The Healing Hands of St. Francis Health Ministry 609.599.5000 www.stfrancismedical.org 2014 St. Francis Medical Center Foundation and Zoe Graphics “For it is in giving that we receive.” – St. Francis of Assisi Mission & Ministry From Our Pastoral Care Team Devotion…compassion… commitment…caring for all. These are just some of the “charisms” contributed by St. Francis colleagues when our Pastoral Care Team asked them to express what our Medical Center symbolizes and what special gifts our colleagues bring to the workplace. A charism refers to any good gift that flows from God’s love to Pastoral Care Team humans and, overwhelmingly, our colleagues’ expressions were about the caring and kindness shown by the healing hands of St. Francis Medical Center. This feedback was inspiration for some new additions to the chapel. Funding from the Foundation had been designated for pastoral upgrades, and after consulting with Father Carmen and the pastoral care team, it was decided these improvements should reflect the charisms expressed from Medical Center colleagues. A life size statue called the “Whatsoever You Do,” by Timothy Schmalz was placed on the wall outside the chapel. It represents that we stand with and care for all persons. The statue of “Our Lady of Guadalupe,” patroness of South America—symbolizes appreciation for all who contribute traditions, gifts and new life to St. Francis Medical Center. Our Lady of Guadalupe Statue Additional items to express the charism of St. Francis through the eyes of our colleagues include two lovely glass doors at the entrance of the chapel. The symbols etched into the glass represent “Christ the beginning and end of everything,” and “The Eucharist,” which is the center of our spiritual life. New lighting and framed photographs of the Most Reverend David M. O’Connell, C.M. Bishop of Trenton and Pope Francis have also been hung outside the chapel. We, St. Francis Health Ministry and CHE Trinity Health, serve together in the spirit of the Gospel as a compassionate Whatsoever You Do Statue and transforming healing presence within our communities. It is an honor to lead this team of professionals and to know these are the hands that care for you and the Trenton community. and gentle touch.” Christy Stephenson Interim President & Chief Executive Officer, St. Francis Medical Center Excellent Cardiac Care Is Close to Home We Put Our Hearts into Caring For Yours Our cardiac team is pleased to share their experience with Dan Danko—a dad, local business owner and community leader. He and his wife Lori, long-time supporters of the Medical Center, have a son born with a heart condition, and “The Heart Hospital at St. Francis has the expertise and technology to deliver the highest Lori was director at the American Heart Association for many years. But it’s Dan who’s at the heart of this story. level of cardiovascular When Dan first came to see us, his only symptom was a tingling in his left arm, but diabetes and other factors complicated care,” says Dr. Shariff, his situation. After performing cardiac catheterization, cardiologist Dr. Mark Soffer determined that Dan needed immediate Chairman of the surgery, specifically a quintuple coronary artery bypass surgery. This procedure uses arteries and veins from elsewhere in the body, grafting them to the coronary arteries to bypass blocked areas and improve blood supply to the heart muscle. Department of Cardiovascular Surgery. Dan and Lori faced a daunting procedure and important decision for their family, so they met with Dr. Fredric Seinfeld to discuss their concerns. High on their list of questions was why they should choose St. Francis over a large Philadelphia hospital. Dr. Seinfeld reassured them about the Medical Center’s skilled surgeons and the individualized care Dan would receive. Dr. Haji Shariff, Chairman of the Department of Cardiovascular Surgery would perform the surgery along with Dr. Seinfeld and they would follow up with round-the-clock personalized care. With offices right in the hospital, they are available for their patients night and day. It was this level of caring and commitment that convinced them to have Dan’s surgery at St. Francis. The Dankos knew the healing hands of Dr. Shariff and Dr. Seinfeld would be there for them at every step in their journey. “After discussing the procedure with the cardiac surgeons, we were put at ease,” says Lori. “We knew St. Francis was the best place to be for heart surgery.” Dr. Haji Shariff The surgery was performed last August, Dan recovered quickly and patient, family and doctors are all pleased with the outcome. “The Heart Hospital at St. Francis Medical Center has