHEREAS, BUD'S Ambulance, an lllinois corporatlon, is a support,/ paramedic services licensed by the State of lllinois Department of Public Health; and private entities wHEREAS, the VILLAGE OF DIXMOOR is empowered to contract with with BUD'S for advanced life support,/paramedic services and is desirous of contracting Ambulance., an lllinois corporation, to provide said service; and NOW, THEREFORE, be it agreed as follows: Page 1 This Agreement Nr \ at+ \N[l{.tnbgL , 2013, by and between the made this|l-day of (hereinafter referred to as VILLAGE) VILLAGE OF DIXMOOR, an lllinois municipal corporation and BUD'S Ambulance., dfl lllinois Corporation, (hereinafter referred to as BUD'S) in follows: consideration of the mutual covenants and promises contained herein as l. The police Department of the VILLAGE shall respond in situations where safety unruly or intoxicated could be a concern, i.e., domestic violence, injury due to criminal activity, persons. BUD,S shall have the authority to request that the Police Department respond to calls as provided for in this paragraph' 9. BUD',S shall also comply to the following requirement or concerns: A. Shall maintain Quality Assurance Program to insure that the best possible patient care is delivered' B. For example, line item for a year saw fuel prices exceed $3.00 per gallon, the second month bill would contain an additional prices dropped z% tuel surcharge totaling $63.93. This charge would not appear on the third month bill if fuel in a 4% surcharge' below $3.OO during the second month.
