It begins at home
Our three agencies work together to help our patients and their families enjoy the special moments of life – big and small. Home Health
VNA, Merrimack Valley Hospice, and HomeCare, Inc. each offer specialized services and medical care that keep people healthy, safe and
comfortable at home.
With your support, our 800 professionals serve patients in many settings: the community, private residences, assisted living and skilled
nursing facilities, hospitals, and at Merrimack Valley Hospice House when care at home is no longer possible. We are able to do
all this with medical expertise, an unwavering focus on compassionate care, collaborative care teams, and the use of technology –
achievable in part by the generosity of those like you who understand the importance of those things that ensure the quality of life.
As a not-for-profit health care provider, we are responsible and accountable to the communities and people we serve. We place importance
on providing the expertise and resources needed for you, your neighbors, and friends to be cared for and supported in a safe and
comfortable setting – providing compassion along the journey.

Dear Friends,
Healthcare is a dynamic industry, evolving as national healthcare reform
takes shape and providers face the challenge of delivering what is best for
patients amid widespread payment reductions.
The agencies of Home Health Foundation are fortunate to receive
charitable support that allows Home Health VNA, Merrimack Valley
Hospice, and HomeCare, Inc. to each remain committed to providing
high-quality care to all. I can assure you that we are ever-diligent about
maximizing the effectiveness of our time and resources to be able to deliver
such care in the most cost-effective ways possible. Our investments in both
time and money in 2013 allowed us to create new ways of expanding our
care, work more efficiently, and make our services more valuable to our
patients and the greater healthcare system we are part of.
Our organization’s greatest asset remains the full spectrum of caring that
our three agencies create as a continuum that allows us to provide tailored
care to our patients based on their changing needs. We continue to leverage
our resources as we pioneer new programs such as palliative care and
population wellness, and we take the lead in innovative approaches to
healthcare such as information exchanges and collaborations. More than
ever our clinical partners turn to us to help ensure the effectiveness of
care for our mutual patients.
The achievements of the past year – the expansion of Merrimack Valley
Hospice House, the launch of several groundbreaking programs, and the
effect we have had on quality of life for our patients – were all possible
through your assistance. Funding has allowed us to succeed where others
have not and it is one of the reasons we are and remain the leader in
home health and hospice care in the communities we serve.
We are grateful to each and everyone who has helped us continue our
mission to create and deliver innovative and responsive community health
programs that provide effective and compassionate care. Thank you for

Merrimack Valley Hospice
So they can treasure each moment along the journey
Hospice is a form of compassionate, comprehensive care for those facing a terminal illness that is no longer responding to curative treatment. It’s designed
to improve quality of life by meeting physical, social and emotional needs with a team of professionals and volunteers. At home, in a hospital, in an assisted living
facility, or at Merrimack Valley Hospice House in Haverhill, MA – hospice care fosters peace and comfort with support for patients and their families.
In 2013 major milestones included two expansions which are serving vital needs in our community: we furthered the compassionate journey along our
care continuum with the expansion of Merrimack Valley Hospice House, and enhanced the adult palliative care program which often smoothly transitions
patients from one level of care to the next.

Palliative care
Growth in our Palliative Care Program was achieved by collaboration with our referral sources
who have found our services invaluable for patients with chronic disease or those newly diagnosed
with serious illness. Helping to establish goals of care, creating plans to meet patients’ hopes for
living their days as they wish, prepared for the challenges they may face, is empowering. Our
pediatric palliative care programs, one of the longest running in the state, grew in number of
children cared for and in funding by the state to do so.

Medical specialization
A team of three physicians, all board certified in palliative care and hospice, provides the
medical expertise for the focused needs of our patients both in the community and at
Merrimack Valley Hospice House. JoAnne Nowak, MD, Medical Director for Merrimack
Valley Hospice recently earned the designation Fellow of the American Academy of
Hospice and Palliative Medicine, representing the focus on this specialty for our patients.

York Hospital Hospice growth
The number of patients cared for in York, Maine doubled in 2013 as we increased awareness of our presence in the region. Bringing a medical model of
care with the resources of professional bereavement support has earned us the trust of the York community.

