It’s a true honor to do the work we do at Colorado Health Network, Inc. dba Colorado
AIDS Project to help prevent the spread of HIV and other disease, and to help bring
stability to the lives of those impacted by HIV. Often times, many of those we serve are
struggling to meet some of their basic needs in life that many of us may take for granted.
Throughout this annual report, you’ll find a great deal of information about the services
we provide that bring stability to clients’ lives including linking or retaining them in care
and other health-related services. In this letter, we’d like to share information about the
future of Colorado Health Network.
Colorado Health Network, Inc. is a 31-year-old community based organization that was
created in response to the AIDS epidemic of the early 1980s. Our founders had the
foresight to create an organizational name that would allow us to be nimble in the
post-AIDS era. Although there is not a cure for AIDS, advancements in treatment and
care have allowed for the transformation of the epidemic of three decades ago to a
chronic disease. Living with HIV is not easy, but with proper treatment and care,
HIV-positive people can now look forward to living their golden years.

Today, with health care reform and the implementation of the Affordable Care Act, our
clients have greater access to health care than they’ve ever had in the past. As a result
of these wonderful changes, our focus has transitioned to providing more health-related
services for clients and even slowly expanding the populations we serve in support of
healthier lives and healthier communities.
Last summer, we celebrated a “coming home” for Howard Dental Center, as the
organization was merged back into Colorado Health Network where it started over
twenty years ago. This expansion of the organization and the services we provide
positioned Colorado Health Network as a leader in the not-for-profit community that has
taken a proactive approach to health care reform.
We are currently in the process of searching for a new home for Denver Colorado AIDS
Project, Howard Dental Center, and our statewide administrative service offices. When
this move is completed about a year from now, we’ll be a much more efficient
organization with the ability to provide a greater number of services to clients and
patients, all under one roof. We envision expanding to provide more general health and
wellness services and general prevention services. We continue to have conversations
with complementary organizations to discuss potential partnerships that will enhance
and expand the services we provide. As we move forward, please watch for news and
updates about these exciting changes.
We couldn’t do this important work without you! Thank you for your continued support as
we work toward bringing healthier lives to persons with HIV, reducing the spread of the
disease, and building a new future serving clients and communities.

In the early 1980s, the AIDS pandemic was just starting to come into public light. It was a time of confusion,
without anybody understanding what was happening and little communication to help guide efforts to save
the many people that were dying of this new disease.
Communities across the world acted swiftly, trying to develop whatever action plans they could in reaction to
the growing crisis, including many communities throughout Colorado. From these rapid responses grew a number of
organizations that, over the years, would each develop into the largest AIDS Service Organizations (ASO) in the state.
The Denver-based ASO took on the name Colorado AIDS Project with the thought that what was occurring
would be a finite project. Other metropolitan areas of Colorado followed suit
with the Colorado Springs ASO adopting the name Southern Colorado AIDS Project,
the Fort Collins ASO assuming the name Northern Colorado AIDS Project,
and the Grand Junction ASO implementing the moniker Western Colorado AIDS Project.
For more than two decades, each AIDS Project throughout Colorado operated cooperatively, yet independently.
In 2008, at the request of several funders, each office came together to begin discussions about how services
might be provided to the HIV community if federal funding were to disappear.
As discussions evolved over the next three years, organizational leaders began to see the benefits of merging,
and in October 2011 made the merger official.
The newly-merged organization would be known as the statewide Colorado Health Network working under the dba
of Colorado AIDS Project. The Denver office gave its name to the statewide organization and underwent a name
change to Denver Colorado AIDS Project (DCAP), matching its name to its regional service area.
Northern Colorado AIDS Project (NCAP), Southern Colorado AIDS Project (SCAP)
and Western Colorado AIDS Project (WCAP) retained their regional names as dba entities.
The creation of the newly-formed Colorado Health Network increased the ability to sustain necessary services
to meet the evolving needs of our clients and of those impacted by HIV in Colorado,
while decreasing the overhead of operating four independent offices.
In the summer of 2014, Denver’s Howard Dental Center merged with Colorado Health Network,
adding oral health care to the list of services already provided, and allowing Colorado Health Network offices the
opportunity to expand dental services offered in various parts of the state.
Services at CHN’s regional CAP offices are provided at no cost. Costs for Howard Dental Center services are minimal.
All services are offered without regard to age, sex, race, religion, disability, sexual orientation or gender expression.
Penny DeNoble
Penny DeNoble was diagnosed
with HIV early in the epidemic’s
existence,
contracting
the
disease in 1986. Her story of
increased
stability
and
self-sufficiency
is
a
prime
example of why DCAP is in
existence.
After being on a treatment
regimen for almost a decade,
Penny was hospitalized at Denver
Health for over a month due to AIDS-related
complications and turned to DCAP for services in
2007. At the time of her becoming a DCAP client, she
was home-bound and very ill with an AIDS diagnosis
(T-Cell count less than 200). A Case Manager from
DCAP went to her home to complete her intake and
to get her connected to services.
“It meant so much to me that someone would come
to my home, to meet me where I am, to help meet
my needs,” said Penny.
Penny received assistance with insurance, housing,
nutrition and transportation, and worked with her
Case Manager to get back to a point where she was
empowered to self-sufficiency. Today, her T-Cell
count is over 950, and she only uses DCAP services to
connect her to health insurance with periodic
6-month check-ins with her Case Manager. Penny
also receives regular checkups and dental work
through Howard Dental Center.
Through her empowering story of healing, Penny was
invited to sit on Denver Mayor Michael Hancock’s HIV
Planning Council, using her story and her knowledge
of HIV to make change in our community. One such
effort is the organization of a task force to address the
disparities in infection rates that continue to exist in
the African American community.
Penny is clear and direct in her message to people:
“Ignorance is not bliss - what you don’t know will kill
you. It’s so vital for people to know their status.”

