Brian L. Long, FACHE President & Chief Executive Officer Memorial Healthcare has a long tradition of serving and meeting the health care needs of Shiawassee County and beyond that dates back to 1921. This tradition is one we take great pride in meeting and exceeding through our commitment of continual improvement and the broadening of services we offer. These expectations have never been more challenging, yet our resolve has only increased to ensure we successfully achieve these expectations. Thanks to the dedication and teamwork shown by our medical staff, employees, and volunteers, we have never been more capable of accomplishing this mission. Joining us this past year are four seasoned senior leaders: James Barb, Vice President of Performance and Quality; James Nemeth, Vice President of Patient Care Services; Tim Susterich, Chief Operating Officer/CFO; and Amy Blaising-Wallace, DO, Vice President of Memorial Medical Associates. Leadership Development. Employee Engagement. Physician Engagement. Strategic Partnerships. These are Memorial Healthcare’s strategic priorities. The development of these strategic priorities reinforces my belief in both the strength each employee, provider, and volunteer brings to Memorial Healthcare and the critical role of partnership and collaboration in our healthcare network. It also affirms that Memorial Healthcare is stable and well positioned for growth and development, despite the rapidly, ever-changing health care environment. This strategic plan provides a roadmap for Memorial Healthcare and is intended to be a dynamic and integrated effort. The overarching goals will provide a framework of what needs to be performed by each and every one of us to attain our vision and fulfill our mission. While some initiatives will be undertaken hospital-wide, many others rest with the dedicated individuals throughout the organization who strive to deliver quality care and compassionate caring each and every day on the front lines. We have an incredibly strong foundation of excellence on which to build as we explore how we will deliver the highest quality, affordable care to our patients, train the next generation of clinicians and other healthcare professionals, and improve the health of the communities we serve. These priorities will assist Memorial Healthcare in meeting the expectations of our patients and insurance payers, improving the overall reputation of the organization and increasing overall market share within the region. As our constituents, you shouldn’t expect any less. The healthcare field is an ever-changing, always-challenging place to be. We’re working smarter to not only keep up with regulatory and fiduciary requirements, but to make sure our patients and the welfare of the communities we serve remain front and center. Let me take this opportunity to share the 2014 Strategic Imperatives we’ve been using as a road map for the future: 1. Maintain a healthcare delivery platform within Shiawassee County and its surrounding service areas to include hospital, outpatient, primary and specialty care services. 2. Pursue strategic partnerships with recognized leaders of quality within the Memorial Healthcare service area to: • Improve reputation and increase community awareness of available service offerings; • Provide effective coordination of care across the entire spectrum of healthcare services required by our community; and • Provide a broader portfolio of services coupled with a seamless system of delivery. 3. Heighten our organization’s focus on quality. These are no small tasks. From each of these imperatives come layers of intentional, measurable, focused tactics that affect every part of Memorial. Leadership, employees, volunteers, physicians, suppliers: you name it, they’re playing some sort of a role in moving Memorial toward its vision of being recognized as a national model for excellence in personalized healthcare. Two new board members recently joined us on our journey: Aaron Maike, Baker College of Owosso President and The Honorable Gerald Lostracco. The expertise and commitment they bring will only enhance the visionary, strategic work our board is doing. We also say good-bye and “thanks” to long-time board member Charlie Green. No board chair agrees to step into such a position of leadership without his or her own vision for the future. I see Memorial Healthcare evolving into THE place people in Shiawassee County and beyond want to come for all of their health needs. I see expanding a model that puts patient service above all else. Let’s make it easier to have a preventative screening or be referred to a specialist that’s working hand-inhand with other Memorial physicians to ensure every patient’s continuum of care is met – always with an eye toward excellence. I want our community to see us as their resource, their partner. In December, Dr. Bishop, who is board-certified and fellowship-trained in emergency medicine, received the Memorial Healthcare Physician of the Year Award. He was bestowed this prestigious honor by his fellow medical staff members. Rany Aburashed, DO Named Partner in Multiple Sclerosis Care In March, Rany Aburashed, DO, board-certified neurologist, received designation as a Partner in Multiple Sclerosis Care - Neurological Care from the Michigan Chapter of the National Multiple Sclerosis Society. Dr. Aburashed was the first neurologist in Michigan to receive this designation, which recognizes clinicians who have demonstrated their capability to provide optimal care and support to people with Multiple Sclerosis. To learn more, visit MemorialHealthcare.org/neurology. 