This Document is to be issued and used in conjunction with GROESTAR LIMITED 'Generic Risk Assessment' Document. It shall be
used to evaluate the suitability of the generic risk assessment in relation to the potential risks and hazards found on site by
engineers prior to the commencement of any work. The particular hazards faced when dealing with DC power are highlighted in the
'DC Risk' document list and the risks and control measures to be adopted for work associated with power system upgrades and
battery removal/installation are detailed therein. Please review that document and those associated with other aspects of the work
(i.e. Manual Handling, Working at Height, Safe Use of Work Equipment etc.) and sign this document above to confirm that ALL
perceived hazards have been addressed and that suitable control measures are in place to reduce the risks faced to an acceptable
level. If you find that there are significant hazards which have not been identified or addressed then they must be noted on this form
along with suggested control measures. Such control measures will be relayed to your Line Manager for approval. The control
measures and procedures highlighted in the Generic Risk Assessment Documents must be adhered to at all times. Employees found
to have deviated from the Safe System of Work provided for the task may face disciplinary action or prosecution.
The Generic Risk Assessment covers (and is suitable for) the
task on this site

Are cabinets installed facing the correct way and in the correct position.
Has adequate space (cabinet width + nominal 10mm) been left for each RSUs for customer supplied cabinets. Note: Fail if a 600mmW x 2200mmH or
800mmW x 2200mmH cabinet would not fit in the RSU provided. (Refer to Guidance document in FCP A planners guide) Pass where single RSU width

Do all cabinet installations have supplementary support (eg upright / external floor brackets) to aid stability.
1)600 x 600mm Power Base cabinet stability - standalone single cabinet PoPs - Where installed with bolted-up superstructure and cable basket link onto
fixed arterial runway this is deemed to be adequate additional support.
2)600 x 600mm Power Base cabinet stability - standalone single cabinet PoPs under TEP-1 E - Must be bolted to the OHI using either at least one

standard figure of 8 brackets or bolted to a fixed upright adjacent to the cabinet.
3)600 x 800 and 800 x 800 Power Base cabinets under TEP-1 E - Bolting up to the figure of 8 brackets is recommended but is not a failure reason as
long as the OHI is adequately supported by adjacent fixed upright metalwork. Earth bonding is deemed to be adequate where the TEP-1 E is bonded to
the room CEP and the FCP Power Base cabinet likewise.

indicated in job pack drawings.
Dummy uprights are necessary where PoP is installed under TEP1 E, these shall be positioned to allow customer cabinet installation without being
removed. Have they been labelled ‘Do Not Remove’. Refer to EIPB briefing.
Have all Openreach supplied cabinets been fixed to the OH ironwork or CMS using appropriate (non-adjustable) brackets. Has Earth bonding been
provided where 600x800 and 800x800 cabinets are used under legacy ironwork without top fixings.
Has the correct cabinet tops been fitted (the lattice top is acceptable for ac only).
Are all fixings for the cabinet in the position as specified in the relevant assembly drawings.
Are cabinet side panels fitted only at each end of a contiguous block of customer use cabinets.
Are access slots in cable ears fitted to 800mm wide cabinets having security doors fitted with cover plates secured with screws.
Are Cabinets and ironwork undamaged. No scratches visible at arm’s length, scratches down to bare metal are unacceptable.
Does the cabinet have any dents that may affect functionality? These are not acceptable.
For all
dent defects a photo with ruler alongside is required.
Are doors fitted as required (standard on AC+DC Power base, Expanded Battery Capacity Cabinets & CuC).
Are all door locks and catches fitted, undamaged and functioning. Ensure user codes are correct relative to customer order. (2244 if not specified) Door

Is the cable tray within a 600mm wide Power Base cabinet fitted to the left hand side of the cabinet?
Concession to leave on right applies where standalone cabinet already cabled and/or landlocked.
Access for customer cabling has not been compromised by un-necessary cable risers (only fitted to 600mm wide Power Base, left hand side when
viewed from the front), containment systems, cable tray or cabling etc.
The 1 50x50mm cable riser channel is not required in 800mm wide x 600mm deep Power Base cabinets as it obstructs tie cables and customer MSAN
cabling. Where supplied it shall be removed in order to meet requirements.
HDF sub-racks have been provided to match the number of ‘expected future maximum ties’ as stated by the customer in their order.
Up to 6 tie cables may be provided in a 600mm wide FCP Power Base cabinet.
Up to 30 / 24 / 18 tie cables in an 800mm wide Power Base (based on 1/2/3 battery strings).
An HDF rack is provided when expected future maximum ties required exceeds the capacity of the Power Base cabinet. Where 54 – 108 tie cables are
required a second HDF rack is supplied. Each HDF rack has its own cable runway link.
Openreach LLU Tie cables ordered for installation with a ‘new build’ FCP installation are part of the OR QA.
Optical fibre cabling provided for ethernet services is NOT treated as part of the PoP handover. Fibre defects found are to be raised with the ethernet
teams.
Central number TBA but at this moment in time use 08006783078 option 2
All checks above complied with Health & Safety requirements e.g. Cable holes fire stopped permanently and no
