CHAC Director's Dialogue May and June were very busy months for CHAC, with an educational summit and donated care planning at the top of the agenda. There are a number of educational opportunities coming up this summer, please take advantage of these and don’t forget to SAVE the DATE on September 18 for our Annual Health Summit! Please contact [email protected] projects or initiatives. for information on any CHAC At the May meeting, a rich discussion took place about the ongoing plan for donated care. The La Plata Community Clinic will offer limited service two (half days) a week beginning in November, 2012. This small integrated care clinic is dependent upon medical, dental and mental health providers donating their time and expertise to care for low-income, uninsured residents of La Plata County. Planning continues with office space negotiations, hiring clinic executive and medical directors, and most importantly calling for community resources and volunteers to support the effort. We hope that CHAC members will lead the way! A June mini-summit on the Community Care Teams targeted La Plata County providers. Twenty-eight people attended the summit, gaining knowledge and services about the program. The session was led by a team from the Colorado HealthStory Project. The CCT/nurse navigator program (contact Kathleen McInnis at 970-335-2005) and Mercy Home Health/Palliative Care Program (contact Dan Keuning at 970-382-200) offers solutions to some of challenges presented in our local health stories. Thank you for your ongoing participation in this important community health team.