88 2014 ANNUAL REPORT HOPE Program This year, the Memorial Cancer Center oncologists launched the HOPE program which is centered around Helping Oncology Patients Early in their diagnosis. This program is coordinated by the oncology physicians at the Cancer Center and serves the region’s medical providers by expediting the initial staging and work-up of patients once a malignancy is suspected or diagnosed. Patient and program benefits include a focused coordination of care for diagnostic testing, surgical referrals both within and outside of the Memorial network, regular progress reports to providers, and a high level of collaboration with all agencies for comprehensive care. Memorial Partners with TeamHealth for Inpatient Management Services Memorial Healthcare welcomed TeamHealth’s Hospital Medicine Associates to provide round-the-clock inpatient hospital medicine services to its patients last September. Hospitalists manage and coordinate the entire inpatient experience for those patients whose physician has chosen not to provide inpatient care. Once discharged, patients do return to the care of their primary healthcare provider. Together, we are more successful. 1,084 Employees 200 Nurses (Full-time, Part-Time RN’s) Nurse of the Year – Lisa Gilna, RN Nurses’ Week recognition at Memorial Healthcare began on May 6 and was focused on honoring nursing staff from all areas of the organization. At Memorial, there are hundreds of nurses providing hospital-based care, physician office-based services and home health and hospice services. Nurses’ Week 2014 culminated in the announcement of Lisa Gilna, RN as the 2014 Nurse of the Year. Gilna delivers patient care services on Memorial’s 4-South Long-Term Care Unit. She exemplifies the core nursing values of competence, teamwork, excellence, compassion, integrity and evidence-based clinical care. 14 14 2014 ANNUAL REPORT President’s Award Recipient – Lorna Quirk, RN Each year, the President’s Award is presented to an individual who exemplifies the essence of Memorial’s core values and mission. The 2014 President’s Award recipient was Lorna Quirk, RN. Lorna promotes excellence through her work with the Professional Nurse Practice Committee, bringing together the brightest and most dedicated nursing minds in our organization to improve our facilities, processes, and staff. Lorna promotes integrity by understanding her role in her department and the opportunities she has to build relationships and understand through role-modeling positive interaction, holding herself and others accountable, and protecting the privacy and dignity of patients and staff as they heal or grow. Lorna’s trusted guidance, loyalty to common goals, and dedication to those she works with and serves is deserving of this award. In my career I have been blessed with the talents of many unforgettable people, but none that have offered themselves and their gifts as sincerely as Lorna, and it is an honor to work beside her to achieve our dream of being a national model for excellence in personalized healthcare. Employee of the Year – Shelly Reynolds The employees and physicians of Memorial Healthcare annually vote to name one employee of the month from the previous year as the employee of the year. At the annual Hospital Week ceremony, Shelly Reynolds, medical records technician in the Health Information Management Department, was designated as the 2013 Employee of the Year. She exemplifies the core values and standards of behaviors of Memorial Healthcare on a daily basis as recognized by her coworkers. Good Safety Catch Award Recipient – Linda Cramer, RN Memorial regularly solicits safety suggestions from healthcare workers throughout the organization. Linda Cramer, RN, was awarded the annual Good Safety Catch Award for her suggestion that a “pop up” appear on the computer screen when a patient does not want blood product when questioned during the admission process. This allows Memorial Healthcare to be sensitive to our patients’ religious beliefs directly from the admission process. Thank you Linda! MemorialHealthcare.org 15 LEADERSHIP DEVELOPMENT In alignment with the organization’s strategic priorities, leadership team members at Memorial Healthcare are focused on skill building in critical