RESOLUTION APPROVING AN AGREEMENT
WITH ROUTE 47 TAXI TRANSPORTATION FOR THE
RUTLAND I HAMPSHIRE TOWNSHIP SENOR
TRANSPORTATION VOUCHER PROGRAM

WHEREAS, in November 2013 the Village of Huntley had entered into an arrangement with
Route 47 Taxi Transportation to provide services in Rutland and Hampshire Townships; and
WHEREAS, these services began December 1, 2013 and continued through April2014; and
WHEREAS, there is a continued interest in providing transportation services to residents in
Rutland and Hampshire Townships and Route 47 Taxi Transportation is interested in continuing the
current arrangement to provide these services; and
WHEREAS, it is the Policy Direction of the Village Board to enter into an arrangement with
Route 47 Taxi Transportation and to allocate $10,000 to fund these services through December 31,2014.
If the $10,000 runs out prior to December 31, 2014 Village Board approval will be necessary to allocate
additional funds; and
NOW, THEREFORE, BE IT RESOLVED BY THE PRESIDENT AND BOARD OF TRUSTEES OF TH
VILLAGE OF HUNTLEY as follows:
SECTION I: The Village of Huntley adopts and approves the execution of a letter of agreement with
Route 47 Taxi Transportation Inc. to provide senior transportation services to Village residents in Hampshire and
Rutland Townships as outlined in the agreement attached hereto and copy of which is incorporated herein.
SECTION II: The Village Manager is authorized to execute said agreement with Route 47 Taxi
Transportation Inc. in an amount not to exceed $10,000.00.
SECTION III: This resolution shall be in full force and effect from and after its passage and approval as
provided by law.
SECTION N: All resolutions and parts of resolutions in conflict herewith are hereby repealed.

Re: Rutland and Hampshire Township Ridership Voucher Program
Dear Mr. Villasenor:
The Village of Huntley wishes to continue the Ridership Voucher Program with Route 47 Taxi subject to
the following conditions:

The intent of the Program is to provide senior transportation services to Village residents living in
Rutland and Hampshire Townships within the corporate limits of the Village of Huntley only.
•
The Village has allocated $10,000 to continue the program through December 31, 2014. If funds
run out prior to December 31, 2014, Village Board approval will be necessary to allocate
additional funds.

The Village agrees to reimburse Route 47 Taxi based on an agreed upon voucher system subject to
the following fare schedule:

Rider Fare: All riders are required to pay $2.00 per ride. No Charge for caregivers of riders
Payment to Route 47 Taxi will be made weekly by the Village subject to review and verification
ofvouchers submitted. Vouchers are to be submitted with accompanying invoice by 5:00pm
Friday to receive payment by the next Friday.

Route 47 Taxi will provide proof of insurance and agree to name the Village as an additional
insured.
Route 47 Taxi will agree to hold the Village harmless and the Village will assume no liability
from any claims made by riders as part of this arrangement.
Riders are required to register with the Village of Huntley prior to becoming eligible for the
Voucher Program.
All riders are required to schedule rides a minimum of 24 hours in advance with Route 47 Taxi.
A senior should be a minimum of 55 years of age
No pets allowed, except guide dogs
Curb to curb service
All riders must be able to board and exit the vehicle independently
Cancellations required 24 hours in advance.
Based on the funding of $10,000, the expected time-frame for the program will be from May 1,
2014 through December 1, 2014.
Prior to the spending level being met, either party may tenninate the arrangement upon ten (10)
days written notice.
All notices permitted regarding this arrangement shall be transmitted only be personal delivery to