the expertise and technology to deliver the highest level of cardiovascular care,” says Dr. Shariff. “Our patient-focused approach ensures that when it comes to healing you or your loved ones, nothing is routine and every patient is considered unique,” says Dr. Seinfeld. The healing hands of St. Francis Health Ministry Dan was very pleased with the nursing staff and their commitment to their jobs and to their patients. “The care that I received both before and after my surgery was outstanding,” says Dan. Dr. Fredric Seinfeld State-of-the-Art Heart Care New Hybrid Lab Expands The Heart Hospital As the first and only facility in the region to feature a hybrid lab, St. Francis Medical Center provides patients with better treatment and advanced technologies in the safest possible environment. The new multi-million dollar facility is the best of both worlds, combining the imaging capabilities of a cardiac catheterization lab with the sterile environment of an advanced operating room. This reduces the need for multiple surgeries, minimizes recovery time and improves patient outcomes. Dr. Jay K. Patel Highly regarded cardiologists, Dr. Jay K. Patel and Dr. John Caplan of Hamilton Cardiology and Dr. David Drucker and Dr. Eran Zacks of Mercer Bucks Cardiology, currently perform procedures in the new hybrid lab, supported by outstanding non-invasive cardiologists. “The hybrid OR enables physicians to do procedures that may require minimally invasive and open-surgery techniques simultaneously,” says Dr. Patel, Medical Director of the St. Francis Medical Center Cardiac Catheterization and Interventional Department. Dr. Patel was an integral part of planning and establishing the new hybrid lab. “As demand for more complex interventional procedures increases, we needed this solution to ensure more efficient Dr. John Caplan performance and deliver broader imaging capabilities,” adds Dr. Drucker, a specialist in the minimally invasive treatment of peripheral artery disease, thoracic and abdominal aortic pathology and structural heart disease. In the near future, the hybrid lab will also offer non-surgical treatment of aortic stenosis, also known as TAVR. Equipped with cutting-edge imaging via robotic technology with unique applications in angiography, cardiology and surgical procedures, the new hybrid lab is the latest investment in our cardiovascular surgical program. Over the past several years, the Medical Center has invested nearly $10 million in facility and equipment improvements to support the program’s growth. As identified by the most recent Community Health Needs Assessment, heart disease remains one of the major health Dr. David Drucker concerns in the region. The addition of the hybrid lab allows St. Francis to handle high-risk cardiac procedures in the safest environment possible, which will improve quality of life. Patients can receive diagnostic and surgical procedures at one convenient location. The Heart Hospital is all about keeping your heart healthy and caring for it when it’s not. You can count on the healing hands at St. Francis to deliver caring, cost-effective and patient-focused services at the only The healing hands of St. Francis Health Ministry state-recognized Regional Cardiac Center in Mercer County. Dr. Eran Zacks Emergency Surgery Saves A Life A heart-warming success for the St. Francis Cardiovascular Team One memorable day last November, a young Latino man arrived at the emergency room with severe chest pain. He was just 28 years old, worked as a landscaper, and had no history of heart problems. On this particular day, he’d drunk a popular energy drink before doing the normal heavy lifting his job required. But the rest of his day turned out to be anything but normal. Severe chest pain occurred after eating lunch and resulted in a trip to the emergency room. At the ER, the patient had no pulse to his extremities. A CT scan showed a tear in the wall of the aorta, the body’s largest blood vessel, and he had extreme “Over the past 25 years I have seen the cardiac program grow from a hospital performing pacemaker insertions and thoracic surgery to a hospital now able pressure on his heart. He was experiencing a medical emergency that can lead to death so rapidly that many people don’t meet the cardiovascular make it to the hospital in time. Because he was at The Heart Hospital at St. Francis Medical Center, the only full-service needs of Mercer County cardiovascular center in Mercer County, he was diagnosed and able to have surgery immediately which saved his life. Cardiologist Dr. Dan Sodano quickly consulted Dr. Fredric Seinfeld who speaks fluent Spanish, and took the patient