Geo-Hydro Engineers, Inc. has completed the requested Phase I Environmental Site Assessment at
the above referenced location. The primary purpose of the Phase I study was to identifY recognized
environmental conditions at the subject site. The scope of services for this project was outlined in
our proposal number 4102-6. This report was prepared for use by the R. J. Wood and Company
and the City of Cedartown, and may not be used or relied upon by any other party without our prior

SUMMARY
The subject site is located approximately 2100 feet east of the intersection Davis Road and Main
Street (0. S. Interstate 27) in Cedartown, Georgia. The subject site consists of approximately
204 acres located south of Davis Road. The property is currently wooded and undeveloped land.
The approximate subject site location is shown on a street map (Figure 1, Appendix I) and a portion
of the Cedartown East, Georgia, United States Geologic Survey Topographic Map (Figure 2,
Appendix I).
A site reconnaissance of the property and surrounding area was performed by Jeanne M. Buck on
December 14, 1995. In addition to the site reconnaissance, we interviewed a number of persons
familiar with the subject property and surrounding properties, reviewed regulatory listings, aerial
photographs, and a number of other data bases concerning the subject property and surrounding
properties ..
This Phase I Environmental Site Assessment has revealed no evidence of recognized environmental
conditions in connection with the subject property with the following exceptions:
•
One empty IS-gallon drum and one empty 55-gallon drum were observed on the subject site.
Markings on the I 5-gallon drum indicated the drum once contained oil. The 55-gallon drum was
unmarked.
•
Evidence of improper disposal of household trash, construction materials, a heater, and several
tires was observed on the north-central portion of the subject site. Debris was observed in piles
with trees and soil along the dirt road leading south from Davis Road and scattered on the
ground surface in the vicinity of the former residential buildings on the subject site.
•
Improper disposal of household-generated waste was observed along the dirt road running east
from the former residences. The waste appeared to contain glass, plastic.

2.1
INTRODUCTION
Purpose
The primary purpose of the Phase I environmental site assessment (PESA) was to evaluate the
potential for present or past contamination of soil or groundwater at the site with hazardous
substances. This was accomplished by performing a site reconnaissance, reviewing several data

bases concerning present and past site activities, and reviewing government lists of sites in the area
with known hazardous substance activity.
2.2
Special Terms and Conditions
The scope of services for this project was outlined in our proposal number 4102-6. This report and
our observations are intended solely for the benefit of the R. 1. Wood and Company and the City of
Cedartown, and may not be used or relied upon by any other party without our prior written consent.
The findings and opinions conveyed via this PES A report are based on infonnation obtained from a
variety of sources enumerated herein, and which Geo-Hydro Engineers believes are reliable.
Nevertheless, Geo-Hydro cannot and dJes not guarantee the authenticity or reliability of the
infonnation it has relied upon.
2.3
Limitations and Exceptions of Assessment
Although we perfonned a reconnaissance at the subject site to observe for hazardous substance
activity, it is possible that evidence of such activity exists at the site which is not readily visible.
Evidence of hazardous substance activity may exist below the ground surface, below a thick growth
of vegetation, or at some other location where the view is obstructed or access is prevented. In
general, we expended reasonable effort to perfonn observations in these areas. However, we did not
perfonn excavations, removal of vegetation, or other effort to obtain visual access to these areas.
This study did not include any sampling or testing of soil, groundwater, radon, methane, asbestos, or
other materials at the site. In short, the site reconnaissance was intended to investigate for conditions
which indicate to us the possible presence of hazardous materials at the site. Such reconnaissance
does not definitively demonstrate the existence of, or the absence of, hazardous substances.
This report is subject to the limitations in ASTM E-1527, Standard Practice for Environmental Site
Assessments: Phase I Environmental Site Assessment Process.
2.4
Limiting Conditions and Methodology Used
A site reconnaissance was perfonned to observe for indications that hazardous substances or
hazardous wastes might have been stored on, disposed of, or utilized in bulk quantities on the
property. Hazardous substances and hazardous wastes are many and varied. They include mineral
substances such as asbestos; inorganic substances such as metals, metal salts, and acids; and organic
chemicals such as fuels, pesticides, polychlorinated biphenyls (PCB's) and solvents.

