Waiver and Release of Liability

In consideration of Extreme Paintball Ta' Qali furnishing and equipment to enable me to
participate in paintball games I agree to the following terms and conditions:
I fully understand and acknowledge that (a) risks and dangers exist in my use of paintball
equipment and my participation in paintball activities; (b) my participation in such activities
and/or use of such equipment may result in my injury if such equipment is not used in
accordance to the guidance and rules provided to me, including but not limited to body injury,
disease, strains, fractures, partial and/ or total paralysis, eye injury, blindness, heart stroke,
death or other ailments that could cause serious disability; (c) these risks and dangers may be
caused by the negligence of the participants, the negligence of others, accidents, breaches of
rules and regulations and force majeure which risks may arise from foreseeable or
unforeseeable causes; (d) by participating in these activities I hereby assume all risks and
dangers and all responsibility for any losses and/or damages, caused through my negligence.
I, on behalf of myself, my personal representatives and my heirs, hereby voluntarily agree to
release, waive, discharge, hold harmless, defend and indemnify Extreme Paintball Ta’ Qali and
its owners, agents, officers and employees from any and all claims, actions or losses for bodily
injury, property, damage, wrongful death, loss or services or otherwise which may arise out of
my negligent use of paintball equipment. I specifically understand that I am releasing,
discharging and waiving any claims or actions that I may have present or in the future through
my negligent acts on owners, agents, officers or employees of Extreme Paintball Ta’ Qali.
I HAVE READ THE ABOVE WAIVER AND RELEASE AND BY SIGNING IT AGREE THAT IT IS MY
INTENTION TO EXEMPT AND RELIEVE EXTREME PAINTBALL TA’ QALI FROM LIABILITY FOR
PERSONAL INJURY, PROPERTY DAMAGE, OR WRONGFUL DEATH CAUSED THROUGH MY
NEGLIGENCE.
REMEMBER YOU ARE PLAYING AT YOUR OWN RISK!IT IS ABSOLUTELY PROHIBITED TO DRINK
ALCOHOL OR MAKE USE OF DRUGS (medical or other) BEFORE OR DURING PLAY! Upon signature of this waiver you will automatically become a member of Extreme Paintball Ta’
Qali for the duration of 1 year whereby you will receive updates of events
