community hospital - WCA Hospital.
WAIVER &
RELEASE LIABILITY
I hereby waive all claims against the Woman’s
Christian Association Hospital (WCA Hospital),
LINKS Charity, the Chautauqua Mall, Simon
Property Group, the 2012 LINKS Pink Walk,
event sponsors, and all organizers/volunteers for
any injury I might suffer in this event. I attest
that I am physically fit and prepared for this
event. I grant full permission for organizers to
use photography of me in legitimate accounts
and promotion of this event.

LINKS Charity and WCA Hospital are
partnering with the Chautauqua Mall in a
grassroots effort to encourage our community
to embrace the fight against breast cancer, pay
tribute to those who are battling breast cancer,
and to promote good breast health throughout
the month of October – Breast Cancer
Awareness Month. In addition to hosting the
2012 LINKS Pink Walk, the Chautauqua Mall
and its stores will be undertaking a series of
pink-themed promotions and initiatives which
raise awareness and support for local breast
cancer care at WCA Hospital.

In 2011, in honor of its 20th anniversary, LINKS Charity introduced
the inaugural LINKS Pink Walk and solidified its commitment to enhancing local cancer care. In 2012, LINKS Charity embarked on a new
$300,000 pledge toward the purchase of 3-D Breast Tomosynthesis.
This technology will allow the professionals at WCA Hospital to better
diagnose and treat breast cancer in our community.
It’s all about going PINK! As a walker, you are encouraged to be creative and wear as much pink as possible!
There are several ways you can support the LINKS Pink Walk, including:
♦ Participate as a Walker - securing as many pledges as possible to support your efforts
♦ Organize a Team to walk together (challenge friends, co-workers and creatively outfit yourselves in PINK)
♦ Sponsor a Walker - make a generous pledge
♦ Serve as an Event Sponsor
♦ Purchase “Tribute Link” In Honor of or In Memory of a loved one that will be used to build our LINKS Tribute Chain
♦ Make a gift-in-kind donation for the Basket Auction at the Walk
♦ Make an outright tax-deductible gift to: LINKS Charity
Our goal this year is to raise $30,000 toward the purchase of
3-D Breast Tomosynthesis in the WCA Center for Imaging
& Medical Arts (Jamestown, New York).
