This section describes the MIT program and what it provides to associates who are off work due to a long-term medical condition. MIT Program Overview If you are unable to work due to a medical condition, you may be eligible for a Leave of Absence of up to 104 weeks (two years). The MIT program begins after you have been on an approved Leave of Absence for 26 weeks (six months) and, provided you remain eligible, will continue up to a maximum of 104 weeks. If you are unable to return to work after 104 weeks, your employment at Honda will end. During this program: A team from Honda will coordinate and communicate with you concerning your medical treatment You will continue to be eligible for Honda benefits for which you would be eligible while on a leave. See the Leave of Absence portion of the Life Events section Program Requirements Participation in the MIT program begins after an approved Leave of Absence for 26 weeks (six months) due to your own medical condition. Upon entering the program, you are required to file for Long-Term Disability (LTD) benefits. A member of Honda’s Disability Group will contact you to explain the MIT program. Subsequent meetings will be held with you every six months to monitor your progress and ensure that you have access to the appropriate care and services. As a participant in MIT, you are required to sign a release to allow a member of Honda’s Disability Group to interact with your treating physician(s) and to actively participate in the program. Failure to do so will result in denial of your continued Leave of Absence. If by 52 weeks on a Leave you have not filed for LTD benefits or LTD benefits have been denied and are no longer under appeal, your employment will end at that time, and you will not qualify for postemployment MIT benefits (as described later in this section). If you qualified for LTD benefits at the 52 week mark, and LTD benefits are later denied and are no longer under appeal, your employment will end at that time and you will not qualify for post-employment MIT benefits. Returning to Work at Honda Hopefully, during your participation in the MIT program, you will seek to return to work at Honda. To ensure your safe return to work, you will be scheduled for, and must successfully complete, a medical fitness-for-duty evaluation. Following a return to work at Honda, you will remain in MIT for a 60-day monitoring period. Your participation in MIT will not end until you have returned to work for 60 consecutive calendar days without a medical Leave of Absence for the same or different medical condition. Other Services and Programs | HAM/EGA/HNA 203 January 1, 2014 Program Timeline 26 Weeks The MIT program begins after you have been on a medical Leave of Absence for 26 consecutive weeks. As a requirement of the program, you must apply for Long-Term Disability (LTD) A member of Honda’s Disability Group will contact you to schedule an initial MIT meeting. Future meetings will occur every six months, either until you return to work for 60 consecutive calendar days or you exhaust your time in the MIT program. As a requirement of the program, you must actively participate in these meetings and the interactive process with Honda and your treating physician 52 Weeks A member of Honda’s Disability Group will conduct a second MIT meeting with you If you have not filed for LTD benefits, or if LTD has been denied and all appeals are exhausted, your employment with Honda will end and you will not qualify for post-employment MIT benefits 78 Weeks A member of Honda’s Disability Group will conduct a third MIT meeting with you If, at any time after 52 weeks of leave, LTD benefits are denied and no longer under appeal, your employment will end, and you will not qualify for post-employment MIT benefits 104 Weeks If you have not been able to return to work at Honda, your participation in MIT will end and your employment with Honda will end. Your separation from employment does not impact your right to continue to receive LTD or to appeal the denial of LTD Certain Honda post-employment benefits may apply for you and your family Post-Employment MIT Benefits If, as of your last day worked, you had 12 months or more of service with Honda but were unable to successfully return to work before the expiration of time in the MIT program, you may be eligible to receive post-employment benefits fully paid by Honda for 12 months. Your eligibility for postemployment benefits is also dependent upon: Your compliance over the entire course of the MIT program and participation in meetings and Timely execution of a separation agreement and general release These post-employment benefits (under the Honda Benefits Plan) include: Medical, dental, vision and prescription drug coverage for associates and their dependents* Basic life and accident insurance coverage for associates Optional life insurance coverage may be continued while receiving Honda post-employment benefits, if the associate pays the premiums When Honda benefits end, associates may elect COBRA continuation coverage at their expense for the period allowed under COBRA (see the COBRA section for information). Associates also may be able to participate in Honda’s Retiree Medical Plan. * If you are Medicare eligible or have applied for Social Security benefits, your status may impact the general base-level health insurance benefits that you may receive. Other Services and Programs | HAM/EGA/HNA 204 January 1, 2014 Separation from Employment and COBRA If you are unable to return to work after 104 consecutive weeks of leave, your employment with Honda will end. Upon separation, your Honda benefits will continue until the end of the month in which your employment ends. Associates who do not qualify for post-employment MIT benefits may continue benefits through COBRA at their own expense, if they choose, for the period allowed under COBRA (see the COBRA section for more information). Associates who qualify for post-employment MIT benefits will be offered Honda general base-level health insurance benefits at no cost for 12 months through a separation agreement and general release.