The visual reconnaissance was made to observe for the presence of the following specific conditions
(hereinafter the "conditions"):
• evidence of spills at the ground surface
• areas of dead, dying, or distressed vegetation
• unnaturally stained or discolored surface soils
• unnaturally colored or milky seepage or seepage
causing obvious precipitation of solids
• oil slicks or scum on surface waters
• chemical odors
• above ground storage tanks
• vertical pipes, casings, or other evidence of underground storage tanks
• water wells
• evidence of open dumping, trash, or other debris
• barrels or waste storage areas
• electrical transformers
We obtained regulatory listings from the Envirorunental Protection Agency (EPA) and the Georgia
Envirorunental Protection Division (EPD) which identify sites with reported hazardous substance
activity. The listings were reviewed to determine if the subject site or sites in proximity to the
subject site were included on the listings.
We reviewed aerial photographs at the University of Georgia Science Library in Athens, Georgia.
These vertical photographs were taken by the US. Government and generally have scales ranging
from 1:12,000 to 1:20,000.
A chain-of-ownership search was provided by York, McRae and York, Attorneys at Law, of
Cedartown at the request of Mr. R. 1. Wood.

3.1
SITE DESCRIPTION
Location and Legal Description
The subject site is located approximately 2100 feet eas~ of the intersection Davis Road and Main
Street (0. S. Interstate 27) in Cedartown, Georgia. The subject site consists of approximately

Site Vicinity Characteristics
The subject site is located in Cedartown, Georgia, approximately two miles northeast of downtown
Cedartown. The Central of Georgia Railroad runs along the western property line of the subject site.
U. S. Highway 27 By-Pass borders the site to the southwest. Uses of the properties within one-half
mile of the subject site include conunercial, residential, agricultural, and undeveloped wooded land.
3.3
Site Improvements
There are no buildings currently located on the subject site. The site is vacant land covered by trees
and dense vegetation. Several dirt roads ~ross the site. The remains of a former residence and
construction material debris are located on the north-central portion of the property. A creek crosses
the central portion of the site, flowing in a westerly direction. Plates I through 4, Appendix I,
present views of the subject site.
3.4
Current Uses of the Property
The site is currently vacant land covered by trees and dense vegetation.
3.5
Past Uses of the Property
Aerial photographs indicate that prior to 1980, several small residential buildings occupied the
north-central portion of the subject site. Agricultural fields occupied the western and northeastern
portions of the subject site. The remainder of the site has been \Nooded since at least 1960.
3.6
Current and Past Uses of Adjoining Pt·operties
Land within one-half mile of the subject site includes conunercial, residential, agricultural, and
undeveloped land. Utilization of the properties adjoining the subject site includes undeveloped land
to the north, and south; residential and agricultural land to the east; and a railroad right-of-way and
undeveloped land to the west.
The adjoining property to the north is undeveloped wooded land. Aerial photographs indicate the
property was agricultural land prior to 1980. Two residences occupied a portion the property north
of the intersection of Davis Road and Lake Creek Road. The eastern portion of the property does
not appear to be upgradient from the subject site. The western portion of the property appears to be
upgradient from the subject site.