Looking ahead to 2014
We are increasing the expertise of all hospice staff to care for those with dementia and Alzheimer’s by providing them training and tools to better engage
patients and their families.
We are launching a program to acknowledge and honor our US Veterans that will tend to the processes that these men and women may go through related
to grief and loss needs as result of their service to our country.
We are improving and ensuring reliability and quality of care with a standard of practice, RIGHT Care, that focuses on the patient experience.

So he could relish the joys of 100 years with those he holds dear
Ray Lavin’s Seabrook Beach home has been the center of his life for 30 years and where his family, friends, and faith came together to celebrate his 100th
birthday last June. Despite advancing congestive heart failure and kidney disease, Ray’s oldest daughter Ann credits Home Health VNA with allowing Ray
to enjoy not only his birthday, but a family wedding in the fall, being honored as the town’s oldest resident, time at the beach with his dear friend Kay and
many treasured moments since. “Last spring, when we realized Dad wasn’t sleeping well at night because of anxiety about his health, our Home Health
VNA nurse introduced us to palliative care, support that helped ease his mind, so he could sleep well at night and enjoy his days. ”
Ray’s milestone birthday was celebrated over two consecutive June Saturdays. Laughter, warm embraces from friends and family and chuckles from dozens
of birthday gifts and cards, including those that reflected Ray’s passion for the Boston Red Sox defined the celebration of a man whose vibrancy and love
of life means the world to his 7 children – Ann, Barbara, Ray, Sally, Jack, Jeff and Julie – his 16 grandchildren and 14 great grandchildren, and more than
75 others who joined the party and a special mass and blessing in his backyard.
In January, Ray began receiving care from Merrimack Valley Hospice, which adds regular visits by a nurse to the palliative care he had received, helping him
remain comfortable to enjoy each new day of his remarkable life.
“He is looking forward to spring training and just being able to enjoy his backyard, though he is loving the time he spends in his sun room during this
long winter” says Ann. “We cherish his being able to remain here at home for each day that he has. The Palliative Care Program and Merrimack Valley
Hospice have made it possible to reach this point, along with helping our family – to ease our minds, set our expectations, keep us aware and informed,
and take wonderful care of our dad.”

So moments can be treasured in a place like home
Nothing brought more joy to Dorothy Graham’s life than being surrounded by her children and grandchildren. When she suffered a third stroke and
it became clear that she would not recover, family members traveled from around the country to say goodbye to their beloved Nana.
Bob Graham was devastated at the thought of his mother dying in a hospital with limited visiting hours. “When word came from Merrimack Valley
Hospice House that ‘We have a room for your mother,’ I can’t tell you what a relief and blessing it was,” Bob says. “I couldn’t imagine in her final days
being told we were on a waiting list because there wasn’t a room available.”
With unlimited visiting hours at the Hospice House, the 15 grandchildren all gathered together comfortably in her large private suite, serenading
Dorothy with her favorite country songs. The children and grandchildren felt welcome to make themselves at home, getting snacks from the wellstocked kitchen, watching movies or reading books in the library, and staying overnight in her room.
“We knew what it meant to come here, and there is sadness in that,” says Bob’s wife, Donna. “But the minute we walked through the door, there was
an immediate feeling of serenity and peace and caring. And for our time here, we were able to focus on being together, sharing good memories and
treasuring every moment.”

Merrimack Valley Hospice House
Always positive, always hopeful, always respectful
Most hospice and palliative care is provided to patients in their own homes. But when it’s no longer possible for someone to remain at home for advanced
care, Merrimack Valley Hospice House provides a comfortable, welcoming home-like alternative.
Patients who may need more intensive pain management for a short time or require the oversight of a physician to help address complex symptoms may
turn to the specialized care at the Hospice House and then return home.
Merrimack Valley Hospice House is a place that brings trained hospice professionals together — physicians, registered nurses, nursing assistants, chaplains,
social workers and volunteers — to provide highly specialized care to patients with advanced illness as well as support to their loved ones.

The need to expand
When Merrimack Valley Hospice House began welcoming patients in 2009, it
quickly became a valued resource in the community. It began having a profound
impact on the lives of thousands and proved to be a critical component in the
healthcare continuum for those who could no longer remain at home for their
palliative or end-of-life care. However, demand often exceeded the fourteen
suites that were built.
Licensed for a total of 21 patient suites, Merrimack Valley Hospice House
accelerated construction to add the additional seven suites to complete the
facility. In September 2013, we opened the new wing with the House nearly
filled that first weekend.

