KRMC is a 235-bed not-for-profit hospital located in northwestern Arizona and serves a
population of about 80,000 people. The hospital employs over 1,500 employees, 250 volunteers,
and 190 physicians/allied health professionals. KRMC provides a full range of health and
wellness services, including a wellness and fitness center, primary and specialty care physician
practices, advanced medical programs in cardiology, cancer, rehabilitation, and home health and
hospice care. KRMC also serves as a teaching hospital in affiliation with Midwestern University
to train doctors specializing in family practice and emergency medicine. KRMC CEO, Brian
Turney, has been in the community for 27 years and has spent 17 of those years as CEO. As of
2011, KRMC became an official member of the Mayo Clinic Care Network, which allows access
to Mayo Clinic expertise and resources.

The Need
Kingman currently has one urologist servicing the area, Dr. Melissa Brown. She came to town
in 2008 to fill the need in the community due to the retirement of Dr. Waldemar Klimach. Dr.
Brown was previously the sole urologist in a multi-specialty group at the Fairmont Medical
Center, which is part of the Mayo Health System in Fairmont, Minnesota; she also served as
Chief of Surgery at Fairmont. There is currently a three-month waiting period to see Dr. Brown
in clinic and the goal is to reduce that wait to two weeks.
Dr. Brown is doing bread and butter urology, seeing about 28-32 patients a day in clinic; she is
not very aggressive in the OR, so a lot of the cases get referred out to Phoenix and Las Vegas.
On Monday, Tuesday and Thursday, Dr. Brown comes in at 8am and works until noon, takes a
lunch break, then heads back to the office at 12:45 and the last patient is schedule at 3:45pm. On
Wednesdays and every other Friday, she has scheduled OR time from 7:30am to noon, then back
to the office at 12:45 and sees her last patient at 3:45pm.
We are looking for an additional urologist to become an employee of KRMC. This individual
should be hard-working and willing to build a program that will best suit the need of the hospital
and the community. This is a general urology position, but has plenty of opportunity to subspecialize; there is especially a need for someone who has an interest in prostate work. The
intensivists, radiologists, radiation oncologist, COO, CFO, general surgeon and other staff all
agree that the market is wide open. Dr. Van Tuyl (radiation oncology) estimates that a urologist
can build a practice to be about 70% outpatient and 30% inpatient.
We currently have locum tenens coverage through Dr. Sweeney; his assignment ends on October
8th of this year and there will be another locum tenens provider joining in September of this year.
Dr. Sweeney has an OR block time every Monday morning, and every other Friday morning. He
has been a bit more aggressive in the OR and has done procedures that the hospital has not seen
65 Enterprise Suite 250 * Aliso Viejo, CA 92656 (800) 741‐7629 * (714) 549‐8869 fax * www.pacificcompanies.com Prepared by Ernie Campos (800) 741‐7629 [email protected] done in four years. Dr. Sweeney basically has a full-time practice, which would be the case for
our new urologist on day one.
Call Schedule and OR Time
There is a contractual obligation to take at least five days of call, for which there is no additional
pay; you will receive a stipend once you reach seven or more days of call. Responsibilities
include ER call and being available for consults. There is currently no weekend coverage and
very limited in-patient coverage.
When a patient presents in the ED, you will get called for a consult; if you determine the case to
be serious and requires immediate attention, you will come to the ED and attend to the patient. If
it is then determined that the patient needs immediate surgery, you can call Bridgette to try and
schedule for an OR. She will let you know if there is an OR available, and if not then she will
try and schedule as soon as possible depending on availability. Usually, if a surgery is not
scheduled three days before the block schedule, then permission is required. The OB/GYN is
typically scheduled in the OR on Wednesdays, but if there are no additional surgeries, the slots
will be given away where needed. KRMC has five ORs and there is a dedicated after-hours oncall team that will be ready to go at all times.
Stacy McDaniel, Director of Surgical Specialties, makes herself very available and is willing to
work with surgeons to work out the best OR schedule.
First Case of the Day
The first case of the day will present at 6:15am to check-in, get dressed, start IVs, and have the
anesthesiologist consult; the patient will be ready to go under by 7:30am. There is one
anesthesiologist scheduled at all times, and anesthesiologists’ schedules are made to complement
the surgery schedule.
Pre-Op Patients
Patients will be contacted prior to the surgery for a work-up; the hospital is currently working on
a pre-op clinic that is projected to be up and running by August of 2013. All operative cases will
utilize the clinic, except OB/GYN and Cardiology since they do a lot of their own pre-ops.
Competition
The competition is somewhat limited; the closest urologists are situated in Bull Head City and
Lake Havasu. Bull Head City is about 30-40 minutes west and Lake Havasu is about an hour
southwest.