* Benefits include: Medical Dental Vision Prescription drug Basic life insurance Accident insurance Thereafter, associates may continue benefits through COBRA at their own expense, if they choose, for the period allowed under COBRA (see the COBRA section for information). If eligible, associates may elect the Honda Retiree Medical Plan. * If you are Medicare eligible or have applied for Social Security benefits, your status may impact the general base-level health insurance benefits that you may receive. Honda Retiree Medical Plan After Honda post-employment benefits end, eligible associates may elect coverage in the Honda Retiree Medical Plan. To be eligible, associates must: Be eligible for LTD benefits and As of their MIT last day worked, have at least 10 years of service, and either be: Age 50 or older when employment ends OR Regardless of age, be approved for Social Security Disability Insurance Associates who elect Honda Retiree Medical Plan coverage will receive credits for medical coverage based on length of Honda service at the time of separation of employment. For more information about this plan, see the Other Retirement Benefits section Other Services and Programs | HAM/EGA/HNA 205 January 1, 2014 Workers’ Compensation To help you understand the process for filing Workers’ Compensation claims, this section outlines the steps you should take if you have a work-related accident, injury or disease. Introduction to Your Workers’ Compensation Benefits Note Most of the information contained in this section is based on state law or Honda policy and procedure, which is subject to change at any time. All associates of Honda are covered by Workers’ Compensation laws and regulations, which provide protection in case of a workrelated injury or illness. Honda has been granted self-insured status by the Bureau of Workers’ Compensation (BWC), which allows for more efficient claims processing and direct payment of compensation and medical benefits to and on behalf of an injured worker. This program is administered in accordance with the laws of the state. Workers’ Compensation benefits compensate associates for time off work necessary and related to an allowed, work-related accident, injury or disease. Workers’ Compensation benefits may also include reimbursement or payment for related medical and prescription expenses. The Paper Trail The following forms are available from the Workers’ Compensation Department, the Medical Department or from the Bureau of Workers’ Compensation (BWC). Step 1: Report Your Situation Any work-related illness or injury that occurs on Honda premises or during an activity required by your employment must be reported immediately to the appropriate Honda representative or supervisor. Any injured associate must go or be taken to the nearest In Plant Medical facility to have the injury assessed and the proper medical treatment arranged. In a serious or lifethreatening emergency, the emergency response system should be used. In all cases, an accident/incident investigation report must be completed by the associate and appropriate Honda representative. Requesting a Leave of Absence If you need to take time off work for your alleged work-related condition, you must contact Sedgwick at 1-888-538-2732 to request a Leave of Absence. Step 2: File a Workers’ Compensation Claim If you choose to file a claim for Workers’ Compensation benefits, applications are available from the Workers’ Compensation Department, In Plant Medical facility or the Bureau of Workers’ Compensation. Associates in these departments can give you information and assistance when filing a claim or completing documents related to a claim. By filling out your claim form completely and providing all information requested, you may avoid delays in the processing of your claim. You will also be asked to complete and sign a form authorizing your physician to release your past medical information to Honda. This will help Honda evaluate your claim and gather all the information needed to determine whether Workers’ Compensation benefits are available to you. Reactivating a Claim If you have and report subsequent health problems in accordance with state law and the problems are determined to be related to a prior claim, you may request to reactivate a prior Workers’ Compensation claim. To reactivate your claim, you must complete and sign a C-86 Motion Form and include any medical evidence that supports your request for reactivation. If the reactivation is accepted by Honda, medical bills and compensation related to and necessary for the allowed conditions will be paid under the prior claim If the reactivation is denied by