PASSPORT FOR PRIZES CONTEST
Back by popular demand, AHMP will promote a Passport to Prizes Contest to all registered
attendees. Participation in this program is a fun and proven effective way to boost traffic at your
booth and to keep attendees on the exhibit floor.
Here’s how the program works:
1. Each attendee will receive a passport with their registration credentials.
2. Each attendee must stop by each participating exhibitor booth to receive a stamp on his or her passport. (Each exhibitor will be supplied a stamp at the Exhibitor Registration Desk)
3. The attendee must complete the entire Passport in order to be eligible for the Prize Drawing that will be held on
the final day of exhibits (Tuesday, September 1). Please ensure that someone is always staffing your booth during
exhibit hall hours. Attendees must be present at the Prize Drawing to win.
Exhibitor can receive prizes too!
Attendees will be asked the following two questions about the exhibitors and their interaction with booth staff personnel:
1. Which exhibitor had the best booth staff personnel?
2. Which exhibitor had the best new product?
The exhibitors with the most votes for each question will be awarded 50% off next year’s exhibit booth package
(excludes Special Offer 2015 pricing) for the AHMP 2016 National Conference in Washington, DC.

AHMP 2015 NATIONAL CONFERENCE EXHIBITOR TERMS & CONDITIONS
Upon AHMP’s acceptance of the application the exhibitor
agrees to abide by the terms and conditions listed below.
C. Liability
The Exhibitor takes full responsibility for and shall
defend, indemnify, hold harmless and release AHMP, any
of its Chapters and the New Orleans Hyatt Regency
from and against all claims, causes of action, obligations,
suits, liabilities, losses, damages, expenses and/or costs,
including attorneys’ fees related to or arising from or out
of any injury to any person, or any loss of or damage to
property where such injury, loss or damage is incidental
to, arises out of or is in any way connected with the
Exhibitor’s participation in the conference, use of the
New Orleans Hyatt Regency or negligent act or omission
of Exhibitor or caused by its product.
Fifty percent of the full fees are refundable (or in the
case of unpaid purchase orders, invoices adjusted) for
cancellations received via fax or in writing by June 24,
2014. Cancellations received after June 24, 2014 are
nonrefundable. (Please note that if your organization
fails to cancel by the deadline and does not attend, your
organization is still responsible for payment.)
G. Cleanliness
A clean building will be furnished at the start of the
contract period and prior to the start of each day’s
events. Exhibitors must maintain and keep in good order
their respective exhibit spaces.
H. Smoking Policy
No smoking is allowed in the New Orleans Hyatt
Regency exhibit area.
I. Utilities
GES will be the utilities service contractor. Details and
costs regarding electric, telephone, compressed air,
water and drainage, custom cleaning, labor and special
furniture or security services will be included in your
exhibitor packet.

All Exhibitors are responsible for complete security
within exhibit areas, meeting rooms and other areas
to be used by the Exhibitors. Overnight security will be
provided for the exhibit area by AHMP.
M. Payment Options
To reserve booth space return the application form
in this brochure with payment. Requests for refunds
must be made by fax or in writing and be received by
June 24, 2014 to receive the maximum 50 percent
refund. Cancellations received after June 24, 2014 are
nonrefundable and unpaid invoices are still due. We will
accept purchase orders, checks or credit cards. Payment
must be made before booth assignment will be issued.
N. Program Accessibility
We accommodate persons with disabilities. Please call
(800) 437-0137 or mark the appropriate space on the
event registration form and an AHMP representative
will contact you to discuss your accommodation needs.
To ensure accommodation, please register at least
two weeks before the start of the conference. AHMP
is committed to providing programs and activities to
all persons regardless of race, religion, color, national
origin, ancestry, sex, age, disability and veteran status. In
addition, policies prohibit discrimination on the basis of
sexual orientation, marital status and parental status.

In the event that AHMP must cancel or relocate the
conference due to circumstances not under the direct
control of AHMP, AHMP will have no liability of any kind.
Exhibitors are responsible for delivery, setup and removal
of all exhibit materials. Conference organizers will not
provide staff or equipment. Freeman will provide drayage
information.
The Exhibitor shall comply with all applicable laws
governing the use of patent, copyright or trade secret
materials. The Exhibitor shall indemnify and hold
harmless AHMP, its officers, directors, employees,
agents and any Chapter from all claims, causes of action,
obligations, suits, liabilities, losses, damages, expenses
and costs, including attorneys’ fees related to or arising
from or out of any violation or infringement (or claimed
violation or infringement) by Exhibitor, Exhibitor’s agents
or employees of any patent, copyright, trade secret rights
or privileges for other proprietary rights.

Booths are 10’ x 10’. Booths will be equipped with an 8’
high backdrop, a 3’ high side drape, a 6’ draped table,
two chairs and a wastebasket. The booth area will be
carpeted.

payment must be made before a booth assignment will
be issued. Booth assignments will be made on a firstcome, first-served basis with sponsors receiving select