General Surgery & Gastroenterology
There are four general surgeons in the community of Kingman and they are very busy; each of
them spends a fair share of their time doing GI procedures. There is one GI in town, Dr. Carol
Lee-Collins, but she is new to the area and is in the process of developing her practice; KRMC is
also in the process of building new GI suites. Dr. Lee-Collins joined KRMC on July 10, 2013.
The general surgeons are very supportive and would like the addition of a urologist who can
make themselves more available. Our general surgeons share a call schedule of 1:4, which
includes seven straight days of call; five weekdays and a weekend. If one goes on vacation or
something comes up, they have a very good relationship and can easily alter the call schedule to
best accommodate everyone.

Emergency Department
The ED is covered by a private group in town. On average, our patients spend two hours less
in the waiting room than the four and half hour Arizona average. The number of daily visits to
the ED continues to grow and patient-satisfaction scores consistently rank the quality of our
care among the top 20% of hospital EDs nationwide. The ED is staffed 24-hours-a-day with
skilled and experienced emergency physicians, physician assistants and nurses; all of our
emergency healthcare professionals are certified in advanced ACLS and in PALS.
The ED sees about 45,000 visits per year, serving an estimated population of 80,000 in
northwestern Arizona, which includes the city of Kingman and many outlying small
communities. Due to location, the ED also serves a significant number of tourists and travelers.

ICU
The intensivist program was originally started by Dr. Matheny, who came to Kingman about 10
years ago. He is originally from Arizona and always knew that he wanted to come back, so he
started researching opportunities near Phoenix. He eventually came across the position in
Kingman and decided to interview. He was made aware that he would have the opportunity to
build the program his way with full support from the hospital, which is what sealed the deal.
After a year of practicing in Kingman, Dr. Matheny realized that he was going to need help so he
recruited his colleague, Dr. Nguyen; Dr. Nguyen came to Kingman straight out of fellowship and
has been here for nearly seven years. Dr. Matheny is a member of the KHI executive board.
65 Enterprise Suite 250 * Aliso Viejo, CA 92656 (800) 741‐7629 * (714) 549‐8869 fax * www.pacificcompanies.com Prepared by Ernie Campos (800) 741‐7629 [email protected] We have a 17-bed ICU, which has officially become a closed ICU. Although it is closed, Dr.
Capriles (general surgery) stated that the intensivists will allow you to check on your patients if
necessary. When a patient presents in the ER, the emergency physician will admit if the patient
is not in need of CCM; if the emergency physician determines that the patient needs CCM then
the intensivist will admit the patient.
The ICU is run by four intensivists, one of whom is a board certified Anesthesiologist/CCM, and
is also working with cardiothoracic surgery. They have a very friendly, collegial work
environment and have developed a schedule that satisfies everyone in their group. They work a
7-on/7-off schedule that usually goes from Thursday-Thursday. If you plan on scheduling an
inpatient surgery, our intensivists are more than willing to manage the patient. Dr. Sweeney has
grown accustom to planning ahead and contacting our intensivists to give them a heads-up,
which they really appreciate.
ICU Call Schedule
Provider #1:
 Designated on-call provider
 Covers the ICU for seven straight days
 In-house from 7am-4pm, but available all day
Provider #2:
 Designated to the office Monday-Thursday
 Gets a three-day weekend and no call coverage
Providers #3:
 Sees patients at the office in the morning, works Monday-Thursday
 Sees patients outside of the ICU at the hospital
 No call coverage
If provider #1 gets overwhelmed, his colleagues have no problem leaving the clinic for a couple
of hours to assists him in the ICU. Once the fourth provider joins the group full-time, he will be
added to the rotations.