Honda, you will be notified of the reasons for denial. You may then request an Industrial Commission hearing for further consideration. See “About the Claim Hearing Process” on page 212 Other Services and Programs | HAM/EGA/HNA 207 January 1, 2014 Step 3: Claim Review Process After you submit your forms, the Workers’ Compensation Department may request past medical records from your treating physician and any physician who provided care for your condition to begin evaluating your claim. The claim review process can take up to 30 days. Processing may be delayed if your forms or medical records are incomplete. To avoid delays, you should ask your treating physician to forward copies of all related notes, records and test results to Sedgwick - Honda at P.O. Box 14669, Lexington, KY 40512-4669, fax: 1-877-475-6170. If additional information is needed, you may be contacted by a member of the Workers’ Compensation Department. When all necessary information regarding your claim has been received and reviewed, Honda will notify you by mail as to whether your claim has been certified or not certified. Recognized Treating Physician The BWC allows for only one treating physician for each Workers’ Compensation claim. Requests for payment of compensation and medical bills will only be accepted from the treating physician named in your claim. If your physician determines that you need treatment from another doctor, such as an additional consultation, surgery or other forms of treatment, he or she must obtain prior authorization from the Workers’ Compensation Department. You may change your treating physician on your claim by completing a Notice to Change Physician of Record Form C-23 available from the Workers’ Compensation Department or the BWC. If Your Claim Is Certified Notification that your claim has been certified tells you that the Workers’ Compensation Department has reviewed your claim and found your medical condition(s) to be work-related and eligible for Workers’ Compensation benefits under state law. Upon certification, your Workers’ Compensation claim is sent to Sedgwick for processing. Compensation begins on the eighth calendar day of absence due to your certified work-related condition. If you are off work 14 consecutive days, Honda will pay you for the first seven days of missed work. Regardless of the number of days missed, you are still entitled to medical benefits for expenses reasonably related to the medical conditions allowed in the claim. If compensation benefits are granted, you will receive payment within 21 days of Honda’s receipt of a C84 Form “Request for Temporary Total Disability (TTD)” and a MEDCO-14 “Physician’s Report of Work Ability” verifying your inability to work. Other Services and Programs | HAM/EGA/HNA 208 January 1, 2014 Sedgwick Honda has contracted with Sedgwick to assist in the administration of the Workers’ Compensation program. Sedgwick will file the following claims with the Bureau of Workers’ Compensation: Certified lost time claims (more than seven calendar days off work) Any claim that is not certified Occupational disease claim(s) Any claim specifically requested by an associate to be filed Claims filed with the BWC will receive a claim number. You should keep this number with your files. In addition, Sedgwick does the following on Honda’s behalf: Processes requests for medical treatment Processes payment of medical bills Processes payment of compensation benefits Schedules Independent Medical Examinations About Your Disability Compensation The rate of your compensation benefits will be based on your wages applied to a formula established by the Bureau of Workers’ Compensation. The BWC also establishes minimum and maximum limits for Workers’ Compensation payments. The example below shows how Workers’ Compensation benefits are calculated for different lengths of disability. Temporary Total Disability Length of Disability Rate of Benefits First 12 weeks FWW equals the greater of: Compensation is paid at Wages earned one week prior to the date of injury, excluding overtime or 72% of your Full Weekly The average of wages earned over the last six weeks worked prior to the date of injury, including overtime Wage (“FWW”) Note: Payments may not exceed the maximum rate established by the BWC for the year in which the injury occurred. After 12 weeks AWW is the average weekly wage for the past 52 weeks prior to the date of injury. Compensation is paid at 6623% of your Average Note: Payments may not exceed the maximum rate established by the BWC for the year in which the injury occurred. Weekly Wage (“AWW”) Workers’ Compensation benefits are paid on a biweekly basis and are not taxable. If you are subject to a child support order, the law requires Honda to deduct this payment from your Workers’ Compensation indemnity benefits. Other Services and Programs | HAM/EGA/HNA 209 January 1, 2014 About Your Medical Benefits If your claim is certified and your condition is eligible for Workers’ Compensation benefits, all medical bills and prescriptions related to the condition(s) certified in your claim will be paid according to the BWC’s fee schedule and the Preferred Provider Agreement with Sedgwick. You will not have to pay copays nor will you be balance billed for services received. You may seek care from any physician you choose who is certified by the BWC to provide treatment for injured