Beginning February 1, 2004 through January 31, 2005, qualified
groups who purchase or renew any Blue Shield medical plan can
take advantage of electronic enrollment at no additional charge,
subject to the conditions below.
Through this arrangement with Benetrac, Blue Shield will cover the initial
set-up fee and monthly service fees for all your employees who enroll
through the service – including those who enroll with an alternative carrier
– allowing you to utilize the system to enroll all your employees at no
additional cost.

Unlike many carrier-based systems that administer only their own plans,
BeneTrac's electronic enrollment offering helps you manage multiple benefit
plans from a variety of carriers, all from a single, consistent and secure
point-of-entry. This cross-carrier capability enables you to save time
and resources while enhancing your ability to make comprehensive
comparisons of enrollment data and plan activities between carriers.

By compiling benefit data in a centralized, digital format, electronic enrollment offers customized management reporting capabilities that can help
you gain strategic insights on plan utilization metrics, facilitate future plan
design decisions and identify potential cost efficiencies. On-demand
accounting tools are also included to help simplify your data and make
bill reconciliation more transparent across carriers. Reports can be
exported and viewed using either Microsoft Access or Excel formats.

authority for all changes.
• Using a secure online connection, your data is electronically
transmitted to Blue Shield or other carriers, if applicable.

How to qualify for electronic enrollment
Terms and conditions
All qualified groups can take advantage of electronic
enrollment as part of their plan package, regardless
of their Blue Shield medical plan selection or current
relationship with BeneTrac (see chart).
• Through this offer, Blue Shield will provide a subsidy to
BeneTrac of $3.25 per employee per month on behalf of the
group for each eligible employee who enrolls using BeneTrac
services, regardless of enrolling in a Blue Shield medical plan
or a plan offered by another carrier. The $3.25 subsidy applies
only to BeneTrac’s basic system package services, available to
Blue Shield groups at a preferred rate of $3.25 per employee
per month. In addition to the subsidy, Blue Shield also will
cover the initial one-time set-up fee, equivalent to $3.25 per
employee multiplied by the total number of employees who
enroll through the BeneTrac system.

To qualify, groups must meet the following criteria:
• Have 51 to 1,000 employees
• Purchase or renew a Blue Shield medical plan
between February 1, 2004 and January 31, 2005
• Have at least 50 percent of employees enrolled in a
Blue Shield medical plan with a minimum of 25 employees enrolled through BeneTrac's enrollment system
For complete information on how to qualify for electronic enrollment with your group’s Blue Shield medical
plan, see the adjacent terms and conditions.
• BeneTrac preferred rates apply to basic system package
services only. Additional services and features may cost extra
and are the financial obligation of the group.
• To qualify, groups must enroll or renew in a Blue Shield medical
plan between February 1, 2004 and January 31, 2005.
• To be eligible to continue to receive the subsidy, groups must
remain a customer of Blue Shield, use the BeneTrac services
and continue to be a qualified group.
• Eligibility for groups with more than 1,000 employees will
be determined by Blue Shield on a case-by-case basis.
• Groups currently receiving a Blue Shield subsidy for BeneTrac
services are not eligible for the $3.25 subsidy prior to completing
their existing 12-month contract with BeneTrac. Following
the fulfillment of this contract, only groups renewing prior to
January 31, 2005 are eligible to receive this subsidy upon their
renewal date with Blue Shield.

• Electronic enrollment services are made available through a
contract between BeneTrac and the employer group and are
not covered services under any Blue Shield of California medical
plan or Blue Shield of California Life & Health Insurance
Company product. BeneTrac manages its own enrollment
system and contracts directly with the employer group. None
of the terms and conditions of Blue Shield’s health plans apply.
• Blue Shield may modify or terminate this program at any time.
In the event of termination, all subsidy payments will cease as
of the date of termination of this program. In such an event,
the group may continue to receive services at their own expense.
• Blue Shield accepts electronic enrollment transmissions from
other vendors on a case-by-case basis. If you are currently using
electronic enrollment services from a vendor other than BeneTrac,
speak with your Blue Shield representative. Blue Shield subsidy
does not apply to vendors other than BeneTrac