Pathology
KRMC has hired two new Mayo graduates to join the pathology team later this summer; Dr.
Swapp and Dr. Bedke.
Radiology
Originally came to KRMC as a group and were able to set-up the practice their way. KRMC has
built-in interventional suites and has access to telerural medicine with Mayo. According to the
physicians we interviewed, radiology is very good at getting back with the images; usually takes
about four hours. The group consists of seven radiologists.

65 Enterprise Suite 250 * Aliso Viejo, CA 92656 (800) 741‐7629 * (714) 549‐8869 fax * www.pacificcompanies.com Prepared by Ernie Campos (800) 741‐7629 [email protected] Dr. Van Tuyl came to Kingman just over a year ago and has been able to develop the program
his way. The hospital has made a big commitment to oncology; he has seen about 232 new
cancer patients during his first year. Dr. Van Tuyl has 23-24 patients under treatment, and with
the addition of another urologist he could expect to increase those numbers into the low to mid
30s. He is averaging eight consults a week, but sees that number potentially increasing to 10-12
with more referrals from the urology service. One third of his practice should be prostate cancer
patients, but he rarely sees referrals from Dr. Brown. Dr. Brown’s patients are aware of the risks
that come from radiation oncology, so they tend to prefer the surgical option or to simply
monitor the cancer.
Dr. Van Tuyl has a strong relationship with his colleagues at the Mayo Clinic; he has weekly
video conferences where they present and collaborate on unique cases. Dr. Van Tuyl stated that
he has seen more cancer in the last year than he had in the previous eight years of practice. He
has come across very unique tumors, which he had never seen before in his career. In the past,
he has brought guest speakers to visit KRMC; one of those being a surgical oncologist, and has
an expert on Leukemia scheduled to speak at the hospital.
We do not have a prostate seed implant program, but Dr. Van Tuyl has built two programs in the
past and is willing to build a third with the support from our new urologist. KRMC is the in the
process of hiring a physicist, which will be followed by a dosimetrist. We will be upgrading to a
new Elkta Linear Accelerator in 2014.
“If you have the desire and energy to build a program, it’s pretty easy to do so.”
~ Dr. Michael Van Tuyl
Conversation with the Chief Operating Office, Ryan Kennedy
Mr. Kennedy became COO six and a half years ago; we interviewed him regarding the hospital’s
visions and the need to improve the urology service line.
“The board has recently adopted the Physician Leadership Committee (PLC) as a board
committee. We are looking for a urologist to develop something very special. We need someone
who is value-driven and willing to take on a leadership role that will improve the quality of
care. Urology plays a huge role in the diagnostic services and radiation oncology is currently
disappointed with the lack of support from urology…This individual will have every
opportunity to have us build a program around him/her.” ~ Ryan Kennedy, COO
KRMC is the only not-for-profit hospital in the county and has become the regional player in
northwestern Arizona. Financially, KRMC is debt $10M on a short-term finance due to the
acquisition of Hualapai Mountain Medical Center; the debt will be paid off in three years. All
projects are funded through operations and reserved and KRMC has 115 days cash on hand.
Hualapai Mountain Medical Center
Hualapai Mountain Medical Center was purchased by KRMC in 2011. The medical center was
initially opened by a few physicians out of Phoenix, but after two years they filed for bankruptcy.
KRMC purchased the medical center for 1/3 of the original cost, fully equipped. KRMC is in the

community.
On June 24th, KRMC made its first move into HMMC by moving the acute rehab center into the
new location; it is a 12-bed center located on the second floor.

The next move might be to transfer all of the women’s services to the new campus. They have
also penciled in some plans to potentially open up an ER in the fall/winter of 2014, and have
thought about opening up an ambulatory surgery center. The main goal is to have HMMC to
function as a facility where less critical care patients can bee seen.
