Intellectual/Developmental DIsabIlIty servIces Communicare Connections Oxford, MS • 662-234-7521 Adult Day Services Services to assist and support individuals in retaining and/or improving skills which afford the greatest level of independence possible. Numerous and varied opportunities to participate in activities in the community are provided. Behavior Supports Intervention Provides assistance to individuals, staff, and family members to assist them in implementing specific behavior supports and intervention programs. Community Respite Offers respite to caregivers and provides the individual with a place to go which has scheduled activities to address preferences/ requirements and also provides for the health and socialization needs and community integration of the individual. Home Community Supports Assistance with activities such as personal hygiene, meal preparation, eating, dressing, and shopping,leisure activities, community integration, and light housekeeping provided in the home. (over) Job Discovery Activities and exploration that lead to the development of a person centered career profile and employment goal or career plan. Supported Employment Intensive ongoing support to obtain and maintain an individual not in competitive or customized employment or self-employment. Pre-Vocational Training Meaningful day activities of learning and work experiences, including volunteer work, used to develop general strength and skills that contribute to paid employment in an integrated setting. Who Can Be Served Communicare Connections serves individuals with a diagnosis of intellectual/developmental disabilities and their families seeking assistance with maintaining and developing skills necessary for greater independence.