straight to the OR, where heart pressure was relieved and his pulses returned. When the patient started bleeding from the torn aorta his condition went from bad to worse. According to Dr. Haji Shariff the patient was chilled to bring his temperature and pulse for more complex open heart procedures,” says Dr. Anil Deshpande. down, his aorta was clamped above and below the heart, and he received transfusions to keep his organs functioning. Dr. Haji Shariff, Chairman of the Department of Cardiovascular Surgery, Dr. Fredric Seinfeld and the cardiovascular team worked skillfully to rebuild the patient’s aorta, re-attach his coronaries and complete the surgery successfully. The next morning the patient was taken off the ventilator, and asking for breakfast. This medical emergency was caused by a congenital heart condition that the patient was unaware of, combined with the physical strain of his work, resulting in a dangerous situation for his heart. Today the patient is doing well. He’s back at work and receiving regular checkups at SFMC. “I thank God for the cardiovascular Dr. Anil Deshpande team at The Heart Hospital at St. Francis Medical Center,” he says. “Without them I would not be here today.” Patient’s name was not used to protect his privacy. The healing hands of St. Francis Health Ministry Cardiovascular Team Dr. Haji Shariff LIFE is Good For Seniors in Our Community Thanks to Living Independently For Elders Since 2009, LIFE St. Francis has been helping seniors stay independent and remain at home. The popular program is a wonderful alternative to nursing home care, providing an array of medical services and opportunities to join a host of activities in the warm, supportive Center. Optimizing Health. This is the goal in our busy Health and Wellness Center, where geriatricians provide everything from routine checkups to care for someone ill with the flu, plus initial assessments for seniors interested in joining the program. Physical Therapy Staying Active. LIFE’s recreational therapy helps seniors stay physically flexible and mentally sharp with word games, bingo, art projects, cooking demos and entertainment—always a favorite when the music gets everyone up and dancing. Getting Out and About. With ten vans, LIFE can transport participants to and from their homes to the Center and medical appointments. Special outings include trips to the store, park, farm market and other local spots of interest. Making Connections. Sharing activities and meals together sets the stage for new friendships. Mr. N. and Mr. O. became instant friends at LIFE, a big benefit for Mr. O. who previously spent a lot of time alone in his apartment. Now both men look forward to sharing time and reminiscing at LIFE, where others call them the President and Vice President of the Center! Living Strong, Staying Safe. LIFE participants receive the physical therapy and occupational therapy they need, whether Home Care it’s daily exercise classes or specialized therapy for stroke recovery. The program also provides equipment and home modifications, such as walkers, grab bars and hospital beds, to keep participants safe. Delivering Home Care. LIFE nurses make home visits, where they help with medication management, wound care and more. Home health aides are also available to provide personal care, meal preparation and light housekeeping. Meeting Every Need. We take a holistic approach to participants’ quality of life, providing chaplains to address spiritual needs, dieticians to help with healthful eating and social workers to support families’ needs. Providing Peace of Mind. This may be the biggest benefit of all for LIFE participants and their families. And with LIFE, The healing hands of St. Francis Health Ministry cost is not a barrier to getting the care that’s needed. If you know a senior who would benefit from these services, call LIFE at 609.599.5433. Recreational Therapy Community Outreach and Education Improving healthcare, inspiring healthy living “Taking healthcare and education beyond the hospital walls and into the community is not new to St. Francis Medical Center. Living the mission started with the Sisters of St. Francis who founded the hospital 140 years ago. What is new is how community outreach and education are delivered,” said Norma Monaghan, CFRE, President of the SFMC Foundation and Senior Vice President of Development, who leads the Medical Center’s Community Outreach and Education team. SFMC was a leader in forming the Trenton Health Team, a group that came together because they agreed the people of Trenton deserve better healthcare. This collaboration includes St. Francis Medical Center, Capital