Denver Colorado AIDS Project (DCAP) is a
31-year-old community-based organization
that helps those living with HIV/AIDS, and those
at risk, to make healthy choices for a
self-sufficient, enhanced quality of life. DCAP
provides innovative, individualized services to
those most in need, educates high-risk
populations, and advocates for social and
health care equity.
DCAP empowers those living with or at risk of
contracting HIV to live healthier, enhanced
lives by providing a continuum of support
services
including
case
management,
counseling, transportation, housing and a food
bank. DCAP also educates those at risk of HIV,
and offers regular free HIV testing on site and
through
community
health
center
collaborations.
DCAP’s caseload of 3,023 individuals
represents approximately 52% of the clients
served statewide.
Roy Curry
Following
an
automobile
accident in 1982, Roy Curry
contracted HIV through a blood
transfusion. At the time of
diagnosis, Roy had two children
attending college. His weight
dropped to 90 pounds, and he
was unable to work, which is when
his doctor referred him to
Colorado AIDS Project.
During those early years when
antiretroviral medications weren’t
available, Roy relied on CAP to obtain medicines and
other basic staples such as food and warm jackets for
himself and his family. Roy will also tell you that he
relied on CAP for moral support. Outside of his
immediate family, CAP was a place where he knew
he could access an understanding support group.