A unique resource
One of only two inpatient facilities in Massachusetts to care for children as
well as adults, Merrimack Valley Hospice House addresses specialized medical
needs of those with advanced illness, with compassion and dignity.
Patients may come to Merrimack Valley Hospice House after being cared
for by a hospice provider at home or in an assisted living or skilled nursing
facility. They may not have had any type of hospice care before coming, or
perhaps they have been receiving only palliative services. Or, some families
turn to Merrimack Valley Hospice House for respite, a break. Every patient
is unique and an experienced admission team guides the transition to or from
the Hospice House.

Specialized care for advanced illness
With physicians board certified in both palliative and hospice specialties and a team of others
to assist with patient and family needs, the House serves as a place to turn when needs extend
beyond the scope of what may be provided in the home setting. For some, it means a few
days spent at the Hospice House can help regain control over pain, restlessness, anxiety and
appetite – allowing them to return to their homes physically and emotionally stronger.

shared together
During life’s journey we celebrate many events and milestones. Merrimack
Valley Hospice House’s expansion from 14 to 21 patient suites is a milestone
that benefits every community served by our agencies – whether they are
receiving care from Merrimack Valley Hospice, Home Health VNA or
another home health or hospice provider.
The expansion to the House increases access for short-term acute symptom
management, respite care, and those who need or want to be in a setting with
a higher level of specialized care as their illness progresses. With more rooms
to accommodate patients and their families, more treasured moments can be
spent together as fears are erased and comfort is regained.
We are so grateful for those who supported A Compassionate Journey Campaign
and invite all to be part of this wonderful project as it continues forward.

A Compassionate Journey
A Compassionate Journey campaign was established to help fund the recent expansion of Merrimack Valley Hospice House and the care and supportive
services offered to patients and their families. Adding seven new suites in 2013, we are now able to say “yes” to more people when they turn to the Hospice
House – more families who can treasure time together in a setting that supports the challenges they and their loved ones may be facing.

Home Health VNA
So that healthcare moves forward with positive results
For patients of all ages, returning home following an illness or surgery, allows for recovery or care in a familiar and comfortable environment. In some
instances, our specialized nursing and innovations in treatment may improve a lingering condition that has not improved in the past.
Home Health VNA brings resources and expertise along with care management in the home setting to improve health, prevent complications, and reduce
the risk of hospital re-admissions. The clinical team collaborative approach with our community healthcare partners has brought better coordinated care
to our patients.

Wound care
Those struggling with non-healing wounds face life-altering limitations, pain, and high risks of infection. Seven wound certified clinicians have expanded
the Wound Care nursing specialty for Home Health VNA, leveraging two newly available technologies made possible through grant-funding. The WoundMatrix® System has helped monitor and communicate treatment effectiveness back to physicians so that doctors and visiting nurses can collaborate on
the patient’s care. We added MIST® ultrasound healing therapy which has been used to treat difficult to heal, long-standing wounds that have been
unresponsive to other forms of treatment, bringing relief after many months and sometimes years of suffering.

Integrated Care Management
We have invested in staff training for Integrated Care Management which helps stop the cycles
of complications with progressive chronic disease, allows self-management by patients, and
improves the quality of life for those with progressive, long-term diseases such as congestive
heart failure, COPD, and diabetes. More than 30 staff nurses and clinical managers have been
certified, improving management of cases and engaging patients in their own care and wellbeing.

Effectiveness and quality
Restructuring our clinical teams in 2013 has allowed us to more efficiently deliver comprehensive care across the large geographic territory we serve.
Regular meetings of interdisciplinary teams and staff focused on quality had helped to improve care coordination especially for high-risk patients, to
anticipate issues and intervene before they materialize.

Looking ahead to 2014
We look forward to continuing advanced collaboration with healthcare providers in the
community as we launch Patient Activation Measures that will allow us to work together
to engage patients in their own health and improve the overall effectiveness of their care.
Expanding cardiopulmonary rehabilitation training to our staff with a model we are
developing with the University of Massachusetts Lowell, we will be able to make significant
gains with those suffering from chronic cardiopulmonary disease.