workers. However, the BWC allows for only one treating physician for each Workers’ Compensation claim. Requests for payment of compensation and medical bills will only be accepted from the treating physician named in your claim. Medical bills for your certified condition(s) should be submitted by you or your physician to the Workers’ Compensation Department. The time limitation for submitting medical bills is two years after the date of service. Medical bills will be processed within 30 days of receipt. Additional Conditions If you develop additional conditions related to your claim, you may complete and sign a C-86 Form requesting coverage under your claim. You should attach related medical documentation to the C-86 Form to support your request. Authorization for Treatment Certain types of treatment require prior authorization from Honda. Your treating physician needs to request this authorization by calling the Workers’ Compensation Department or submitting a C-9 form (Physician’s Report/Treatment Plan). Independent Medical Examinations (IME) Honda or the BWC may require you to undergo an Independent Medical Examination (IME) to get a second opinion regarding your condition. When you receive notice of an IME related to your Workers’ Compensation claim, you must provide Human Resources with a copy of the notice three to seven days in advance of the examination date. If the exam is planned at a time you are scheduled to work, Human Resources will work with you to determine the appropriate amount of time (including travel to and from) for you to be away from work. For Honda-requested IMEs, Honda will pay for the predetermined amount of time necessary to attend the examination. Your attendance bonus, perfect attendance award, bonus sharing, 401(k) Savings Plan and Pension Plan will not be impacted. Honda will provide unpaid time away from work for the predetermined amount of time necessary to attend an exam requested by the BWC. If you use PTO time, your attendance bonus, perfect attendance award, bonus sharing, 401(k) Savings Plan and Pension Plan will not be impacted. If you choose no pay (unpaid time), this will not influence the attendance bonus or perfect attendance award. However, it will impact bonus sharing, the 401(k) Savings Plan and Pension Plan. In both cases, you must notify your supervisor and make proper arrangements for your absence with Human Resources. Please refer to your Associate Handbook for more information. Other Services and Programs | HAM/EGA/HNA 210 January 1, 2014 If You Need Occupational Healthcare Onsite occupational health centers are available with case management services through HealthPartners. The centers are available for treatment of associates who are injured while working. They are staffed with a team of medical professionals qualified to diagnose and treat work-related conditions. HealthPartners is located at Honda’s Support Office (HSO), as well as the In Plant Medical facilities at the Anna Engine Plant, East Liberty Plant and Marysville Auto Plant. HealthPartners is also able to work with your physician of record and assist with your return to work. If Your Claim Is Not Certified If Honda does not certify your claim for all or some conditions, the claim will be filed with the BWC. You will be notified by mail with an explanation of why the claim or condition was not certified. When a claim is denied certification, for all or some conditions a hearing is automatically scheduled by the Industrial Commission to resolve any issues. Before the date of the hearing, you will have an opportunity to provide any missing information or additional evidence for Honda to reconsider your claim for certification. If Honda approves the requested conditions after reviewing the additional information, you will be notified of the decision and Honda will request that your hearing be cancelled. If your claim is still not certified after Honda considers the additional information, you may continue with the claim hearing process. A hearing is not necessary if you ultimately agree with Honda’s decision with respect to conditions allowed in your claim and you may choose to request cancellation of the hearing. For assistance in cancelling a hearing, you can call Honda’s Workers’ Compensation Department at 1-937-645-8366. Short-Term Disability Benefits If the Workers’ Compensation Department denies or contests your disability, you may apply for ShortTerm Disability (STD) benefits. You may apply for STD by calling Sedgwick at 1-888-538-2732. Benefits are not payable for the waiting period required by Workers’ Compensation or if you have reached Maximum Medical Improvement (MMI). See “Short-Term Disability” in the Disability Plans section for more information. If your disability is later approved for Workers’ Compensation benefits, any payments you received while on Short-Term Disability will be reimbursed to Sedgwick through deductions from your Workers’ Compensation payments. Other Services and Programs | HAM/EGA/HNA 211 January 1, 2014 About the Claim Hearing Process You or Honda may request a hearing before the Industrial Commission if a dispute arises regarding your claim. There are three levels of hearings which may be used to resolve claim issues. The levels are: District Hearing Officer (DHO) level Staff Hearing Officer (SHO) level Industrial Commission level Time Off for Hearings If a hearing is scheduled