The adjoining property to the south is undeveloped land. Aerial photographs indicate the adjoining
property to the south was undeveloped, agricultural, and residential prior to 1980. The northern
portion of the property was undeveloped wooded land. The southern portion of the property was
agricultural with several residences, barns, and small outbuildings fronting on John Hand Road. The
property does not appear to be upgradient from the subject site.
The adjoining properties to the east are residential and agricultural. A residence fronting on Davis
Road has occupied the property to the northeast since at least 1972. Prior to development, the
property to the northeast was agricultural land. Several pastures occupy the adjoining properties
east and southeast of the subject site. The properties to the east and southeast have been agricultural
since at least 1960. The adjoining properties to the east appear to be upgradient from the subject
site.
At the time of site reconnaissance, two abandoned underground storage tanks (UST's) were observed
on the residential property to the east of the subject site. The UST's were stored above ground and
did not appear to have been used at that location. The UST's were rusty. Evidence of leaks or
surface staining was not observed. Plate 5, Appendix r, presents a view of the UST's.
The adjoining property to the west is occupied by the Central of Georgia right-of-way. The property
west of the railroad is undeveloped land. The railroad has occupied the adjoining property since at
least 1960. Prior to 1980, the property west of the railroad \\'as agricultural land. The property
does not appear to be upgradient from the subject site.

4.1
RECORDS REVIEW
Regulatory Listings
We obtained regulatory listings from the Environmental Protection Agency (EPA) and the Georgia
Environmental Protection Division (EPD) which identify sites with reported hazardous substance
activity. A copy of this information is included in Appendix III.
We did not find a reference for the subject site in the regulatory agency listings reviewed for this
report. We also did not find a reference for any facilities within the ASTM minimum search
distances in the regulatory agency listings reviewed for this report.

4.2
Physical Setting Sources
4.2.1 Review of Topographic Maps
The 1967 (1980 photorevised) U.S. Geological Survey (USGS) topographic map for the Cedartown
East, Georgia, quadrangle was reviewed to provide information about the topography of the site and
surrounding area. A portion of this map is presented as Figure 2, Appendix I.
4.2.2 Review of Aerial Photographs
Geo-Hydro Engineers, Inc. reviewed a chronology of aerial photographs at University of Georgia
Science Library in Athens, Georgia. The earliest available aerial photograph including the subject
site was taken in 1960. The reviewed photographs' date and number are:

In the 1960 aerial photograph, Main Street (Georgia Highway 1), Lake Creek Road, Davis Road,
John Hand Road, and the Central of Georgia Railroad are evident. The western portion of the
subject site appears agricultural. The northeast corner of the subject site is agricultural. A dirt road
is visit" on the site leading from Davis Road to a cleared area near the southern property line.
Several residential buildings are visible along the northern portion of the dirt road. The adjoining
property to the north is agricultural with two residential buildings north of the intersection of Davis
Road and Lake Creek Road. The adjoining property to the east is wooded. Two areas of
agricultural land are visible to the northeast and southeast of the subject site. The northern portion
of the adjoining property to the south is undeveloped wooded land. Several residential buildings,
barns, and agricultural fields occupy the southern portion of the adjoining property south of the
subject site. The adjoining property west of the subject site is agricultural. Land within one-half
mile includes agricultural, residential, and undeveloped land.

4.3
Several agricultural tracts
Historical Use Information
A chain-of-title was provided by York, McRae and York as requested by Mr. R. J. Wood. The
names of previous owners of the property can provide indications of past site usage and possibly
previous hazardous substance activity at the site. This information is summarized in Table 1,
Appendix II.
Based solely on the names mentioned in this information, we found no indication of past storage or
disposal of hazardous substances at the site.

5.1
INFORMATION FROM SITE RECONNAISSANCE AND INTERVIEWS
Hazardous Substances in Connection with Identified Uses
We observed no indications that hazardous substances or petroleum products are actively used on
the subject site.
5.2
Drums and Unidentified Substance Containers
One empty IS-gallon drum was observed along the eastern property line on the northeast portion of
the subject site. Markings on the drum indicated the drum once cor.tained oil. The drum appeared
rusty and flattened. There was no evidence of surface staining or stressed vegetation in the vicinity
of the drum. Plate 6, Appendix I, presents a view of the drum.
We observed one empty unmarked 55-gallon drum on the north central portion of the subject site.
The munarked drum was rusty and crushed. The drum was piled with construction debris,
household waste, tree branches, and tree trunks. The ground surface in the vicinity of the drum did
not appear to be stained. Plate 7, Appendix I, presents a view of the drum.

