The fee for physicians is Course $150; Course plus Hands-On
Lab $200. The fee for fellows and nurses is Course $25; Course plus Hands- On Lab $50. Continental breakfasts, lunches
and buffet dinner 8/8 are provided. Space is limited. Registra­tion is accepted on a first-come, first-served basis. Refunds

be issued.
