Introduction
The School Support Program began in 2004 as a new joint initiative between
the Ministry of Children and Youth Services and the Ministry of Education.
The program was created to offer additional services and supports for the
increasing number of students with Autism Spectrum Disorder (ASD) in Ontario.
Surrey Place Centre is the agency responsible for the School Support Program
(SSP) in the Toronto Region.
The SSP offers services to enhance existing supports in publicly funded school
boards and school authorities. The programs objectives are to:
• enhance the knowledge and skills of school board staff with respect to ASD,
the principles and applications of applied behavioural analysis (ABA), and
other evidence-based teaching strategies
• promote linkages between schools, school boards, autism service providers,
and the community
The core services of the SSP as mandated by the program guidelines are
consultation, training, resource development and distribution.
The Community Resource Directory was created to assist adults with ASD
and their families, schools and the community in navigating the often complex
system of service delivery for individuals with ASD. The directory is part of the
School Support Program’s resource development initiative.
This fourth edition has been revised and updated in order to provide the most
comprehensive and current listings. Every effort has been made to represent
each agency and service as accurately as possible, however, programs and
services do change frequently and readers are encouraged to call agencies to
find out specific details of service availability.

About Surrey Place Centre
Surrey Place Centre provides specialized clinical services that are responsive
to individual need and promote health and well-being in the Toronto region.
We help children and adults living with developmental disabilities, Autism
Spectrum Disorder and visual impairments reach their full potential. We offer
a variety of groups and workshops for clients, families and caregivers, as well
as extensive education and consultation services to community agencies. Our
comprehensive programs and services range from assessment, diagnosis, and
one-on-one treatment, to family counselling and group support, and it is all
provided by a broad network of clinicians and professionals.

Entering Adult Services – Helpful Tips and Tools
Planning Tips
As adults with ASD and their families enter the adult services sector, navigating
services can be complex. Listed below are some helpful tips to consider:
• begin the planning and application process for adult services as early as
possible. Develop an action plan with an outline of the tasks that are to be
completed
• join advocacy and community support groups to stay informed on existing
programs and new services
• when inquiring and applying for services, keep a log of phone calls,
e-mails, and appointments made with the agencies and professionals
involved
• research the different community programs and service providers that are
available to determine most appropriate agencies

French Services
Surrey Place Centre supports the development of quality French language
services within the region of Toronto for people with developmental disabilities,
blind and low vision and autism. Surrey Place Centre has generated a plan
in October 2012 to further improve our French Language services across the
centre within the next few years. With 3 programs currently offered in both
official languages (Connections for Students, DSO-TR and ABA servicesAutism) and bilingual signage across the centre, Surrey Place Centre is on its
way to better serving the French-speaking population in Toronto.

de qualité dans la région de Toronto pour les personnes ayant une déficience
intellectuelle, aveugles et malvoyants et l’autisme. Centre Surrey place a
élaboré un plan en octobre 2012 pour améliorer davantage les services

Treatment Services
Crisis Supports
This section lists professional services including those provided by
psychologists, speech therapists, occupational therapists, physiotherapists, specialized medical and dental services, social workers
and dieticians. The agencies provide services to persons over the
age of 18 years (including those with ASD) who are experiencing
emotional, behavioural, developmental and/or mental health issues. Services include: crisis intervention and support, diagnostic/
assessment services, counseling (individual, couple, family, and
group), behaviour management, consultation, service co-ordination,
community referrals, and psychiatric services (including medication
management, inpatient and outpatient treatment).

In crisis situations, access to short-term residential settings (up to 30
days) and short-term case management contact, Griffin Community

Eligibility
Persons with neurological disabilities, including acquired brain injury,
brain injury at birth, cerebral palsy, autism, stroke, fetal alcohol syndrome,
down syndrome, developmental disabilities, speech, reading and learning
disabilities, attention deficit disorder, attention deficit hyperactivity disorder,
perceptual problems and sensory motor disabilities.
Description of Service
Home-centred rehabilitation programs to help improve learning, behaviour
and performance abilities and stimulate brain development, individualized
treatment programs. Fee for services may apply.

Griffin Centre- Griffin Community Support Network
Eligibility
Individuals 16 years and older with mild developmental delay or dual
diagnosis.
Description of Service
Services include: time limited transitional supports (response time within
24 hours), onsite assessment and consultation and case management (1-6
weeks).
Counseling / Treatment: Individual, family and group; access to hospital
supports, short-term case management, short-term and transitional
support; after-hours crisis support.
Short-term Crisis Support: Residential support (0-30 days with possible
extension) options for individuals who are not in acute psychiatric crisis and
are able to be placed in the community.
Community Living Residential Option: for individuals who are homeless,
at risk of being homeless, frequent users of hostel services, have had
multiple hospitalizations and/or involvement in the criminal justice system.
Day Program: 30-90 day supports to transition individuals to less intensive
community-based alternatives.