So she can still treasure her independence
At age 94, Helen relishes her independence. Until 2012, she was healthy and safe in her apartment within a senior living community. Surrounded by her
cherished antiques, many of which she restored herself, Helen was managing quite well.
Everything changed when she developed a serious leg wound that became painfully debilitating, restricting her movement and raising fear that her days
of independent living may be over. Helen and her family began thinking that maybe she would need to consider moving to a skilled nursing facility and
that her condition might never improve. However, when Stephanie Ogden, RN, Home Health VNA nurse and certified wound specialist stepped in, she
helped chart a course of care that would not only heal the wound, but restore Helen’s confidence, comfort and independence.
“I first met Helen when a nurse practitioner referred her for therapy to improve her mobility. After an assessment and spending some time with Helen in
her home, it became clear that there was an underlying issue with the veins near the wound site which were making it hard to heal,” recalls Stephanie who
was making daily visits to care for Helen’s wound.
She consulted with Helen’s primary care physician to discuss her observations, and then arranged for Helen to see a vascular surgeon at the wound clinic
of a local hospital. The treatment needed would include a minor surgery to correct the condition and realizing Helen’s anxiety about the procedure,
Stephanie connected her with a counselor to ease her mind and planned her post-operative care.
Stephanie and the other Home Health VNA nurses who visited Helen every day provided the hands-on care to assure proper healing of the wound and
maintained communication with Helen’s physician, the wound clinic specialists and her family. Stephanie also taught Helen’s personal care aides how to
properly protect the wound while bathing and dressing her.
“Stephanie was with my mom every step of the way,” says Helen’s daughter, Linda. “She expertly realized the problem was greater than we had initially
thought and coordinated every aspect of her care before and after her surgery. For my mother, who had been passionate about restoring arts in her
younger years, Stephanie was passionate about restoring my mother’s independence.”

So at-risk patients are supported in their independence
Marnie Shambo, RN, ICM, is more than an experienced homecare nurse with advanced expertise in chronic disease management. She’s a champion for
at-risk patients who is proud that HomeCare, Inc. successfully launched “Health Connections,” a model program for patients and families coping with
chronic disease.
Launched as a pilot program in Newburyport last summer, Health Connections serves patients who may struggle with self-management of congestive
heart failure, chronic obstructive pulmonary disease and diabetes. Health Connections provides these patients with supportive clinical guidance when
they no longer need Home Health VNA services.
“Too often, patients with chronic diseases end up back in the hospital because of seemingly little things – skipping a blood pressure check, not taking
medication or over-indulging in sugary foods,” says Marnie. “Health Connections helps transition care from our visiting nurse teams to effective selfmanagement in collaboration with patients and families… reassuring, reminding and alerting patients and caregivers when concerns emerge.”
Communication is key. Every patient challenged with chronic disease has different levels of understanding and support when it comes to managing their
health. Health Connections offers practical, supportive strategies to help patients be independent while following their treatment plans. “It is more than
just making sure, for example, that our diabetic patients are monitoring their blood sugar. It’s about helping them manage the challenges of life, like the
holidays…have a small piece of that pie that’s been a family tradition for generations, but don’t forget to take these steps before and after. Success means
our patients are confident, educated and as empowered as possible to manage their disease proactively.”
Health Connections also keeps family and physicians in the loop, making sure caregivers are updated regularly and informed of any changes in a patient’s
status. “It’s reassuring, not just for our patients and families, but also for our Home Health clinicians who transition their patients’ care to us – they all
like knowing there’s a safety net.”

HomeCare, Inc.
So they can be empowered and connected
Remaining independent and healthy at home may take a bit of support. HomeCare, Inc. (HCI) has improved the ability for people to remain safe, healthy and
comfortable in their own homes – providing services such as homemaking and personal care attendance – and in playing a critical role in home healthcare.
In 2013, we launched Health Connections with trained 240+ Home Health Aides to properly identify and observe possible signs and symptoms related
to high-risk conditions to help avoid major setback or hospitalizations by re-engaging medical care if needed. Reporting red flags of a possible problem we
can notify those managing the patient’s care – case workers, family members, patient’s physician – to appropriately activate a care plan to meet the emerging
need. Health Connections is a system that keeps patients and those caring for them connected.