during your work shift and you choose to attend, you must notify your supervisor and Human Resources of the hearing date and time three to seven days in advance. You will not be compensated for the time off, but there will be no penalty to your perfect attendance award or attendance bonus (no pay, no penalty).You may choose to cover your time off with PTO. The hearing process begins at the District Hearing Officer level. Hearings are informal and are held in the Industrial Commission district office closest to your home. After the hearing, the hearing officer will issue a decision. You and Honda will receive the written decision in the mail. If you or Honda are dissatisfied with the decision at this level, an appeal may be made to the Staff Hearing Officer level. To file an appeal, you must fill out a Notice of Appeal Form and return it to the Industrial Commission. Please note the specified time limit on the Industrial Commission order in which to file an appeal. The Industrial Commission level is the last administrative step in the hearing process and is more formal than the other hearing levels. If you or Honda appeals from the Staff Hearing Officer level to the Industrial Commission, the Industrial Commission may hear or refuse to hear the case at their discretion. In certain cases, further appeal may be made after a decision is issued at the Industrial Commission level. Compensation during the Hearing Process Payment of Disability Compensation If Honda receives an order to pay compensation at the District Hearing Officer level, payment of temporary total disability compensation or wage loss compensation will begin whether or not an appeal is made to the next level. If Honda pays compensation benefits in a claim that is later reversed in the hearing process, Honda may collect the overpayment from future Workers’ Compensation awards you receive from Honda for any claim. Payment of Medical Benefits If there is a dispute involving medical benefits, Honda is not required to pay Workers’ Compensation medical benefits until an order is issued at the Staff Hearing Office (SHO) or Industrial Commission level. If your claim for Workers’ Compensation is denied or not upheld in the appeals process, you should review Honda’s medical plan guidelines for information about how to submit claims for your medical treatment. Other Services and Programs | HAM/EGA/HNA 212 January 1, 2014 Payment of Compensation and Medical Bills during the Hearing Process Type of Payment When Payments Are Made: Hearing Levels DHO SHO Industrial Commission √ √ √ Medical Bills √ √ Permanent Partial √ N/A Temporary Total or Wage Loss Compensation Other Types of Workers’ Compensation Benefits You may be eligible for other types of Workers’ Compensation benefits depending on your condition and whether your claim has been certified. Eligibility for these benefits is determined on a case-by-case basis; benefits may include: Percentage of Permanent Partial Impairment. An award to an injured worker who has a residual disease or illness disability due to an allowed Workers’ Compensation injury/disease. The awarded amount, if any, is determined by the Industrial Commission or BWC Scheduled Loss Awards. An award for the loss of, or loss of use of, specific parts of the body due to an allowed Workers’ Compensation injury/disease Wage Loss. Compensation for a loss of wages as a result of an allowed Workers’ Compensation injury/disease. (Wage loss and temporary total compensation are not payable over the same period of time) Permanent Total Disability Compensation. Compensation paid to an injured worker whose condition has become totally disabling and, as a result, cannot engage in any type of sustained remunerative employment. The Industrial Commission will determine if an injured worker is eligible for this type of compensation Death Benefits. Benefits may be paid to your beneficiaries in the event of your death due to a workrelated injury, illness or disease, including related medical and funeral expenses and weekly compensation. All death claims are required to go to a hearing before the Industrial Commission Paid Time Off Honda provides you with paid time away from work to relax and celebrate special days. You are eligible for Paid Time Off (PTO) days if you are a regular associate who works at least 16 hours a week. You should reference your Associate Handbook for the PTO Schedule. Part-time associates can contact the Payroll Department or Talent Management Department for a copy of the part-time PTO schedule. Holidays On average, Honda provides 17 paid holidays, including the following: New Year’s Day Good Friday Memorial Day Other Services and Programs | HAM/EGA/HNA 213 January 1, 2014 Independence Day Labor Day Thanksgiving Day Day after Thanksgiving Christmas Shutdown days generally occur during scheduled plant shutdowns in July and December. Eligibility for pay during these holidays and shutdown days is explained in your Associate Handbook. Honda Educational Reimbursement Educational reimbursement is intended to encourage and support Honda’s associates in pursuing continuing, job-related education to improve current job performance, increase job-related knowledge and broaden potential within Honda by attending courses of study at an accredited education institution. Honda will also cover costs for graduate exams (such as GMAT and GRE) and PE (not FE) exams, plus