and healthy? According to a 2015 You Gov poll around a third of
us make resolutions to get fitter and around third aim to eat
more healthily. !

My Life a Full Life
On the Island, a process of significant change is
taking place to help us adapt to the challenges we
collectively face. With more demands on our
health and care services we need to support our
community, staff and volunteers to ensure we have
sustainable services in the future for everyone.!
We need to start by supporting people to lead
healthier lives, right from an early age to prevent ill
health in the future. We need to give people the
right information and advice, focused around their
individual needs, so they can support and care for
themselves and help those with long-term
conditions to plan and manage their future care.!
We need to work differently together to make the
most of the skills and resources we have and bring
together support locally to help people find local
solutions to retain their independence and live life
to the full. !
In short, we need to develop a new model of care
for the Island together that meets our needs as a
local community. Under the MLAFL programme,
The Isle of Wight NHS Trust, Isle of Wight
Council, Isle of Wight Clinical Commissioning
Group, One Wight Health (Island's GP Practices),
and the voluntary, independent health, care and
housing sectors are working in partnership.!
In these bulletins we'll be sharing our progress and
highlighting the difference being made to peoples'
lives.!

Change4Life and for Coppins
Bridge!
The Government’s latest Change4Life campaign
follows revelations that 4-to-10 year olds consume
over 5,500 sugar cubes a year, or around 22kg - the
average weight of a 5-year-old.!
A new Sugar Smart app has been launched to help
parents see how much sugar there is in everyday food
and drink. The free app works by scanning the barcode
of products and revealing the amount of total sugar it
contains in cubes and grams.!
Change4Life has created an eye-opening short film to
warn parents about the health harms of eating and
drinking too much sugar, including becoming
overweight and tooth decay.!
The Change4Life Sugar Smart campaign launched on
4th January across England with television, digital and
outdoor advertising, and updated web content. Five
million Sugar Smart packs are being given away to
primary age children and their families via schools,
local authorities and retailers. A nationwide roadshow
is also taking place across 25 locations from 18th
January. Five major supermarkets have also pledged to
support the campaign through educating and helping
customers make healthier food choices when

Here on the Island, we’re also promoting the
campaign. Look out for our new mural on The Agency
building facing Coppins Bridge, which is sure to grab

Self-monitoring 'first' for the Island
A key part of the My Life a Full Life programme is the ability to help people to care for and manage their
conditions themselves so they can live their lives to the full. !
The Isle of Wight Clinical Commissioning Group (CCG) is the first in England to launch a pilot service
that enables Islanders requiring the anti-coagulant drug warfarin to self-test at home.!
Not only does it allow self-testing, it also enables Islanders to receive their adjusted warfarin dose.
Previously those on warfarin needed to attend clinic every few weeks for a simple blood test, to check
how quickly their blood clotted.!
The new pilot is being delivered with Inhealthcare Ltd, a UK digital health
care company and SCSL Health (INR Star computer aided dosing software),
the UK’s leading anticoagulation decision support software. !
Island GP, Dr Peter Randall, Clinical Lead for the home testing pilot on the
Island, said: “Warfarin clinics on the island are experiencing a high volume of
patients accessing the service, so enabling patients to self-test will alleviate
pressure on General Practices. The reduction in patients attending the clinics
will allow more time to be spent with more complex patients, increasing the
level of care for all.”!
With the new service, a finger prick sample is inserted into the self-testing
device. This reading is then sent securely to the local clinic via their communication of choice, for
example by a phone call or email. Computer aided dosing software is then used to help calculate the new
warfarin dose by the clinician which is fed back into the individual's health record. !
The CCG is initially piloting the service to 100 Islanders in the Sandown area but could be rolled out
further afield if successful.!

behind the news at a range of health & wellbeing
topics. Did you know, for example that women
tend to burn more fat after exercising on a full

exercising on an empty stomach? Want to find out
whether eating carbs after 8pm really is bad for you?