Chronic Disease Self Management Program
We brought an innovative program designed to teach people techniques to manage pain and physical symptoms of chronic illness, including coping
techniques that help with stress, isolation, frustration and fatigue that often accompanies it. Developed at Stanford University Medical School, these
6-week workshops are offered in the community setting by 4 licensed “lay leaders”.

Caring for those with Alzheimer’s
We began training HCI aides with the Alzheimer’s Association’s Habilitation
Training program to equip them with the expertise, background and techniques
to care for those with varying stages of Alzheimer’s and dementia. Learning how
to maximizing strengths and abilities and find ways to minimize or work around
limitations, aides have more positive interactions with the unique challenges of
those with this disease. In 2013, 35 aides were trained by four HCI nurses
certified to teach the program.

Looking ahead to 2014
We are training the entire aide staff to successfully care for those with
Alzheimer’s using therapy, communication methods, daily living and activities.
These techniques help manage behaviors as well as keep patients and staff safe.
We will expand the roll-out of the Health Connections Program to
additional communities.
We will continue serving elder health and wellness programs and clinics
through councils on aging, senior centers and assisted living communities.

Healthcare today
In the evolving healthcare system, our home care professionals have the unique perspective allowed by seeing patients in their own homes on
a regular basis. This enables them to identify and assess changes or clinical concerns to relay back to the patient’s physician and consult for
appropriate action. In this way, our staff serves as a vital resource and trusted partner by patients and others throughout the healthcare system.
As all providers in today’s healthcare industry, we are adapting to recent changes and challenges that require us to reduce costs while increasing
effectiveness and value of the care we provide. We are proud to be working collectively with our healthcare partners to do this. Together we are
aligning our efforts towards seamless care, better outcomes, greater cost efficiency and improved patient satisfaction. Collaborating with hospitals,
physician groups, and elder service organization, we aim to reduce confusion for patients, create consistent care plans and guidance, provide
better follow-up, and enable patients to receive the right kind of care in the most appropriate settings.

Strength of Collaboration
Communication and cooperation along every step of the way
At the center of our agencies are teamwork, collaboration and communication. That includes the cooperation between our staff and supporting clinical

Innovations and technology
While the agencies of Home Health Foundation have used electronic medical records for the past twelve years, with sophisticated reporting software now
in place, we are able to use clinical data in a powerful new way. Case managers have actionable information that helps to proactively impact care, reduce
hospitalizations, improve the effectiveness of each visit by our clinicians, and enable our patients and their care teams to make more informed decisions.

Out in front on the Massachusetts HIway
The collective of Home Health Foundation agencies was the first home health organization in the state to bring together a health information exchange
(HIE) as part of the Mass HIway initiative. Working to share clinical information across the many places our patients receive care, the Home Health
Foundation brought together Lawrence General Hospital, Greater Lawrence Community Health Center, and Pentucket Medical Associations to begin
working on sharing patient information so that we are each providing more effective care for patients. This was the first of several health information
exchanges that were created in 2013 and will continue developing into the future.

This financial support allows us to meet the clinical needs of our patients and lead the innovations
which are improving healthcare.

A commitment to the community
True to our mission, we provide vital home health services to all people regardless of ability to pay. Among the cities and towns served are some facing the
highest rates of poverty and unemployment in the state of Massachusetts. We remain committed to these residents who rely on our organization and other
human service groups for their basic needs.

Helping people move forward
In addition to providing support for those who had a loved one cared for by Merrimack
Valley Hospice, we respond and provide bereavement and coping counsel to those from
throughout the general public, including children from public schools.
In 2013, more than 183 support groups were available in Massachusetts, New Hampshire
and Maine and our bereavement counselors responded to more than 475 requests for
assistance. We offered 33 workshops and presentations for the community to learn
positive ways to cope with grief and loss.

Candles of Remembrance
Each year an open invitation welcomes people to a candle lighting ceremony to honor
the lives of loved ones lost while sharing the warmth of family and friends. Following
the outdoor candle lighting ceremony and names read aloud, guests unite at an indoor
reception with refreshments and sharing.
This event brings comfort and peace each year to many who may have been struggling
with the challenges of a winter holiday season without a loved one. Being able to come
together as individuals touched by a common thread, Merrimack Valley Hospice and
York Hospital Hospice welcomes the community to gather in a special way to honor
memories and also to participate in a tradition that helps people more forward.