Health, Henry J. Austin and the City Health Department. Partnering with 29 community and social service agencies, they performed a comprehensive Community Health Needs Assessment. “This assessment identified Trenton’s top health concerns,” says Judy DiBartolo, Trenton Health Team Liaison for St. Francis. “These issues drive the Community Health Improvement Plan: obesity/healthy lifestyles, substance abuse/behavioral health, safety and crime, chronic disease (heart disease, diabetes, hypertension and cancer), health literacy and disparities.” SFMC Community Outreach and Education offers programs in the community and the hospital to specifically address these concerns. The focus is not on glitzy ads, but on actually working in the community through individuals on the Community Outreach Education (COR-E) team. “The COR-E team holds eight to ten external/internal programs a month with plans to “Taking healthcare and education beyond the walls and into the community is not new to St. Francis. Living the mission started with the Sisters of St. Francis who founded the hospital 140 years ago. What is new is how community outreach and education are delivered.” significantly increase that number,” explains J. Delores Baker, Community Liaison. An important part of this new approach is the Faith Based Community Nursing (FCN) program, comprised of licensed, registered nurses who practice whole-person health. “The goal is to establish regular programs in every area church, as well as to identify nurses within the church community to become Faith Based Community Nurses,” says Mary Jo Abbondanza, RN, BS, FCN, who is the Faith Based Community Nurse Coordinator at SFMC. Working with area churches, she has established regular programs in 15 churches with four established Faith Based Community Nurses. Another unique aspect of COR-E is the direct inclusion of our Mission/Ministry department. Russ Hansel, Executive Director of Mission/Ministry, participates in the programs and coordinates and documents the community benefit. “Mission is at the center of everything we do at St. Francis,” says Russ Hansel. “These wonderful individuals bring The healing hands of St. Francis Health Ministry energy and expertise,” says Norman Monaghan. “They are committed to providing the very best for our community.” The St. Francis Medical Center Foundation helps support the Community Outreach and Education program. St. Francis Care Van St. Francis Medical Center Clinics All About Keeping Patients’ Health On Track Every day, our clinics give Trenton-area residents access to a wide array of services and the convenience of a neighborhood medical practice. The primary care Medical Clinic is run by Dr. Herb Conaway, Jr. and his staff, with assistance from graduate medical residents who are overseen by faculty members such as Dr. J. Michael Smith. After a history of substance abuse, he made the choice to change his life and has been clean for about six years are not needed. now. Although he felt he was doing well after this decision, he suddenly found himself in the Medical Center’s Emergency Room with a pulmonary embolism. Agenor was very sick and required four weeks of hospitalization. During this time, he was diagnosed with diabetes, obesity and hypertension. In addition, the emboli damaged his lungs, resulting in COPD (chronic obstructive pulmonary disease), which makes breathing difficult and requires the continuous use of oxygen. Once released from the hospital, our doctors recommended he follow up in our Medical Clinic and Heart Failure Clinic. Knowing his health is precious, he made his appointments and committed to keeping them. At the Heart Failure Clinic he has found support from caring staff like Debbie Farmer, RN. “They keep me on the right track,” he says, “and answer all of my questions—even those that come up between visits.” Also, Agenor’s diabetes is a challenge so his medications sometimes require “tweaking.” Clinic staff have helped him navigate the healthcare system and have secured equipment, Dr. Herb Conaway, Jr. portable oxygen, a hospital bed for his home, and air-conditioning for his room. Agenor has grown more self-assured in his knowledge of his disease and community resources. He’s now able to navigate for himself and asks for help only when he hits a roadblock. For those seeking to improve their health as he has, he has this advice: “Be patient and treat others like you want to be treated. Also, listen—and don’t be afraid to ask questions.” Agenor demonstrates the success of a patient who accepts responsibility for his well-being and knows when to reach The healing hands of St. Francis Health Ministry out for help!