Phillips, Sedgwick, Washington, Weld,
and Yuma
Northern Colorado AIDS Project (NCAP) is a
29-year-old community-based organization
that helps those living with HIV/AIDS, and those
at risk, to make healthy choices for a
self-sufficient, enhanced quality of life. NCAP
maintains offices in Fort Collins and Greeley
and provides services to people infected with
HIV/AIDS in a mostly rural eight-county area of
Northern Colorado.
NCAP has established key partnerships with
community members and organizations and
provides counseling, educational programs,
affordable care, a food bank, and case
management services to those affected by
HIV/AIDS. NCAP also educates those at risk of
HIV, and offers regular free HIV testing on site
and through community health center
collaborations.
NCAP’s caseload of 1,012 individuals
represents approximately 17% of the clients
served statewide.
Roy often talks about his fear during this period of not
being able to watch his children grow up, and of living
with the stigma society put on people living with the
disease. CAP counselors helped Roy understand life
choices and how to respond to society’s messages.
Through this, Roy learned to persevere. Thirty-three
years later, Roy has not only seen his children grow into
adults, but is also watching his grandchildren grow up.
In 2001, Roy moved to Greeley to be closer to his son
and grandchildren. Today he relies on the support of
Case Managers from Northern Colorado AIDS Project
to help him with his transportation and basic support
needs.
Roy is amazed by the fact that he was able to watch
his third child graduate from college in 2014. He
remarked, “I can’t believe I have lived with HIV for so
long. It’s amazing to be able to tell people that I’m a
Baby Boomer!”
At a recent awareness event in Fort Collins, Roy spoke
of how today he stands on the shoulders of many
caring individuals at Colorado AIDS Project who have
helped him on his journey.
Roy believes we must continue to reach out to
younger populations. “Young kids need to know
about this disease, and if I can help put a face on it,
like Ryan White did, hopefully it will take away some of
the shame.”
Jeffery Shepherd
In the mid-1980s, Jeffery Shepherd
was living in the San Francisco Bay
Area with his wife and kids. He
underwent invasive surgery in
1985, prior to the start of blood
screenings for donated blood
products. Eleven years after his
surgery, he was diagnosed with
HIV and turned to support services
in the East Bay.
In the early 2000s, Jeffery’s eldest
daughter passed away, and his
18-year-long marriage dissolved. Lost and alone while
dealing with his daughter’s death, Jeffery turned to
friends in Colorado for support and relocated to
Colorado Springs in 2005. Prior to his relocation, his
California medical team helped him research support
services in Colorado, so he was connected with SCAP
immediately upon his arrival.
Jeffery had strong concerns about the social
differences he expected to find in Colorado versus
Northern California. Having grown up in the Bay Area,
he knew that same-sex relationships, HIV, and spiritual
practices were accepted and even mainstream in his
hometown, but Colorado held a lot of unknowns. His
worry about dealing with HIV stigma and transitioning
from Northern California services to Colorado services
were put at ease almost immediately upon
connecting with his SCAP Case Manager.
SCAP facilitated Jeffery’s living situation and got him
connected to housing through a pilot program that, at
the time, took care of his rent for several months while
his Case Manager helped him with referrals to arrange
his child support responsibilities back in California.
Today, Jeffery’s Case Manager is helping him navigate
his Social Security recertification and has connected
him to an outside organization to assist him with his
current bone disorder and medical issues.
Jeffery can’t say enough positive things about CAP.
“If not for SCAP, I wouldn’t be living now. I thought I
would be in Colorado for just a couple of years, but
now, 10 years later, my Case Manager and the
volunteers at SCAP have been the ones who have
kept me going – kept me living.”