areas such as operational analysis, employee management, conflict resolution and quality management techniques necessary to achieve individual and organizational objectives. As a leader in the continuously evolving field of health care, today’s leaders have the formidable challenge of creating both short- and long-term strategies. Focused efforts have been undertaken in the areas of engaged workforce development, motivation, negotiation, patient safety and financial management. The following is a review of several key facilities that are now providing exceptional care and support for residents of its service area. We also continue to facilitate partnerships through our Commons Project that will result in expanded healthcare options in the future. Occupational Health Center In March 2014, the Memorial Occupational Health Center opened at the hospital’s 239 N. State Road facility across from Meijer in Owosso. A wide range of occupational medicine services are available by appointment or on a walk-in basis. The clinic is open Monday through Friday with services provided by occupational health specialist, Mike Dada, DNP, FNP-C, COHN, COHC. Dada is a certified DOT medical exam provider, certified occupational health nurse and a certified occupational health hearing conservationist. Mike Dada, DNP, FNP-BC, COHN, COHC Elsie Building Donated to Elsie Food Bank Memorial Healthcare’s Board of Trustees donated the building that previously housed the practice of David Huff, DO to the Elsie Food Bank. The nonprofit organization is now operational from its new location, serving those in need in the local community. MemorialHealthcare.org 17 Ovid Elsie Health Center A ribbon-cutting open house was held the spring of 2014 at the newly constructed Ovid Elsie Health Center. This building houses the physician offices of David Huff, DO of Ovid Elsie Family Health Care, as well as the office of Narender Thatikonda, MD and Gary Trochlil, PA-C of Ovid Elsie Primary Care. Additionally, outpatient lab and x-ray services are available to the public. The retail Harvest Pharmacy is also conveniently available onsite. 18 18 2014 ANNUAL REPORT The Commons Ground Breaking Ground breaking took place in 2014 on The Commons, a Memorial Healthcare community development project . The Commons property is in Caledonia Township and consists of more than 100 acres owned and managed by the Memorial Healthcare Development Corporation. The vision for The Commons is to create a commercial development that combines similar healthcare offerings in one common area for the collective benefit of the community. Current services at The Commons include primary care services at the Shiawassee Community Health Center (HDI Inc.) and the Memorial State Road Health Services building. Urgent care services and occupational health services are also available at the State Road facility. Opening just this past month, The Meadows, an assisted living and memory care community, began serving residents. With the opening of the Shiawassee Community Health Center, full-time dental services will now be available for those individuals in our county who are uninsured or underinsured and have traditionally not had access to dental services within the surrounding area. In addition, the Shiawassee County Medical Care Facility has officially announced an agreement with the Memorial Healthcare Development Corporation to work collaboratively on financing the construction of a new facility in The Commons with an anticipated opening date in 2017. Most notably, The Meadows, HDI Inc. and Shiawassee County Medical Care Facility are independent entities helping to fulfill Memorial’s vision for The Commons as a community development. The Meadows Ground Breaking Inside The Commons and now serving residents with assisted living and memory care needs is The Meadows. This 37,000-square-foot, “all-inclusive” community features 47 private rooms , 20 of them designated to memory care. Residents at The Meadows enjoy the independence of home living combined with the support services they need to enjoy a more worry-free life. But even though we are proud of the statistics shared below, what’s most important is the outstanding care we strive to give each of our patients. 