Storage Tanks
Visual evidence of underground storage tanks (UST's) such as pumps, fill ports, or vent pipes was
not apparent at the time of the site inspection. In addition, the regulatory review did not indicate the
presence of a UST on the subject site.
5.4
Indications of PCB's
No transformers were observed on the subject site.
5.5
Indication of Solid Waste Disposal
No solid waste disposal dumpsters were observed at the subject site.
Evidence of improper disposal of household trash, construction materials, a heater, and several tires
was observed on the north-central portion of the subject site. Debris was observed in piles with trees
and soil along the dirt road leading south from Davis Road. Debris was also observed scattered on
the ground surface in the vicinity of the fonner residential buildings on the subject site. The debris
appeared to primarily be building material from the demolition of the former residential buildings on
site and miscellaneous items from inside the fom1er residences. Plates 8 through 11, Appendix I,
present views of the debris.
Improper disposal of household-generated waste was observed along the dirt road running east from
the former residential buildings. The waste appeared to contain glass, plastic, paper, and metal. The
waste did not appear to contain evidence of hazardous substances. Plate 12, Appendix I, presents a
view of the debris.
5.6
Physical Setting Analysis
The subject site is located along two east-west trending ridges at approximately 800 to 960 feet
above mean sea level (MSL). The surface of the subject site generally slopes toward a creek in the
center of the site and to the west with a maximum vertical relief across the entire site of
approximately 160 feet. Runoff from the subject site and the drainage area in the vicinity of the
subject site flows generally west across the site toward an unnamed tributary of Cedar Creek.
The subject site is located in the Appalachian Valley and Ridge Province of Georgia. Based on
review of geologic maps, it appears that the site is underlain by a geologic unit known as the Knox
Group. This formation includes sedimentary rocks such as dolomite, sandstone, and limestone.

Based on review of the Georgia Department of Natural Resources Ground-Water Pollution
Susceptibility Map of Georgia, the site is located in an area with a DRASTIC index of greater than
181. Areas having a rating of greater than 181 are considered to have a relatively high pollution
susceptibility.
5.7
Wetlands and Flood Plain Records Review
We reviewed the National Wetland Inventory maps prepared by the U.S. Department of the Interior,
Fish and Wildlife Service. These maps provide identification of wetlands superimposed on USGS
topographic maps. Based on a review of the Cedartown East, Georgia, Quadrangle, there are no
wetlands identified on the subject site.
We reviewed Community-Panel Number 130256 0075 A of the Federal Emergency Management
Agency, Flood Insurance Rate Map (FIRM) for Polk County which includes the subject site. This
map indicates that a small portion of the southwest corner of the subject site is within the 100
year flood zone. The approximate flood zone boundary is shown on Figure 3, Appendix I.
6.0
FINDINGS AND CONCLUSIONS
This Phase I Environmental Site Assessment has revealed the following evidence of recognized
environmental conditions in connection with the subject property.
One empty 15-gallon drum and one empty 55-gallon drum were observed on the subject site.
Markings on the 15-gallon drum indicated the drum once contained oil. The 55-gallon drum was
unmarked. Both drums appeared rusty and flattened. There was no evidence of surface staining or
stressed vegetation in the vicinity of the drums.
Evidence of improper disposal of household trash, construction materials, a heater, and several tires
was observed on the north-central portion of the subject site. Debris was observed in piles with trees
and soil along the dirt road leading south from Davis Road and scattered on the ground surface in the
vicinity of the former residential buildings on the subject site. The debris appeared to primarily be
building material from the demolition of the former residential buildings and miscellaneous items
from inside the former residences.