Encouraging clinical excellence in community
As a partner in caring, we offer continuing education opportunities to the nurses
and social workers who care for those in our communities. In 2013, we provided
28 CEU programs to advance the training of more than 660 professionals outside
our organization who provide services to patients in a variety of residential settings.

Created by a passionate group of individuals touched by the work of Merrimack Valley Hospice and Merrimack Valley Hospice
House, Friends of Merrimack Valley Hospice is a newly founded, all-volunteer service organization. Support comes through
yearly activities that raise both funds and awareness of Merrimack Valley Hospice House as a unique resource available to the
community. Membership is open to anyone in the community.
Join the Friends of Merrimack Valley and become a part of fun events and opportunities throughout the year, volunteering time if
you are interested and able. Funds raised by the group’s efforts supports things like charitable care at Merrimack Valley Hospice
House, bereavement programs, and other services that add to the quality of life of patients and their families moving forward.

Volunteering
Volunteering comes in many forms – as time spent with patients on hospice and their
families, as a volunteer at an event. Hosting a third-party fundraiser in support of
any of our agencies is another opportunity to volunteer your time and talent to make
a positive impact. Volunteers are important to our work and help extend the range of
support to the greater community. Friends of Merrimack Valley Hospice is a newly
formed volunteer group that helps increase awareness and support.

Legacy of Leading
Legacy of Leading pays tribute to those who support the health and well-being of
our community as well as a commitment to the mission of our agencies. Proceeds
from this annual event support all three of the agencies of Home Health Foundation.
This year’s honorees were Massachusetts State Representative Brian Dempsey, and
Rosanne DiStefano, president of Elder Services of the Merrimack Valley who each
have supported positive changes in our region.

Food, Wine and Beer Pairing
Food, Wine and Beer Pairing is an annual “destination” event as guests enjoy a festive
theme-based atmosphere, activities, cuisine and beverages while participating in silent
auctions and raffles. Many of the region’s finest retailers and restaurants support this
event with donations and sponsorships that make the event such as success each year.
More than $110,000 was raised with 2013’s “Off to the Races” event in support
care and services of Merrimack Valley Hospice.

Ways to Give
Thousands of people are touched by the generosity shared with our agencies, and support
can come in many forms. To discuss any of these options or for questions, contact Donna

Cash Contributions
Cash contributions are fully tax deductible. Many companies match the gifts of their
employees – check with your employer to see if your gift can qualify.
Gifts of Appreciated Property
You can donate appreciated property (stocks, bonds and real estate) to support any of our
agencies. When you donate property that you have held for more than a year, your gift is
exempt from capital gains taxes. Your charitable deduction is based on the fair market value
of the asset at the time of your gift.
Vehicle Donations
Donate for Charity, our recycling partner, offers an opportunity to support our agencies
by donating a car, truck, van, boat, RV, or motorcycle by calling 866-392-4483. Donors
receive a tax deduction of the auction sale price of the vehicle or $500, whichever is the
greater amount.
The Leaders in Home Health and Hospice Care
A simple way to support is through a bequest in a will or living trust. Any part, percentage,
or component of estate or trust assets can be designated to Home Health Foundation or
one of its agencies and will help to reduce inheritance taxes.
Founded in 1895 to provide care to families who
worked in the local textile mills, we are now one
of the largest home health and hospice agencies
in the region. Our trained professionals and
paraprofessional staff provide the highest
quality, compassionate medical and supportive
care to more than 3,500 people on any given
day from our main office in Lawrence and from
offices in Newburyport, Lowell, Peabody, and
North Hampton, NH and at our hospice house
in Haverhill, MA.

You can also use cash and appreciated property to establish a charitable remainder trust
or charitable gift annuity that will provide income and tax benefits to you, while benefiting
one of Home Health Foundation’s agencies.
Bequests
There are many ways to get involved and give of your time and talents. Hold your own
fundraiser to help support the mission of our agencies; volunteer in a patient care role,
office setting, or for special events; or help raise awareness and funds through the newly
founded Friends of Merrimack Valley Hospice group.