Dual Diagnosis Service – Inpatient Program

Eligibility: Individuals 18 and 65 years old with a serious mental illness and
a developmental disability with associated behavioural challenges.
Description of Service: a 10-bed inpatient unit that provides service to
adults with a serious mental illness who are developmentally challenged
and show signs of significant behavioural challenges. Services include
specialized interprofessional assessments, diagnostic clarification,
medication review, individualized programming including life-skills,
recreation, supportive counselling and group programming, behavioural
interventions, family support, and education and consultation. Referrals are
accepted from a healthcare professional, an individual with mental illness,
or a family member. All referrals are welcome, including those outside the
agency’s primary service area .
Dual Diagnosis Service – Outpatient & Consultation Service
Eligibility: Individuals between 18 and 65 years old who are medically
stable and present both a serious mental illness and a developmental
disability and/or associated challenging behaviours.
Description of Service: services include assessments, consultations,
behavioural support, shared care, and assistance connecting patients
with community resources. Referrals are accepted from a medical doctor,
healthcare professional, or by self-referral. All referrals are welcome.

Eligibility
In-home health and social services - Includes adults and children with any
illness, physical, psychiatric or developmental disabilities whose needs
cannot be met on an outpatient basis.
Placement in long term care facilities - adults 18 years and over with
disabilities. Must need assistance with one or more of the following: tasks
of daily living, monitoring of medication availability of on-site nursing, or is
at risk of being abused or abusing others, and community services are no
longer sufficient.
Description of Service
Arrangement of in-home or school-based health and social services, and
placement in long-term care facilities, service coordination information and
referrals for related community services.
In-home services: nursing care, physiotherapy, occupational therapy,
speech-language pathology, social work, nutritional counseling, personal
support and homemaking. If eligible for above services, other services may
be provided such as drug card medical equipment or supplies, laboratory
services, transportation.
Placement in long-term care facilities: access for admission to all longterm care facilities in Toronto or for transfers to facilities outside Toronto.
Does not include senior’s apartments, retirement homes, or chronic care
hospitals.
Short stay care (vacation or respite): respite for caregivers, supportive
care for those who need to recover strength, including those with
Alzheimer’s Disease.
Crisis placements: if support breaks down at home, person is placed
wherever a bed is available while awaiting placement in the facility of

Financial
Services
This section provides information about financial supports available
to adults with ASD and their families. Some of these services are
government programs that require a formal application process and
some are charitable organizations that provide funding for specific
needs. It is advisable for families to discuss all available financial
assistance with someone from the DSO. These professionals are
knowledgeable about funding availability and the procedures that
must be followed in order to access financial support.

Passport to Community Living
This is a provincial government initiative. Succesful applicants with
a developmental disability and have left school can use Passport
funding to buy supports and services to help them be more involved
in the community. The money is usually for three years.
Passport is not an entitlement program.There are limits on the
amount of funding available through Passport, and the allowable
amount may not be sufficient to fund everything an applicant might
like. Passport also may have an impact on the amount received
under the Special Services at Home (SSAH) program. Anyone having
questions about the potential impact of Passport on SSAH funding
should check with their SSAH local office. Also, those receiving day
supports through a Ministry-funded developmental services agency
will need to give them up if they accept Passport funding.

RDSP allows caregivers to contribute a lifetime maximum of $200,000 to
an individual beneficiary with a disability up to the end of the year in which
a person attains 59 years of age. Annual RDSP contributions will attract
Canada Disability Savings Grants (CDSG).

Provide 2 -3 day supply of food based on number of people in family.

Henson Trust (The Absolute Discretionary Trust)
Description of Service
Parents or caregivers of individuals with a disability can place assets
aside without disqualifying them from receiving the ODSP payments to
which they would otherwise be entitled. A Henson Trust is a discretionary
trust used for a beneficiary with a disability to protect his or her social
assistance benefits and to ensure that the funds are managed properly
after the death of the parent/caregiver. For more information please speak

Community Participation Supports
These are programs for when an individual finishes school. These
may be necessary for individuals who are not attending a postsecondary institution or working full-time. They offer various activities
and supports in the community or program settings.
It is important to select the most appropriate program based on the
individual’s strengths, needs and interests.