20 20 Licensed Beds: 150 Inpatient Admissions: 2,804 Births: 486 Inpatient Surgeries: 776 Emergency Department Visits: 25,046 Outpatient Registrations: 201,413 Urgent Care Visits: 6,872 Outpatient Surgeries: 3,179 2014 ANNUAL REPORT PATIENT SATISFACTION Memorial Healthcare Receives Four-Star Rating from Centers for Medicare & Medicaid Services The Centers for Medicare & Medicaid Services (CMS) has introduced a star rating system on Hospital Compare, the agency’s public information website, to make it easier for consumers to choose a hospital and understand the quality of care they deliver. Memorial Healthcare has been given a four-star rating, exceeding the ratings of several surrounding Michigan hospitals! The ratings are based on data from the Hospital Consumer Assessment of Healthcare Providers and Systems Survey (HCAHPS) measures, which include topics such as how well nurses and doctors communicate with patients, how responsive hospital staff are to patient needs, how clean and quiet hospital environments are, and how well patients are prepared for post-hospital settings. Of the 3,553 hospitals nationwide that had sufficient patient survey data to review, only 251 received all five stars. Just over a third of hospitals (1,205) received a four-star rating, while three-star ratings were earned by 40 percent of the facilities (1,414). MemorialHealthcare.org 21 #1 Patient Engagement Award Axial Exchange and Becker’s Hospital Review National Patient Engagement Survey announced in May of 2014 that Memorial Healthcare ranked number one in the State of Michigan and 28th nationally out of 3,077 hospitals for achieving the best patient engagement scores. MemorialHealthcare.org 25 Healthy Michigan Plan Memorial FIT Kids Update On April 1, the Michigan Department of Community Health (MDCH) launched the Healthy Michigan Plan - a new, affordable healthcare program for low-income adults between the ages of 19 and 64. The Healthy Michigan Plan covers many services, including preventive and wellness services, doctor visits, hospital care, prescription medications, dental and vision services and much more – all while offering incentives for people who adopt healthier behaviors. At the same time, people with pre-existing conditions won’t be denied coverage. In 2014, the Memorial FIT Kids program began its 10th year in collaboration with the region’s school systems. The proactive Memorial FIT Kids program identifies cardiovascular health risks in our children and educates them to prevent future serious health problems. FIT Kids continues to prove its effectiveness and its sustainability as a Memorial Healthcare Foundation grant-funded program. Over the past 10 years, 4,700 sixth-grade students have participated in cardiovascular screenings and approximately 11,600 students have been impacted overall. Each year, approximately 1,200 students are provided lessons on healthy eating and active lifestyle habits as delivered by two Memorial FIT Kids educators, Rebecca Dahlke and Mary Mallory. S. Department of Agriculture (USDA) to provide digital x-ray Computerized Radiology (CR) units, workstations and interactive digital telemedicine connectivity between Memorial Healthcare in Owosso and five rural, primary and specialty care clinic sites. This enhanced technology enables live interactions between rural primary care physicians/patients and radiologists at Memorial’s main campus and significantly reduces current costs associated with the use, interpretation, quality and storage of analog x-rays at each clinic site. The rural sites receiving the digital upgrade are located in Chesaning, Caledonia Township, Durand, Perry and Owosso. Amy Blaising-Wallace, DO, board-certified primary care physician in Perry says, “This new technology provides my patients and our providers with better quality images that I can view and enhance electronically at my clinic.” One of the Country’s Most Wired Hospitals Memorial Healthcare was again recognized as one of the nation’s “Most Wired” hospitals, according to the results of the Most Wired Survey released in the July issue of Hospitals & Health Network’s magazine. Home health services are provided to patients recovering from a wide range of illnesses or injuries and those who have been diagnosed with a terminal illness. CHIME Collaboration Award Memorial was awarded the 2014 Collaboration Award from the College of Healthcare Information Management Executives (CHIME) organization for its efforts in the development of a robust software product to assist the hospital in meeting Meaningful Use Stage 1. Memorial’s partner in the development of that product was Iatric Systems, Inc., a comprehensive healthcare IT integration company. In 2009, Memorial Healthcare and Iatric recognized collaboration between the two organizations would be of mutual benefit with Memorial’s Owosso-based hospital serving as the development partner for Iatric’s Meaningful Use Manager™ software product. Additionally, Memorial Healthcare recently attested to Meaningful Use Stage 2 compliance. As of November 2014, just 17 percent of hospitals around the country had successfully met Meaningful Use Stage 2. Achieving Stage 2 means the organization is successfully using its electronic health record system to capture and share patient information related to immunizations, reportable laboratory results and syndromic surveillance. This stage also means the hospital has the capability to identify and report cancer cases to a national registry and specific cases to a specialized registry. Volunteer of the Year – Sharon Flannigan At the April 2014 Volunteer Appreciation Luncheon, Sharon