6 Eyelid Tumour Clinical Benign non-inflammatory eyelid lesions Seborrheic keratosis. Seborrheic keratosis is a pigmented cutaneous lesion that can occur through out the skin in older individuals but can develop in the eye lid area .This raised lesion can be smooth of verrucous, with varied pigmentation, size and distribution. Management Nevus Nevocellular nevi are benign proliferations of melanocytic cells divided into 3 catergories depending on their architecture: junctional,compound and intradermal. The congenital nevus of the eyelid is a special category with implications for malignant transformation. It is managed with surgical excision. Actinic Keratosis Actinic keratosis is a premalignant condition characterized by dysplasia of the basal cell layer of the skin. It ranges from millimeters up to 1cm in size. They usually present as erythematous, scaly macules or papules on sun-exposed skin. Frequently the keratosis or an elevated white flaky crust is seen on the surface. Diagnosis Biopsy of suspicious lesions and follow-ups are prudent in patients with this contition. The base of the lesion must be examined histopathologically to exclude invasive squamous cell carcinoma. Therefore, biopsies of suspected actinic keratosis should include the base of the lesion. Shave biopsies should be avoided. Management Treatment alternatives include excision cryotherapy and a variety of topical agents to extirpate the lesions. Diagnosis On the eyelid, the nevus often arises at the eyelid margin and may be flat, elevated, dome-shaped and even pedunculated. The flat lesions are often junctional nevi.The congenital nevus of the eyelids may present as a kissing nevus in which the melano cytes are present symmetrically on the upper and lower eyelids. Mangement Management of congenital melanocytic nevi depends on a number of factors, including the size of the lesion, the location of the lesion, the age of patient, the effect on cosmesis, and the potential for malignant transformation. Some nevi in which the diagnosis is obvious may be cosmetically unacceptable Verrucous - Covered with warts or wart-like projections; Dysplasia - Abnormality of development; Benign – Not harmful Macule - Change in surface color, without elevation or depression; Keratosis - growth of keratin on the skin; Prudent - advisable A R A V I N D E Y E C A R E S Y S T E M 7 to the patient or may cause irritation, particularly if they are pedunculated. These are easily removed using the slit lamp. Hypercholesterolemia or congenital disorders of cholesterol metabolism may be associated conditions. Xanthelasma Diagnosis Xanthelesma is a rare condition characterized by soft yellow plaques in the medial canthal area invoving the eyelids of patients. Xanthelesma is usually diagnosed by examining the skin growths, but a biopsy of the tissue will confirm a fatty deposit. A blood lipid profile and other tests may be done to determine the underlying condition responsible for the appearance of Xanthelesma. Management - - - Surgical excision Destruction by chemicals (Trichloroacetic acid), diathermy or photocoagulation. Recurrence is very common. - Ms. Muthumari OPD; Aravind - Tirunelveli Extirpate – destroy completely; Intradermal - within the layers of the skin; Pedunculated- attached to a bare; Nevus - raised red patch; Hypercholesterolemia - excess of cholestrol in blood; Erythematos - appearing red; Papule - swelling of the skin.
