May 19, 2015 gray seals 3 harbor seals 1 ! 13: 186-189(2015) Gray Seals : Wasabi NMLC 15-005 PHg strangulation ligature to neck, swollen right hind flipper / abscess, and pulmonary infiltrate / pneumonia, high WBC, eosinophilia male wt=22 kg, SL= 98.5 cm, BS=2.53/5 stranded 3/18/15 Nantucket; admit: 3/20/15 last blood: 5/12/15 WBC increased and increasing, eosinophilia last rads/US: two weeks ago last fecal 5-19-15; cestode ova, Diphyllobothrium type PE: Pulled out flipper tag, looks good using flippers P: deworm with higher dose praziquantel 10 mg/kg, + fenbendazole wait 2 weeks repeat CBC PX is good ! ! ! Gray Seals : Cilantro NMLC 15-007 PHg lungworms and infected brand, Step. phocae abscess (resolved) male wt=32 kg, SL= 114 cm, BS=2/5 stranded 5/3/15 Cape Cod/ IFAW; admit: 5/4/15 5/19/15 Rounds Notes 14:186- 189 (2015) page 2 of 4 last blood:5/4/15; increased WBC, hyponatremia last rads:5/4/12: dorsal pulmonary consolidation, pneumonia likely Lungworm last fecal: 5/12/15: larva 2 morpho-types DX: lungworm (Otostrongylus and Parafilorides); cestode ova (no hooks) Diphyllobothrium sp., Cryptocotyle lingua ! PE: (Dr. Voorhis, and Dyer): TPR= 96.3, 60, 10 : abscess resolved in hind flipper, brand is healing nicely, sinus arrhythmia, other OK for age and condition. P: week 2 of lungworm therapy, fenbendazole 50 mg/kg PO SID 3 day, be ready to add steroids (pred 20 mg PO BID 3 days, SID 3 days, 10 SID 3 day, the 5 mg PO EOD 3 x) start nebulizations with alburteol and saline when Colantro is pulled give ivermectin next week if doing well. -CRW ! ! Harbor Seals : Dill NMLC 15-009 PPv pup, dehydrated, sand ingestion male wt=7.6 kg, SL= 84 cm, BS=2/5 stranded 5/13/15 COA Deer Island ME; admit: 5/15/15; last blood: 5/15/15; 5/19/15 last rads: 5/15/15, 5-19-15 PE: TPR= 100.8, 120, 60 (stressed); languino coat is being shed, no infected umbilicus, just starting to erupt teeth, ears and eyes WNL (IOP R=27, L=28 mmHg), limbs WNL, behavior WNL, abdominal palpation is WNL can not feel sand or rocks, other WNL. A: may still have material in stomach P: continue with fluids and tubing, continue with treatment for sand ingestion [ 3cc lactulose in each feed, 1 tsp psyllium fiber with each feed] OK for controlled water access in pod. ! ! 5/19/15 Rounds Notes 14:186- 189 (2015) page 3 of 4 Gray Seals : Tyme NMLC 15-010 PHg alopecia, swollen left hind, died < 24 hrs after arrival female wt=27.3 kg, SL= 103.5 cm, BS=2X/5 stranded 5/14/15 NH; admit: 5/16/15; Thyme died before 24 hrs at the center, a staff member was bit and the head was sent for rabies testing accruing to protocol. ! ! ! Gray Seals : Ginger NMLC 15-011 PHg alopecia, wounds, lungworm, tapeworms female wt=19.7 kg, SL= 95 cm, BS=2/5 ! stranded 5/17/15 IFAW Cape Cod; admit: 5/18/15; last rads: 5-19-15 last blood: 5-18-15 last fecal: lungworm larva ! PE: TPR= 100, 120, 20 feisty typical gray seal behavior, Adm: 1cc 0.3 mg buprenx IM for exam and wound care. 5/19/15 Rounds Notes 14:186- 189 (2015) page 4 of 4 Clipped wounds. Lacerations and eruptions over body, dorsal, flippers, complex alopecia pattern, wounds not consistent with bites, see pictures. EENT limited to due aggressive nature but appear normal, lungs sound clear (but chest film taken), Ginger coughed up a lungworm at the exam (Otostronglyus), passing large tapeworm segments, general disability and unthrift appearance, mm pink and moist. PX: poor PLAN: wound care, pain control (tramadol 50mg PO BID-TID), antibiotics (cephalexin 500 mg PO BID 2 week), once stable start lungworm protocol, but can start nebulization therapy BID TID PRN now. ! ! ! ! ! !