Flannigan was named Volunteer of the Year. Sharon is an extremely dedicated volunteer who always puts the needs of Memorial Healthcare’s visitors before her own. Sharon joined Memorial Healthcare as a volunteer because she wanted to help others. Her duties include working the front desk, making popcorn, decorating holiday trees, assisting with blood drives, helping to make pillows for Memorial Healthcare patients and volunteering at the annual Cancer Survivor Day. Using proven quality practices to save lives and reduce healthcare costs is vital. At Memorial, efforts are targeting the reduction of ventilator-associated pneumonia, bloodstream infections, peptic ulcers and leg blood clots. Keystone: Hospital-Associated Infection (HAI) Memorial is employing lessons learned from around the nation to focus on the prevention of hospital-associated infection. At Memorial, caregivers employ best practices including hand hygiene, as well as bloodstream infection and urinary tract infection protocols. Keystone: Obstetrics Memorial participates in a statewide patient safety collaborative with the Michigan Health and Hospital Association and 60 other Michigan hospitals. This collaboration is focused on protecting mothers and newborns and seeks to prevent harm during the birth process. The initiatives focus on timely interventions for elective induction of labor; coordinating a safe progression of labor; and appropriate responses to fetal distress. Our goal is to provide not only an excellent birthing experience, but to ensure the safety of mothers and newborns through the use of shared best practices. Keystone: Sepsis Septicemia is the most expensive inpatient condition to treat and was the 11th-leading cause of death in the United States in 2011. Keystone Sepsis is a joint initiative between the emergency room and critical care units developed to improve compliance of the Surviving Sepsis Campaign (SSC). These guidelines are used for patients with the diagnosis of sepsis in an effort to reduce mortality through implementation of timely and appropriate treatment. This serves as a screening and treatment plan that is started in the emergency department and continued into the patient’s critical care unit stay. This grant made possible the purchase of a surgical attachment to enhance services in laparoscopic abdominal surgeries and in orthopedic surgery cases. The equipment is utilized by general surgeons, as well as the hospital’s orthopedic surgeons. Bedside Ultrasound Benefits Emergency Department The emergency department was the recipient of a $38,523 grant in 2014 which made possible the purchase of a bedside ultrasound unit. Dedicated solely to the use of the emergency department staff, this equipment enables providers to conduct clinical and imaging procedures without the delay of transporting the patient to the diagnostic imaging department. This purchase was granted by Memorial Healthcare Foundation’s Emergency Department Fund. 42 42 2014 ANNUAL REPORT Oto Acoustic Emissions (OAE) Machine Allows Early Detection A sum of $25,430 was awarded to Red Cedar Ear, Nose and Throat for the purchase of a state-ofthe-art Oto Acoustic Emissions (OAE) machine specifically designed to accurately assess hearing capability and provide physicians with specific information on the severity of hearing loss – even in young children who cannot communicate about their sense of hearing. OAE testing allows early detection of hearing deficits in the newborn and young population. The Ear, Nose and Throat Fund contributed $16,065 and the remaining $9,365 was made available using unrestricted funds. MemorialHealthcare.org 43 Auxiliary Celebrates 95th Anniversary (May 2015) In May 1919, the work of organizing the drive for funds to build Memorial Healthcare was started by Mr. Cornelius Loder. Mr. Loder’s work resulted in a thorough organization of the women of the entire county into an auxiliary that has been a most important factor in the hospital work ever since. After they had fulfilled their responsibility of preparing the temporary hospital, they went to work at the new building providing linens, kitchen ware, food, equipment, and more. Twenty years later, the auxiliary officially organized its volunteer services which continue yet today. In 1964, the name was changed to The Memorial Hospital Auxiliary, making it possible for them to accept any men who wished to join. The Auxiliary played an important role in the establishment of Memorial Hospital, and 95 years later, they continue on with this important calling. The Four Seasons Gift Shop at Memorial Healthcare is the largest fundraising arm for Memorial Healthcare and relies on the dedication of its committed team of volunteers for purchasing and staffing. In 2013, the Four Seasons Gift Shop reached the $1M milestone in contributions made to the hospital and was voted Best Gift Shop in Shiawassee County by the Argus Press readers in 2014.
