Sports Physical – Students must have a current physical examination by their physician every year – Physicals are only valid 365 days – It is encouraged to have a physical completed between May and July so the physical will be valid the entire school year. – Link to print a copy of the physical is also located in the Parent Community What’s Required? 2. Parent Consent Form (ONLINE) – Click on the form to open the link, under “FORMS” your child/children name(s) will be listed – Click on “Parent Consent Form” to the right of your child’s name to start and complete – Enter all the required information and click submit 3. Athletic Handbook Compliance Form (ONLINE) – First read the Athletic Handbook, then click on form to open, complete, and click submit How to Access Forms and Complete Paperwork? Log-in to the Parent Community using your: NetClassroom user ID and Password Log-in to the St. Mary’s Community: http://www.stmarysannapolis.org/HSAthleticForms Once Logged In: This page allows you to: 1. Find out where to send the completed physical 2. Complete the Parent Consent Form 3. Complete the Athletic Handbook Compliance Form 4. Print a copy of the Physical 5. Read the Athletic Handbook 6. Verify paperwork status #6 #4 #5 #1 #2 #3 Parent Consent Form: • Click on the PCF Link • Under “FORMS” click on “START” link • Select your student, and click on the “PARENT CONSENT FORM” Link to begin completing the form Parent Consent Form: • There are 5 sections to complete • An * denotes required information • Some information may be pre-populated for you • Be sure to click “SAVE” or “SAVE & EXIT WITHOUT SUBMITTING” so you do not accidently loose the information you entered, or if you need to exit and return to work on the form at a later time • Click submit when done with all 5 sections • The box below will appear after you begin the form, and will reference the date and time you started the form, and if the form is completed, or needs to “RESUME” Athletic Handbook Compliance Form • After reading the Handbook Compliance Booklet ( ) ; Click on Athletic Handbook Compliance Form • Enter information into required fields • Click submit when done Checking Paperwork Online: Snapshot of your Student Profile Page: Once forms have been completed, you will receive an auto-generated email response, and a received indicator on the forms page of the Student’s Profile Page in Parent Community under Manage My Information will be marked after the information has been processed. This will serve as your reference point to determine that all athletic paperwork has been completed for the school year. Must have a YES next to all three lines in order to be cleared for athletics for the school year. Frequently asked questions…. This information is sent home in a letter to all new students, including incoming 8th Graders after the registration process has been finalized. For previous Elementary families, this will be different than the User ID & Password you used in the Elementary School. For current parents, this is the same User ID & password you would use to login to check your child/children’s High School grades. ATHLETIC PAPERWORK DEADLINES • All paperwork is due no LATER than: **Thursday, July rd 23 ** • NO Paperwork = NO Tryouts Physical Night for Returning Students • • • • WHEN: WHERE: WHO: TIME: TUESDAY MAY 26, 2015 St. Mary’s High School, A Hall Returning St. Mary’s Students Juniors (2016): 5:30PM Sophomores (2017): 6:00PM Freshman (2018): 6:30PM • COST: $20.00 (Cash or check made out to St. Mary’s High School) • WHAT TO BRING: PHYSICAL – 1ST page of questions are REQUIRED to be completed by parents in order to participate. • NOTE: If you need a copy of this physical for summer camps, please request a copy at check-out. DURING THE SEASON • Practice/Game Field Locations: – St. Mary’s Turf Field – All soccer/football/field hockey games are played at the turf; field turf – Tent is available for team use at the field; facility request required – Parents/Fans wishing to take pictures must receive permission in advance – Annapolis Boys and Girls Club Heritage Turf Field – Practice site, field turf – Germantown Elementary School – Practice site (2 grass fields) – Russell Gymnasium – All practices and games for volleyball TRANSPORTATION • Practice Transportation: – – – – – – – Shuttle bus takes students to all practice locations after school Bus transportation form due to Mrs. Patty Gateau by August 21st, 2015. Shuttle bus to all practice fields beginning on August 31st. Buses departs at 2:45 pm and 3:45 pm everyday on full days of school Players are given a pass that is checked by the bus drivers Practice Shuttle Fee: $175.00 and is good for the entire fall season Weekly schedule is posted outside of the Main Office and announcements made at school ol • Game Transportation – Bus service provided to all away contests – Some exceptions are local contests discussed ahead of time with coach – Weekly bus departure schedule/early dismissals are sent to all teachers ahead of time – Most dismissals are after 1 pm so students are attending fourth period class – A coach will ride the bus to every away game – A coach will wait until every person is picked up following all practices and games • Bus transportation release form for away contests – Due to Mrs. Gateau the day before a contest DURING THE SEASON: • ACADEMIC GUIDELINES: – In order to participate in a practice/game in the afternoon a student must be in school the entire day. If a student arrives after 1st period they must have a doctor’s note for excusal. Any questions can be directed to Assistant Principal, Mr. Reinhart. • Awareness of Activity and Mass Schedules – Eligibility: Any student receiving a failing grade (“F”) on the 10/29/15 report card will be prohibited from participating in after school activities from 11/2/14 to 11/16/15 (10 days) which is playoff season. Students will reinstated by Dr. Morgan following an improved grade and note from teacher. COSTS TO PLAY A FALL SPORT: • St. Mary’s provides team uniforms (home and away) • Student required to have all necessary equipment to