These are some of the types of supports that may be offered:
Centre Based Activities: Opportunity to practice and improve upon
daily living and work readiness skills.
Individualized Programming: Provides an individually tailored
program to work on goals. Goals can be established informally or
through a person directed plan.
Life Skills: Opportunities to develop and practice skills for daily
living (i.e. hygiene, safety skills, cooking).
Literacy/Academics: Activities to assist with reading, writing,
budgeting skills etc.
Recreation, Leisure and Arts: Participating in the community in a
variety of settings including sports, arts programs and community
outings.
Work Experience: Job training and placement opportunities with
the community provided by employment counsellors or job coaches.
Employment Services
Employment opportunities vary depending on the individual and may
range from competitive employment (a job with wages and, usually,
minimal supports) to sheltered workshops (work that takes place
in a self-contained space with long-term support). Please note that
some of the job search agencies listed in this section may not have
supports in place to meet the needs of all individuals. It is up to the
individual and/or their family to ensure there is adequate support or
an escort when necessary.

Description of Service
Centre-Based Day Program: for adults with complex needs, severe
behavioural problems due to mental health and/or developmental
challenges. The program engages participants in recreational, social
and work activities which are offered as therapeutic tools to reach their
individual goals.
Lifestyle Strategies: for young adults with developmental challenges, some
level of independence required. The program assists people in making the
transition from school to community life; helps participants become familiar
with a variety of social, recreational and vocational life choices, enhancing
their opportunities for employment.
Culinary Connections Program: a highly participatory training course
where participants work in a fully functioning commercial kitchen under the
direction of a qualified chef-trainer. The program provides each individual
with the optimal skills appropriate to his or her abilities suitable for basic life
skills and competitive employment.
Farm Project: year-round opportunities to be involved in the physical work
of caring for animals, gardening and beekeeping. Working and caring for
the animals provides opportunities to develop and learn about sensitive
touch and to differentiate appropriate from harmful behaviours.

Pathways of Extended Learning: funded by the Ministry of Community
and Social Services. This program facilitates the transition from school
to the community for adults with ASD by providing support for vocational
goals, further education, leisure activities, social skill building and
community participation. Opportunities are created to provide choices
and options for successful community participation and optimal individual
independence, after school completion.
Eligibility: Adults between 18 – 29 years of age who have a diagnosis
of ASD or Asperger’s Syndrome and live in Toronto. An eligibility

Monarch Café Culinary & Hospitality Program: six month vocational
program that provides students with a blended training experience working
in a Café, combined with classroom instruction. This is a fee based service
and successful applicants will be required to pay $1500 for this 6 month
program.
Eligibility: Individuals between 18 – 29 years of age diagnosed with
ASD or Asperger’s Syndrome. Individuals must also have moderate to
low support needs, communicate verbally or effectively use a functional

Description of Service
If approved for Passport funding, participants can purchase services and
supports that will facilitate their community participation according to their
interests and strengths. They can choose to make their own arrangements
to purchase supports and/or to access services through community based
agencies.
Supports include: work or volunteer activities, activities that develop skills
in utilizing community infrastructure (e.g., transportation services, shopping
and libraries, activities of daily living that enhance social skills, independent
living and personal management) and employment preparation activities
where the individual does not meet the criteria for funding through the
Ontario Disability Support Program (ODSP) Employment Supports.

Pegasus has 4 program sites in east Toronto, each offering support to
between 6-10 individuals. Community inclusion is the focus. Each site
offers a wide choice of activities. Community recreation centres are the
base. Participants are able to play a vital role in mainstream community
life via volunteer and paid work, fitness, social and emotional coaching, life
skills development, exploration of the arts, discovering the city, productive
enterprise and other opportunities. Participants have the option of working
in the Pegasus Thrift Store with the support of staff and volunteers. A wide

their homes safely and comfortably in a timely manner. This transition
service may include: driving and/or accompanying the patient home,

This section includes information about agencies and programs that
provide parent relief and respite services for adults with ASD. Some
of these respite services can be accessed directly by parents while
others require or prefer the involvement of a case coordinator or
service coordinator.

Recreation and Summer Programs
This section provides information about a variety of recreational and
summer programs available to individuals over 18 years of age with
ASD including social groups, games, sports activities, community
outings, life skills training, arts and hobby activities and summer
camps. These services are provided either in a specialized setting or
within an integrated program with supports.
For summer camps it is important to apply as early as the fall/winter
of the preceding year to ensure space is available. Families should
inquire about the use of Passport funding and/or Ontario Ministry of
Community and Social Services – Income supports (ODSP) funding
to help finance summer camps and recreational programs.

This section provides information on transition planning and post
secondary school information for individuals with ASD and their
families.
Students with ASD planning to attend college or university should
consult with the guidance department in their secondary school
about the admissions process and their individual need for disability
services. It is also important for students with ASD to develop a
transition plan for attaining post secondary school goals.
Colleges and Universities
Ontario colleges and universities are governed by provincial and federal Human Rights legislation that stipulates students with disabilities
are entitled to reasonable academic accommodations and access to
services that enable their participation in all aspects of post-secondary programs. It is up to students with ASD to disclose their disability
in order to request and access support and accommodations from
their respective post-secondary institution. Adults with ASD and/or
families can contact the Disability Services department of a respective Ontario college/university to learn more about the types of supports and services available to students with ASD.

includes information about specialized programs that students
with ASD may consider. These programs are designed for students
with disabilities who would not qualify for a traditional college
or university program but would benefit from a post-secondary
experience that offers courses in life and employment preparation.