It is our opinion, based on this Phase I envirorunental site assessment, that the potential for past
contamination of soils or groundwater at the subject site with hazardous materials is relatively low.
However, it is our recommendation that the household debris and waste be removed from the
property for proper disposal. If during excavation of the waste material suspicious soil staining or
odors are encountered, additional soil and groundwater testing may be necessary.

EXPLANATION OF FINDS DATA BASE LISTING
FINDS is an acronym for "Facility INDex System", which is a computerized inventory of sites or
facilities monitored or regulated by the Environmental Protection Agency (EPA). The system serves as
an index to other EPA Program Office databases or records. Most sites or facilities listed in FINDS
relate to permit applications, inspection reports, or Superfund sites, which does not necessarily mean
that they pose an environmental or health threat. A facility is identified by a unique EPA identification
number, and its "FACILITY LOCATION NAME". Each listed facility contains one or more X's" to
indicate the facility's "Source Indicator". The Source Indicator points to those corresponding EPA
Program Offices, such as Air, Water, and Waste, which have some responsibility for a site or facility
contained in FINDS. Users should be aware that the FINDS "FACILITY LOCATION NAME" as
used by EPA may be different from a more often used or common name.

and inventory system of RCRA hazardous waste handlers. Handlers can be characterized as fitting one

EXPLANATION OF ERNS LISTING
ERNS or the Emergency Response Notification System is intended to standardize the process of
collecting, docwnenting, and analyzing nationwide data on reported releases of oil and hazardous
substances. In addition, ERNS records any follow-up gathered by the Environmental Protection Agency
(EPA) or the United States Coast Guard (USCG). This database is necessary to meet provisions of the
revised National Oil and Hazardous Substance Pollution Contingency Plan (NCP) and to provide EPA
headquarters and region management and staff with infonnation on the nature and types of releases
occurring on a nationwide basis. The ERNS database integrates both initial notification information of
releases of oil and hazardous substances and additional follow-up infonnation for those incidents
through a verification module. Infonnation about a release is received from the reporting parties by one
of three entities, the National Response Center (NRC), the EPA, or the USCG. The main ERNS
database is managed by the DOT and The Transportation Systems Center in Cambridge, Massachusetts
and is updated weekly by data transfers from EPA and USCG. ERNS includes infonnation such as,
case nwnber, possible responsible party, material type, quantity, location of incident, spill date, action
taken, etc.

TRIS stands for the Toxic Release Inventory System. It is the computer system used by the
Environmental Protection Agency to track the annual release of toxic chemicals into the environment. It
serves as a management tool that is used by the EPA to simplify the collection and organization of toxic
chemical release data received from industries regulated under the Superfund Amendments and
Reauthorization Act (SARA). TRIS contains infonnation about the release and transfers of over 300
different toxic chemicals. This includes information such as, CAS number and chemical name,
company name, amount of the chemical released or transfered off-site, medium released into (air, water,
land), Standard Industrial Classification (SIC) code, activity and use of the toxic chemical, technical
and public contacts, waste treatment infonnation, and the locations to which chemicals are transferred.
The TRIS data provided by the EPA is for fiscal year 1990.

EXPLANATION OF pes DATA BASE LISTING
The Permit Compliance System (PCS) is the data system of the National Pollutant Discharge
Elimination System (NPDES). As such, it contains facility information, i.e. Name, Address, County,
Cognizant Official's name, Telephone number, Receiving Stream, Latitude/Longitude, and SIC Code,
among others. PCS also contains information about NPDES permit history and current requirements,
including effluent limitations and compliance schedules. All discharge monitoring results are entered
into PCS, as are all enforcement actions. PCS evaluates discharge monitoring results against effluent
limits to determine violations and detects when schedule milestones are not completed on time. The
system determines whether Significant Noncompliance exists and produces the Quarterly
Noncompliance Report automatically.
