H A M I LT O N FA C I A L P L A S T I C S U R G E R Y SPRING 2009 Eye Rejuvenation: Options Abound Signs of aging and looking tired begin in the eye area for most women and men. Many will complain that family, friends and co-workers ask if they are tired even when they are fully rested. We hear again and again: “I look so tired/old, but I feel great”. People are living longer and staying more active later in life; and they want to look as good as they feel. transconjunctival (no external incision, done through the inside eye lining). Most patients require an external incision to remove sagging skin. The incision and resultant minimal scar are well camouflaged by a patient’s eyelashes. Fatty tissue can be removed via both approaches, lessening the puffy look seen in aging eyes. Drooping of eyelid skin and bags under the eyes are what most patients want to go away. Relaxation of skin is a natural part of aging due to changes in collagen and elastin fibers in the dermis and structural changes in the epidermis. The “bags” that we see with time are actually fatty tissue that prolapses with relaxation of supportive tissues under the skin. Both of these are directly addressed with blepharoplasty. Blepharoplasty is typically done under anesthesia, but can be performed under local in select patients. Recovery is much easier than most patients expect. While there is some discomfort, patients are usually pleasantly surprised to find that they are actually numb around the eyes afterwards; most patients rarely take a pain pill. As a specialist in facial plastic surgery, Dr. Hamilton has made blepharoplasty and periorbital rejuvenation a focus of his practice. He has written and lectured on procedures in this area and regularly performs eyelid cosmetic surgery. Blepharoplasty can be done on the upper or lower eyelids. Through an incision in our natural upper eyelid crease, Dr. Hamilton is able to remove excess skin as well as address any redundant fatty tissue. In the lower eyelids, incisions may be made just below the eyelashes or Sutures remain in a week and make-up may be applied shortly after that. Dr. Hamilton was asked to co-author a chapter in Papel’s Essentials of Facial Plastic Surgery on Laser Resurfacing. This textbook is widely considered the authoritative guide in facial plastic surgery. Dr. Hamilton’s chapter covers all aspects of laser treatments of the skin, from minimal downtime fractional treatments to full face CO2 laser. The book will be available later this year. Additional options for eyelid rejuvenation include laser resurfacing, brow lifts, and non-surgical treatments such as eyelash enhancement, eyelid focused skin care and make-up. Many patients who come in seeking eye rejuvenation actually have brow ptosis. An endoscopic brow lift can lift the brows and subsequently lessen upper eyelid skin. With today’s endoscopic techniques, incisions are camouflaged behind the hairline and recovery time is similar to a blepharoplasty alone. Consultation with Dr. Hamilton will help determine if brow lift is part of your eyelid rejuvenation. Dr. Hamilton also authored a chapter on hair restoration for Surgical Facial Rejuvenation, another comprehensive textbook on facial plastic surgery.
