The Zone
• Attend the program 2½ days each week, but may
vary depending on student needs.
• Receive adaptive and life management tools which
may include planner, résumé and other life tools.
• Pay $25 for a bus pass and $5 for a Parks & Recreation pass. In addition, incidental money is needed
for recreation and leisure activities.

Transition Program
For young adults with
moderate to severe needs
ages 18–21 years
Community Connections . . . a place of caring
We help students learn about independent life-style choices based on their strengths, interests and preferences.
Leisure &
Recreation
Whether they go on to live on their own or not, students
learn skills to increase independence, make friends, and be
active in the community. In addition to all of this, students
and staff at Community Connections love to have fun!
Transition
Services
Students in the Community Connections program
transition into their next phase of life by gradually increasing responsibility in order to gain independence.
Services students receive include:
• Building daily living skills such as food planning, shopping and money management
• Riding public transportation
• Developing organizational skills, including keeping a weekly schedule
• Training in work-related skills and job coaching
• Developing friendships

Students have access to many resources
including Larimer County Workforce
Center, CSU music therapy and occupational therapy services, Fort Collins Parks
& Recreation, Division of Vocational
Rehabilitation (DVR), and more!
Job Skills &
Employment
Students learn skills through participating in a
variety of volunteer opportunities throughout the
community.
Volunteer opportunities
include Meals on Wheels,
Poudre River Libraries,
Habitat for Humanity, the
Food Bank, HELP International, Animal House, etc.
In addition, internships are
available at Colorado State
University including the
mail room and gardens,
and through local businesses such as Albertsons,
Jax, Marriott, Taco Bell,
etc.
Transition is a process
of gradually increasing
independence and
becoming a member of
our local community!
Students participate
in adaptive recreation
activities throughout the community including Fort
Collins Parks & Recreation, EPIC and Northside Aztlan
Center, and are exposed to Taekwondo through a
local martial arts center. In addition to physical activities, students take art classes through CSU, attend
sporting events (i.e., Rockies and Nuggets games),
and attend events at the Lincoln Center.
Graduation &
Ceremonies
Upon completion of the Community Connections
Program, students receive a Certificate of Completion and celebrate their transition through a Send-off
Graduation from Community Connections.
Note: PSD Graduation may be available to some students who have met high school graduation requirements through their home high school.