play the sport (field hockey stick, soccer shin guards, etc.). – Football shoulder pads and helmets are reconditioned and issued • Practice shuttle fee if necessary • Some teams choose to set up a team website to purchase optional team gear (sweatshirts, etc.). • Fundraising: – Some teams choose a fundraising activity • Optional for parents • All fundraising money put into an account for that sport. AD and head coach decide how it gets spent: additional equipment, coaches gear, team dinners/senior night, team trip, etc. Keeping up with St. Mary’s Sports: • All athletic games for the week are in the Monday weekly “Happenings” (sent to all students primary and secondary email addresses) • Athletic updates/game cancellations can be found on our website and twitter: @StMarysSaints • Updates can also be found in the Athletic Director’s Update in parent monthly newsletter found in the Parent Community online • Schedule changes are also announced at school to all students during daily announcements • Coaches send weekly emails to all team members and parents • Some teams have Facebook and Twitter accounts What Does a St. Mary’s Team Look Like? • On The Field Expectations: – Represented and led by coaches that support and represent the mission of St. Mary’s High School • Team captains are an extension of the coaches, go through leadership training, communicate with coaches and officials, represent St. Mary’s in a positive manner at all times – Competitive, strong individual and team work ethic, sportsmanship, team work, camaraderie – Individual and team growth – Strive for excellence – Catholic Values/Sportsmanship – Humble in victory and gracious in defeat – Every player has a role and is a valued member of a team What does a St. Mary’s Team Look Like? What is a concussion? • TBI – traumatic brain injury TBI – traumatic brain injury • ALL concussions are serious. (MMWR, July 2007) – Only 10% of concussions involved LOC How does a concussion occur? • Concussion occurs from a fall or blow to the body that causes the head and brain to move rapidly back and forth. • Even a mild bump can be serious. Signs and Symptoms Initial Lucid Period – Athlete remains lucid and coherent for 10-15 minutes post injury before s/s become present Thinking & Remembering Physical Emotional/Mood Sleep Learning disabilities, history of migraines, history of concussions can prolong symptoms and recovery Signs and Symptoms Thinking & Remembering – – – – Difficulty thinking clearly Feeling slowed down Difficulty concentrating Difficulty remembering new information Emotional/Mood – – – – Irritability Sadness More emotional Nervousness or anxiety Physical – – – – Headache, or “pressure” in head Nausea or vomiting Sensitivity to noise or light Feeling tired, having no energy – sluggish, hazy, foggy, or groggy – Fuzzy or blurry vision – Dizziness – Balance Problems Sleep – Sleeping more than usual – Sleeping less than usual – Trouble falling asleep Sports-Related Concussion • Research shows that: – Until a concussed brain is completely healed it is venerable to further injury – The effects of repeated concussion in young athletes are cumulative • Allowing enough healing and recovery time following a concussion is crucial in preventing any further complications and injury • No TWO concussion are exactly alike, and symptoms are not definite Sports-Related Concussion Postconcussion Syndrome • Symptoms occurring for an extended period of time after the injury • Usually more in women than men – – – – – – – Decreased attention span Concentration problems Impaired memory Irritability Persistent Headaches Dizziness Premature fatigue Second-Impact Syndrome • Second impact following prior minor concussion – 28% of athletes continue to play following a hit that causes symptoms • Up to several days post initial injury • Catastrophic increased intracranial pressure St. Mary’s Concussion Protocol ImPACT Baseline Testing – All Freshman, Juniors & Transfers of contact & collision sports By Maryland State Law athletes can not be returned to play the same day of injury, and must follow up with a Licensed Healthcare Professional. Evaluation to determine injury on sideline, or in ATC office Emails will be sent to Administrators, Teachers, Coaches regarding injury and Academic Modifications that are recommended for injured student-athlete Post-Injury ImPACT testing, ImPACT Passport App for iphone/ipad Recommended f/u with physician for further evaluation and modifications (St. Mary’s Team Physician, Head First, AAMC Concussion Clinic) Daily check in’s with ATC ATC will notify Administrators, Teacher & Coaches of updates during injury process, and will notify when modifications have been lifted All student-athletes must complete the Return to Play progression program and complete the required form to Return to Play ATC will notify Coaches when student-athletes is cleared to return to play Neurologist & Neuropsychologist Referrals A neurologist & neuropsychologist are equipped and trained to handle the impact of concussion sequlae on cognitive, emotional, behavioral, and academic functions They are crucial for the transition of an athlete’s return to school and activities via necessary support and interventions. Referral to one of these specialists will be based on several factors History of concussion, Severity of concussion, Chronic post concussion syndrome Area doctors who are trained in this area and have treated our students: AAMC Concussion Clinic HeadFirst Concussion Clinic Kennedy Kreiger Institute Kevin Crutchfield, MD – Siani Hospital, Berman Brain & Spine Institute, Ravens Team Neurologist Jerry Gioia, PhD – Children’s National Hospital Center, Washington, DC Prevention There is NO way to prevent concussions from occurring, we can use the tool available to us to minimize the effects of concussions. Helmets – Designed to prevent skull fx Proper fitting, & maintaining correct fit Mouthguards – Proper fit to the teeth, NO cutting Others – There are lots of marketed concussion “preventers” on the market – none of them are going to “prevent” the concussion ST.
