Working with our patients, physicians, employees, volunteers and community, Bristol Hospital and Health Care Group will utilize our core values to achieve the vision which: • Creates a culture of safety, quality and service that is embraced as an individual and team responsibility • Ensures a user-friendly continuum modeled on providing patient-centered care and services • Continually assesses and promotes new services and technology • Serves as the responsible steward and advocate for the health of our community 2/ANNUAL REPORT 2015 . ANNUAL REPORT 2015/3 Every year in this annual report we say that this is the most exciting era in the history of Bristol Hospital. This year is no different as we share a snapshot of all of our accomplishments. One of the many highlights from this year was the American Nurses Credentialing Center® Magnet Recognition Program®, granting Bristol Hospital Magnet recognition. Achieving Magnet status is proof of our relentless pursuit of excellence in nursing and all areas of the hospital. More importantly, it validates our culture of safety, quality and service. Immediately following our Magnet announcement, Bristol Hospital was awarded Hip and Knee Joint Replacement Certification from The Joint Commission. We then opened our new, 8,000-squarefoot Center for Orthopedic and Spine Health. Our Magnet Recognition and the Joint Commission certification are two additions to a growing list of prestigious awards and recognitions. Our commitment to the health care needs of the Greater Bristol Community resulted in the addition of 29 new providers—19 of whom are members of the Bristol Hospital Multi-Specialty Group. These include primary care physicians as well as specialists in behavioral health, cardiology, hematology and oncology, obstetrics and gynecology, and rheumatology. We also continue to build upon relationships with other partners in the state including Yale-New Haven Hospital, and Saint Francis Hospital and Medical Center. Additionally, Bristol Hospital continues to keep pace with the most advanced technology. Earlier this year, we started our robotic surgery program by adding the da Vinci Xi Surgical System which results in less pain and faster recovery times for our patients. The da Vinci Xi is the latest equipment of its kind and Bristol Hospital is proud to be the only hospital in Hartford County to have this version of the surgical robot. Recently our Beekley Center for Breast Health and Wellness added a new digital tomosynthesis unit which creates a three dimensional picture of the breast using x-rays. The tomosynthesis unit comes as welcome news to our patients since the 3D mammogram provides better quality and improved accuracy, and therefore more peace of mind. None of this growth and success is possible without the hard work and dedication of our employees, medical staff, donors and volunteers. Our vision to be recognized as the best community hospital in Connecticut continues. Our future is strong and our heart as a community hospital will not change, it is unwavering. Thank you for being “Everyday Extraordinary.” Message From Our Leadership Kurt A. Barwis, FACHE President and Chief Executive Officer John J. We continue to undertake a number of initiatives designed to enhance the safety and quality of care we provide. In 2013, Bristol Hospital began a journey to establish a High Reliability Organization which is a culture and methodology that radically reduces systems failures and catastrophic events. This includes the hospital’s Journey to Zero Harm. Since then, our staff’s dedication to this practice of safety continues to result in a decreased number of serious safety events. In 2015, there was a 97 percent decrease in serious safety events, and a more than 100 percent improvement in the average days between serious safety events. 