Person Directed Planning
To assist with transition planning, consider person directed
planning. Person-directed plans explore who a person is and
what his/her goals are. This includes how s/he participates in
the neighbourhood; her/his likes and dislikes, hobbies, interests,
talents, strengths, fears, concerns; what s/he wants as well as what
supports and connections are needed to achieve these goals. A
variety of planning tools are used to assist with the development of
a written plan which includes an action plan with timelines.
This service is available to anyone with an intellectual disability,
18 years or older. The following agencies offer person directed
planning: Community Living Toronto, Corbrooke, Family Service
Toronto, Geneva Centre, Griffin Centre, JVS Toronto, Montage
Support Services and Operation Springboard.

Description of Service
College-adapted program: Gives students the opportunity to participate
in college-like activities. 2 year program, focuses on academic, job and
life skills. Students graduate with a Certificate of Participation. Students
participate in core courses offered through the CICE program as well
as have an opportunity to participate in modified courses in the college
community. Students participate in practice placements, initially beginning
on campus, and in their final year, off campus. Application to the CICE

Financial Supports: bursaries and awards information, Canada student
loans program policy amendment. The support that is offered to students
with psychiatric disorders is education related, not treatment oriented; it is
not meant to replace mental health support available in the community.
Project ADVANCE: a free (refundable deposit required) 2 week
transition program that prepares students with specific language learning
disabilities for the transition to post-secondary (college or university) while
they are in their final year of high school. Funded by the Ministry of Training,
Colleges, and Universities, Government of Ontario and the program is
offered through the Learning Disabilities Program at York University. For

This section provides information on residential and community
housing options as well as other supports that may be necessary
for independent living such as transportation.
Residential Supports & Planning
Residential support options include a variety of living arrangements
made possible through varying levels of staff support.
How to Apply
Either the DSO or the lead agency involved with the individual can
make a referral on the individual’s behalf to register for residential
supports. Priority status is assigned based on a number of factors
including age and health of caregiver. The decision to put someone
on the priority list is determined by the individual/family’s need
combined with professional judgement of the DSO or lead agency.
Please contact the DSO or the lead agency you’re already involved
with to get more information about residential planning.
Residential Options Include:
Group Homes: different staffing levels to meet the needs of the
individuals living there. Generally staffed at a high level, often with
staffing 24 hours per day.
Supported Individualized Living (SIL): individuals are supported
in smaller settings with staffing support up to 20 hours per week,
depending on individual need. More independence is needed for
supported individualized living.
Supportive Home-Share: support is provided in a family home of
an unrelated caregiver.
Individualized Funding: this is a ministry initiative that happens
infrequently. Agencies are notified and families need to complete
a proposal in partnership with one of 32 agencies to apply for this
funding.
Community Service Coordination Supports
There are two types of service coordination supports for adults
with ASD and their families:
1. Adult Protective Services Worker (APSW)
This is for individuals who live independently or are planning to live
independently within the year.
2. Family Support Workers (FSW)
This is for individuals who are living with their families.
To apply for or access either of these service coordination supports,
contact the DSO or the Lead Agency you are already involved with.

Asperger’s Info Line: A knowledgeable program specialist is available
on weekdays to respond via phone or email to requests for Asperger’s
Syndrome related information and to provide guidance on resources and
services available throughout the province. While Info Line services are
provided free of charge, please note that some agencies and professionals
may bill for their services.
Adult Social Group: The ASO Adult Social Group meets monthly at various
locations in the Greater Toronto Area. The focus of the group is socializing,
establishing relationships and enjoying a variety of recreational activities in
the community. There is no fee for this program, however, the activity itself
is to be paid for by the individual. The cost for these activities is typically
$25 or less. The group participates in a wide variety of activities, for
example; bowling, dining out, going to movies, etc. For more information,

Description of Service
The Ombudsman of Ontario is an officer of the Provincial Legislative
Assembly, independent from government and political parties. The
Ombudsman investigates complaints about services provided by the
Government of Ontario and its organizations. He also conducts systemic
investigations on important issues about government services, which can
affect large numbers of people. The Ombudsman cannot deal with matters
regarding federal or municipal governments (except in the case of closed
meetings), courts or judges. The Ombudsman is a place of last resort when
other approaches to resolve complaints have been tried. The Ombudsman
represents neither the government organization or the person complaining.
