How To Make A
Referral To Hospice of
Chesterfield County
Anyone...whether it be a physician, a family
member, friend, professional, or the patient...may
refer someone to Hospice of Chesterfield County.
Admission to the program is dependent upon the
willingness of the patient and the family to participate. The patient should designate an attending
physician who is willing to work with the Hospice
team. The physician may practice outside our service area which is Chesterfield, Lancaster and
Marlboro counties. The Hospice of Chesterfield
County Medical Director, Dr. John F. McLeod, III,
may serve as the designated attending physician
for the patient who does not have a physician.
Anyone desiring information or wishing to make
a referral may call the Hospice of Chesterfield

(843) 672-9155 or 1-800-572-9322, Monday through
Friday from 9 AM³5 PM, or write to: Hospice of

the growing needs for hospice care in our area. As
a community-based, non-profit organization, Hospice of Chesterfield County maximizes its resources to provide the highest quality and compasVLRQDWHFDUHWRRXUDUHD·VWHUPLQDOO\LOOUHVLGHQWV
In the past ten years, Hospice of Chesterfield
&RXQW\·V SDWLHQW FDUH GD\V KDYH LQFUHDVHG RYHU
50% to 16,000. This increase in the utilization of
our services has also resulted in an increase in the
amount of local indigent care we provide which
now averages over $125,000 every year. Just as we
have done for the past 21 years, Hospice of Chesterfield County remains steadfast in its mission to
provide loving care to all terminally ill residents
regardless of their ability to pay.

Hospice of Chesterfield County held its annual Trees
of Celebration Lighting Ceremony on Sunday, Dec. 7,
with over 2,000 lights in honor and in memory of
friends and loved ones.

doctor (current and retired physicians, dentists,
chiropractors, etc.). As a non-profit hospice, your

provide quality, compassionate care for area terminally ill residents regardless of their ability to pay.

Watchman Program Connects Hospice
With Local Congregations
Ninety Area Churches
3DUWLFLSDWLQJ,Q3URJUDP¶VWK<HDU
Ninety local churches have now partnered with
Hospice of Chesterfield County through the
DJHQF\·V:DWFKPDQ3URJUDP7KLVVSHFLDOFKXUFKhospice partnership program was implemented to
help ensure that people receive quality end-of-life
care sooner in their diagnosis regardless of where
they live, their diagnosis, age or economic status.
7KH JRDO RI +RVSLFH RI &KHVWHUILHOG &RXQW\·V
Watchman Program is to identify and train a representative in every congregation in Chesterfield
County who will serve as the Hospice Watchman
Representative in that congregation. This hospice
trained representative will serve as a liaison between Hospice and the church, conveying the
needs of the church family to Hospice as soon as
those needs arise. Representatives will also recruit
other volunteers from the congregation to ensure
that every Hospice patient who wants a volunteer
has the opportunity to benefit from the support and
care a volunteer can provide.
´&KXUFKHV SOD\ D YLWDO UROH LQ RXU FRPPXQLW\

Day of Prayer for Local Hospice Care and Services on Nov. 26th at the Hospice office in Chesterfield. Staff, volunteers and friends gathered to
pray for Hospice patients and families, staff and
volunteers and services with November recognized as Hospice Awareness Month.
