At the May meeting, a rich discussion took place about the
ongoing plan for donated care. The La Plata Community Clinic will offer limited service
two (half days) a week beginning in November, 2012. This small integrated care clinic
is dependent upon medical, dental and mental health providers donating their time and
expertise to care for low-income, uninsured residents of La Plata County. Planning
continues with office space negotiations, hiring clinic executive and medical directors,
and most importantly calling for community resources and volunteers to support the
effort. We hope that CHAC members will lead the way!
A June mini-summit on the Community Care Teams targeted La Plata County providers.
Twenty-eight people attended the summit, gaining knowledge and services about the
program. The session was led by a team from the Colorado HealthStory Project. The
CCT/nurse navigator program (contact Kathleen McInnis at 970-335-2005) and Mercy
Home Health/Palliative Care Program (contact Dan Keuning at 970-382-200) offers
solutions to some of challenges presented in our local health stories.
Thank you for your ongoing participation in this important community health team. We
appreciate all you do!

The Affordable Care Act
On July 10, Marguerite Salazar, the regional director for the U.S. Department of Health
and Human Services, spoke on “The Affordable Care Act: A health care law that
provides basic fairness and a patient’s bill of rights.” She talked about what has
already happened, what is happening and what will happen in 2014, and the
advantages that will come with the act. There were over 100 people in attendance. She
offered brochures on the act and offered www.healthcare.gov as an additional
resource. Her appearance was sponsored by the League of Women Voters, the
American Association of University Women and the Citizens Health Advisory Council of
La Plata County.