Huerfano, Kiowa, Kit Carson, Las Animas,

Rio Grande, Saguache, and Teller
Southern Colorado AIDS Project (SCAP) is a
29-year-old community-based organization
that helps those living with HIV/AIDS, and those
at risk, to make healthy choices for a
self-sufficient, enhanced quality of life. SCAP
maintains offices in the metropolitan centers of
Colorado Springs and Pueblo in Southern
Colorado in Colorado.
SCAP empowers those living with or at risk of
contracting HIV to live healthier, enhanced
lives by providing a continuum of support
services
including
case
management,
counseling, transportation, financial, insurance
and housing assistance, as well as a food
bank. SCAP also educates those at risk of HIV,
and offers regular free HIV testing on site.
SCAP’s caseload of 812 individuals represents
approximately 14% of the clients served
statewide.
Chris Klein
Chris Klein is a Grand Junction
resident and a current client of
Western Colorado AIDS Project.
He was first diagnosed in 2002
when he lived in Fort Collins. The
termination of a long-term
relationship took him to NCAP for
an HIV test. Chris admits to feeling
depressed immediately after his
diagnosis but was determined to
live a full life. He began receiving
Case Management and Medical
Referral services from Northern Colorado AIDS Project
immediately after his diagnosis.
Counties served (22): Archuleta, Delta, Dolores,
Eagle, Garfield, Grand, Gunnison, Hinsdale,
Jackson, Lake, La Plata, Mesa, Moffat,
Montezuma, Montrose, Ouray, Pitkin,
Routt, Rio Blanco, San Juan,
San Miguel, and Summit
Western Colorado AIDS Project (WCAP) is a
26-year-old community-based organization that
helps those living with HIV/AIDS, and those at risk,
to make healthy choices for a self-sufficient,
enhanced quality of life throughout the western
slope of Colorado - a 40,000 square mile region.
In order to increase accessibility, WCAP has
established strong partnerships with a variety of
service providers across the region.
WCAP empowers those living with or at risk of
contracting HIV to live healthier, enhanced lives
by providing a continuum of support services
including case management, transportation
assistance, nutrition assistance, and connections
to medical care. WCAP also educates those at
risk of HIV, and offers regular HIV testing on site.
WCAP’s caseload of 250 individuals represents
approximately 4% of the clients served statewide.
A series of job changes included stints in
telecommunications and retail, which brought about
moves across the state to Cortez, Parker, and finally
to Grand Junction.
Throughout his statewide
relocations, Chris was able to receive Case
Management services from DCAP, as well as COBRA
insurance assistance, rental deposit assistance, and
Medicaid acquisition assistance from WCAP.
Currently, living in a small rural area, Chris admits
building relationships has been a personal challenge
within a small GLBT population. He is happy, however,
that his health has remained strong and that his most
recent move has allowed him to explore his passion in
music. He’s also happy to be a part of the GLBT
community as perceptions of being gay and HIV
change.
A pianist and vocalist, he is currently the choir director
at a Methodist church, accompanies music students
at the local university, and teaches piano and vocal
lessons privately. Although Chris finds a way to get by,
he is grateful that he has been able to turn to WCAP
when needed.
Chris’s message to people about HIV is direct, yet
informative. “Inform yourself. There’s an incorrect
stereotype of what HIV is – a 1980s mentality that
doesn’t acknowledge that a lot has changed since
then. There’s a lot to worry about in conjunction with
HIV including other STDs. It’s frustrating that people in
the GLBT community don’t understand that and that
they’re not being safe anymore.”
Salina Ann Flores
Salina Ann Flores was diagnosed
with HIV on April 19, 1992, and in
2011 began hormone therapy to
transition to her true female
identity.
After relocating to Denver from
Durango, CO in 1994, Salina
became a patient of Howard
Dental Center at a time when
many
dental
professionals
wouldn’t accept patients with
HIV. A decade later she began receiving services
from DCAP for housing and rental assistance.
She admits that she used to have a negative attitude
around the disease, but has grown to a place where
she no longer holds any anger and simply wants to
focus on her health and happiness.
Through Howard Dental Center, Salina was able to
get a new set of dentures, which has helped to instill
more confidence. She speaks with glowing praise
about the staff at Howard Dental Center and credits
them for being able to seek out more transgendered
services.
As a transgender woman, Salina sometimes suffers
from social anxiety but is happy to use her personal
story to educate people about transgender issues
and HIV. Salina doesn’t consider herself an activist
but will take the opportunity to talk about her HIV
diagnosis when the opportunity arises, using the term
“Hives” as a means of comfort when referring to HIV.
Having watched her partner pass away from
AIDS-related complications, and living with the
disease herself, Salina wants to make sure that
people are cautious and heed her warnings, “Be
careful, use protection and get tested.”
Salina is very proud of the way she has been able to
manage her health and the support that she’s
received from her friends at Howard Dental Center
and DCAP. “My T-Cell count was 1,001 at my last
checkup!”
Howard Dental Center (HDC) is a 21-year-old
community-based oral health care facility
that helps those living with HIV/AIDS. HDC’s
patient base is filled with individuals who have
faced discrimination, poverty, and/or a lack
of access to proper dental care. With a focus
on comprehensive care, the “emergency
walk-in” treatment that HDC patients often
faced has been replaced with a care model
that offers patients the full range of oral health
treatments.
HDC empowers those living with HIV to live
healthier, enhanced lives through five
programs that include: Fast Track; Healthy
Baby & Youth; Perioptions; and Screening,
Brief Intervention and Referral to Treatment
(SBIRT) in addition to frequently interacting
with the patient’s physicians in order to
provide the most appropriate treatment
possible.
HDC’s caseload of 747 individuals represents
approximately 13% of the clients served
statewide.

in the Food Bank,
administering HIV tests,
canvassing at festivals,
community outreach,

events are just some of the
areas in which CHN’s
volunteers contribute.
CHN is truly grateful to the

In fiscal year 2014,
volunteers contributed a total
of 12,223 hours of service