80.4 78.9 82.2 Serious Safety Event Rate (SSER) 87.2 85.7 Rolling 12-month average of serious safety events per 10,000 adjusted patient days 87.9 73 70 68 50 Serious Safety Event Rate (SSER) 60 50 40 30 26 20 19 10 0 Mean Rank 10 2009‐10 2010‐11 2011‐12 2012‐13 2013‐14 2014‐15 80.4 19 78.9 10 82.2 26 85.7 50 87.2 68 87.9 73 90 85.9 84.7 84.9 86.6 85.2 70 4.20 2 2.94 2.84 3.08 2.64 Calendar Year 100 84.5 3 5.63 2.47 2.32 2.28 2.16 1 1.63 1.03 0.38 0.66 0.66 0.51 0.62 0.57 0.53 0.53 0.53 0.40 0.27 0.26 0.26 0 Journey to Zero Harm Inpatient OverallPressGaney MeanScore byFiscalYear 80 6.00 5.90 5.80 5.70 5.60 5.50 5.40 5.30 5.20 5.10 5.00 4.90 4.80 4.70 4.60 4.50 4.40 4.30 4.20 4.10 4.00 3.90 3.80 3.70 3.60 3.50 3.40 3.30 3.20 3.10 3.00 2.90 2.80 2.70 2.60 2.50 2.40 2.30 2.20 2.10 2.00 1.90 1.80 1.70 1.60 1.50 1.40 1.30 1.20 1.10 1.00 0.90 0.80 0.70 0.60 0.50 0.40 0.30 0.20 0.10 0.00 60 # of Serious Safety Events 90 80 Journey to Zero Harm EmergencyCareCenter(ED) OverallPressGaney MeanScore byFiscalYear # of Serious Safety Events # of Precursor Safety Events # of Near Miss Safety Events 2012 45 NA NA 2013 25 332 42 2014 4 301 63 2015 2 308 76 50 41.0 40 Calendar Year 33.0 30 26.0 28.0 21.0 20 23.0 10 0 Mean Rank 2009‐10 2010‐11 2011‐12 2012‐13 2013‐14 2014‐15 84.5 26.0 84.9 28.0 84.7 21.0 85.2 23.0 85.9 33.0 86.6 41.0 8/ANNUAL REPORT 2015 Goal 87.1 Average Days between Serious Safety Events 2013 14.6 2014 91.3 2015 182.5 ANNUAL REPORT 2015/9 |1 Financial Statements BRISTOL HOSPITAL, INC. 2015 2014 2013 2012 Operating Revenues: Net Patient Service Revenues $131,717,516 130,360,493 $135,540,204 $141,984,205 Other Operating Revenues 4,301,391 5,173,982 4,242,269 3,838,007 2015 BRISTOL HOSPITAL, INC. 2014 2012 2013 139,378,211 146,285,596 135,959,785 135,534,475 Operating Expenses: Medical/Surgical Admissions 4,995 5,079 5,319 5,251 Births 566 649 569 604 Medical/Surgical Inpatient Days 19,421 20,904 22,491 21,653 203,043 204,370 219,505 Outpatient Visits 216,931 6,498 6,688 6,700 Surgical and Endoscopy Procedures 6,822 Emergency Room Visits 39,641 38,812 38,530 38,557 4,007,799 6,470,291 4,909,425 2,212,274 Bad debt expense, net 6,614,415 6,320,576 6,363,743 7,176,664 Depreciation and amortization 1,641,972 1 ,412,468 1,421,575 1,267,462 Interest 134,486,303 Total expenses 134,613,099 138,845,547 145,236,748 Gain from operations $1,346,686 $1,048,172 $532,664 $1,048,848 Non-operating income $844,070 $1,253,154 $795,166 $1,263,862 Excess of revenues over (under) expenses $2,190,756 $2,301,326 $1,327,830 $2,312,710 2014 2015 BRISTOL HOSPITAL MULTI-SPECIALTY GROUP, INC. The Magnet status recognizes health care organizations that provide the very best in nursing care and uphold the tradition of professional nursing practice. Currently, Bristol Hospital is the only hospital in Hartford County to hold Magnet recognition and is only one of four Magnet hospitals in Connecticut. About 7 percent of the nation’s hospitals have met the criteria to earn this prestigious designation for providing the highest quality patient care, teamwork, professionalism and innovation in nursing practice. 12/ANNUAL REPORT 2015 ANNUAL REPORT 2015/13 Magnet Recognition 14/ANNUAL REPORT 2015 The Magnet announcement took place July 14 at Bristol Hospital when Donna Havens, PhD, RN, FAAN, chair of the Commission for the Magnet Recognition Program®, spoke via a conference call at an event that was attended by hospital leadership as well as dozens of employees, board members and local officials.
