In the meantime... Your practice
is eventually faced with the bill while most of the product

clinics, catalog companies, big box stores and human

Secnidazole - Alternative Giardia Therapeutics
Sec…what? Based on numerous requests from your colleagues, Roadrunner Pharmacy was able to procure
secnidazole for, according to these practitioners, giardia in cats. Similar in structure to other nitroimidazoles
like metronidazole and tinidazole, secnidazole is rapidly and
completely absorbed after oral administration and has a longer
half-life (~17-29 hours) compared to other drugs in this class.
While efficacy between secnidazole and therapeutic courses
of other drugs is similar (in humans, SEC 100%, METRO 96%),
Secnidazole has the advantage of being a single dose in cats; here
the efficacy varies dramatically (in humans, SEC 100%, METRO 3654%). In cats treated with one dose of secnidazole, 100% cure rates
against G. duodenalis were reported. Side effects were minimal,
with minor blood parameter changes noted, primarily elevation in
liver enzymes, although no clinical signs of toxicity were reported.
Clearly, the advantage is as an efficacious SINGLE dose in cats.
While no reports exist of secnidazole’s efficacy against Tritrichomonas spp, the sensitivity of this protozoan
to other benzimidazoles suggests it’s likely sensitive to this drug as well. Also unreported is the efficacy of
secnidazole in treating giardia in dogs. Anecdotal reports exist using a three day regimen with success but
clinicians have other equally easy-to-use drugs with demonstrated success.
Practitioners who have called us about this drug, suggest its therapeutic position may be best in giardia in
cats, especially kittens, as a single dose and possibly as an alternative therapeutic agent in canine giardia
who have failed conventional drugs. Like most in this class, secnidazole is a foul tasting chemical that we are
masking in almond oil and flavoring with vanilla butternut. When ordered as 140 mg/ml, the dosing (at 30mg/
kg) is 0.1ml per pound of animal ONCE. Some shelters and rescues have had impressive success coupling
secnidazole with ponazuril to treat kittens with diarrhea upon intake at their facilities. Please call one of our

All Eyes on Compounding…No Substitute for Quality!
While you can have a great phone interaction with our pharmacy and our amazing shipping accelerates the product
to you quickly, THE most important facet should be—and for Roadrunner, it IS, the quality of the compounded
medication.
Although quality should be implied, the misadventures at the New England Compounding Center illustrate just
how wrong that implication can be. As we’ve seen, standards and their interpretations vary. Once again, it comes
down to practitioners making informed decisions. Ask the hard questions of whoever is doing your compounding!
Not surprisingly, a knee-jerk response to the NECC problems is probably coming to your state soon.
Additionally, the FDA is proposing sweeping changes to the compounding arena that will have significant impacts
on veterinary practices. For example, human practitioners do not routinely dispense—veterinarians do and should
have the option to dispense small amounts of acute medication, at least until the owner can procure sufficient
quantities. Most states do not address this, putting you at risk for misinterpretation. The office stock needs of
human providers are met, more or less, by Big Pharma companies. Not so much for the unique stock needs of
veterinarians. And, as fewer and fewer manufacturers make like items, more and more back-orders and out-ofstocks are being noted.
If you embraced compounding pharmacy, you did not run out of diazepam, atropine, dexmedetomidine,
aminophylline, KCl and many more critical items last year. Make no mistake, there is a valid place for
compounding pharmacy in the veterinary world, but practitioners must have input to make informed decisions.
You have every right to know how items are made, how are they tested, the potency of the ingredients used and
how end-of-use dates are assigned. Relegating this task to a state board or independent organization does not
confer the confidence you need to make impactful decisions. Recent history is replete with examples of such
misplaced confidence. Ask the hard questions!
The FDA and your state regulatory boards will soon be asking for your input regarding the compounding issue.
Be sure your thoughts are heard regarding compounding for office stock items, the use of independently tested bulk
powders, interstate sales and your ability to dispense small amounts of acute medications. We don’t need more
laws…we need clear definitions of existing ones.