Spotlight on Locality working - Community Care Facilitators
One key strand of the My Life a Full Life programme focuses on locality working – bringing people and services
together at a local level in a more integrated way. Two people who are working very hard to achieve this are
Kate Staplehurst and Felicity Michail – the Island’s Community Care Facilitators whose role is to promote
health and wellbeing through facilitating links to local services and activities. !
Kate (pictured left), a former Healthcare Assistant on the Island for many years,
operates in the North East Locality and said: “Many of the people we have seen
had fallen into social isolation and become housebound. We establish trust and
give people the confidence to reduce social isolation while also empowering them
to live in their own homes and an independent life as long as possible and to do
that without a struggle.”!
“There are a lot of people out there who do currently struggle in this way. But we
are there to help them and to put them in touch with the very many organisations
that are out there who can assist.”!
The pair work closely with the locality nurses and community matrons but are
now receiving referrals from the district nurses team, GPs, Community Rehab team and the dialysis unit.!
Kate added: “It seems to be getting more and more busy with every day and as we build up the local knowledge
and experience. We working very closely with others including the care navigators from Age UK and we are
working very well together.”!
From putting people in touch with volunteering opportunities or social groups,
organising day visits, referring people to Age UK services or for various assessments
(eg falls/fire safety) and domiciliary services for the housebound to providing
information from hearing loops to indoor greenhouses! As word of their role has
spread they have also been able to support other community health and care staff by
carrying out other elements of support, making the most of their contact with the
individual.!
Kate added: "One lady had not been out for two years other than to go to the
dialysis unit. She told me that she really wanted to go out to the M&S café for a
coffee but she had lost her confidence. She used to do that a lot and wanted to try
again. I went with her and we sat and had a coffee. When we got back she was so
happy she shed a tear – she was just so happy she was able to do what she used to. It
was quite a step for her but she did it and we are now looking to find someone so she can do it again regularly."!
Felicity (pictured right), who is based in the South Locality said: “I feel incredibly fortunate to have a job that I
love and find genuine enjoyment in. I really enjoy supporting people with their social concerns and find great
satisfaction in being able to see frequent positive outcomes for people.”!

Integrated Access - secondment opportunity
As part of the My Life a Full Life programme the NHS and Isle of Wight Council are investing in new models of
health and social care that will be better able to meet the current and future needs of the Island community; its
residents and visitors. A critical part of this will be the integration of services
to enable partners to make the best use of limited resources across health
services, social care and related public services. !
It is our intention to establish a single integrated point of access (control
centre/hub) bringing together various emergency and non-emergency health
and social care services in a seamless and efficient way. This will be managed
under a single management structure headed by the Integrated Contact
Centre Manager, a post that will be
a secondment for a fixed-term period of a minimum of six months from
February 2016.!
The post holder will be accountable for the operational management and
delivery of services in relation to both the NHS and IWC contact centres
including its budget and resources. He/she will provide managerial
leadership to develop the Integrated Contact Centre ensuring the
implementation and delivery of a socially and clinically safe strategy,
working closely with partners in the statutory, voluntary and independent
sectors to ensure people’s needs are at the centre of all working practices and policies.!

Family Information Zone (FIZ) – new contact details
Looking for information and signposting for families, children and young people to help them live their
life to the full?!
FIZ provides information on Ofsted registered childcare and is an impartial
information and signposting service for families, children and young people
aged 0 to 19 (up to 25 years for
young people who have learning
difficulties or disabilities). It also
provides details of local and
national support groups, sports
and leisure activities. Details of

From Monday 18 January
the FIZ service, run by the Isle of Wight Council, will have a new
contact number – (01983) 823349 – to which all telephone enquiries
should be directed. The redesigned service will be offering improved
online and digital information, including its web-based service
directory for children and young people; Wightchyps
(www.wightchyps.org.uk). It will also be working closely with the
Island’s three family centre hubs, in Sandown, Ryde and Carisbrooke. !
Patient council member’s national role
Dr Kenneth Bayley, a member of the Patient Council on the Island, was
recently chosen to join the recruitment team for the national NHS New
Models of Care Programme in London as a Patient and Public Voice
representative. The team were preparing to recruit a number of people to
help the programme work at a pace to roll out the new care models
outlined in the NHS Five Year Forward View. !
Dr Bayley said: “The opinion of patients and carers is valued alongside that
of clinical staff and managers to ensure the best candidates are recruited to
these senior posts, based in London and Leeds, that will offer support to
the 50 new Vanguard sites across the country. It is hoped that everyone
selected will be in post by Spring 2016.”!

Free health and wellbeing roadshow coming to Ryde
Ryde will be the latest area to host a free drop-in session offering residents the chance to find out more
about the health and wellbeing support available in their community.!
The event at the Aspire centre, Trinity Buildings in Dover Street on January
28 between 10am and 5pm is being organised by Community Action IW
under the Island’s My Life a Full Life programme.!
It is the latest in a series of events giving residents the opportunity to enjoy
a cup of tea as they meet a wide range local groups, organisations and
individuals in their area who can help them lead full, healthy and
independent lives.!
During the event there will also be the opportunity to engage in Chair
Pilates and Rhythm and Moves designed to improve balance, co-ordination
and muscle use while also available will be advice on massage and the
natural cosmetics and medicines available through the international
company Weleda. !
The event will also give residents the chance to meet their Local Area Co-ordinator and Care and
Community Navigators – all of who work to ensure residents are able to access the appropriate care,
support or services to help them lead full and healthy lives.!
Event organiser Paul Savill from Community Action IW said: “One of the main aims of the My Life a Full
Life programme is to help people to take more control of their own health and care needs, with the right
support, so that they can concentrate on living their lives to the full. !
“This is another event that brings together some of the many local community groups and organisations
whose support, information and advice helps people do just that. We have has a series of these events
around the Island and they are proving increasingly popular.”!