the associate study materials under the $750 non-degree courses assistance level. This policy only applies to those courses taken on an associate’s own time, outside the normal work schedule. Courses must be completed with a grade of “C” or better to be eligible for reimbursement. You are eligible to request reimbursement as of the first day of the month following your first six months with Honda. Other Services and Programs | HAM/EGA/HNA 214 January 1, 2014 Safety Equipment Honda will ensure that you have the proper safety equipment required for your personal safety. Purchasing certain personal protective equipment, such as safety shoes and safety glasses, will be completed through payroll deduction or by reimbursing you for a qualified purchase. This section includes a brief description of the programs; please refer to your handbook for more details. Safety Shoes If you are an active associate who has completed at least three months of Honda employment, Honda will reimburse the cost of one pair of ASTM-approved safety shoes up to a maximum of $80 per calendar year. To receive reimbursement, you need to provide proof of your qualifying purchase to your Plant or Division Human Resources. New hires must wear safety shoes in required areas and may need to purchase safety shoes before reporting to work. New hires may submit their receipt immediately after purchase, but are not eligible for reimbursement until after they have completed at least three months of service. If you transfer to a department requiring special safety shoes or foot protection, that department will reimburse you for the cost of the protective equipment according to department policy. For additional details, refer to your Associate Handbook. Safety Glasses If your job requires safety glasses, Honda will provide you with ANSI-approved safety glasses. If you wear prescription glasses, you can receive one pair of ANSI-approved prescription safety glasses per calendar year. All prescription safety glass orders must be placed through your Plant Human Resources Department. Special frames, tinted glass, eye exam charges and other optional features are not eligible for reimbursement under this program. For information about certain limitations and specifications, refer to your Associate Handbook, or check with your Plant or Division Human Resources for details about departments with additional requirements and reimbursements. Honda Product Purchase You are eligible to purchase products under the Honda Purchase Program as of the first day of the month following your date of hire. Within one calendar year, you may purchase, for your personal use only, up to: Four Honda or Acura automobiles (2 new and 2 used) Two motorcycle products (which includes ATV products, motor scooters and watercraft products) and Two power products (which include engines, boat motors and mowers, etc.) within the calendar year Products may be bought and titled to you or your eligible family members (spouse, same gender domestic partner, children, stepchildren, parents, step-parents and siblings). In order to avoid competition with Honda’s dealer network, you or your eligible family member may not sell or transfer ownership title to another person for at least six months for a new product and three months for a used product. Pay and Rewards Honda enhances your benefit program with: Direct payroll deposits for your convenience Rewards for your contributions to Honda’s success through Bonus Sharing and the Voluntary Involvement Program (VIP) activities (award and recognition guidelines are available from your Plant or Division Human Resources department) Direct Deposit Honda has made arrangements with local financial institutions for direct deposit of your pay to your checking and savings accounts. Direct deposit is also available through the Honda Federal Credit Union (see “Honda Federal Credit Union (FCU)” on page 217). Bonus Sharing The Bonus Sharing Program rewards you for your contributions to Honda’s worldwide corporate profit. Bonus payments are made in November to eligible associates. Participation and Eligibility You are eligible to receive a Bonus Sharing payment if: You had base earnings within the Bonus Sharing period (from the first pay day in April of the previous year to the last pay day in March of the year in which the bonus is paid) and You are employed by Honda or EGA on the last day of the Bonus Sharing period (March 31) You are not eligible to receive a Bonus Sharing payment if you are: An employee or agent of an organization that provides temporary associates for employment at Honda A student associate or contractor as determined by Honda personnel policies or An associate of an international affiliate on assignment at Honda Taxes on Your Bonus Sharing In accordance with current federal, state and local laws, your bonus is considered to be compensation. Therefore, appropriate taxes (federal, state, Social Security, Medicare, city and school) will be withheld at a flat rate as determined in accordance with applicable laws. Other Services and Programs | HAM/EGA/HNA 216 January 1, 2014 Voluntary Involvement Program (VIP) Through VIP, Honda encourages associates to be involved in daily operations with an emphasis on continuous improvement and associate development that contributes to our continued success. VIP is comprised of two programs: NH Circle and Suggestion. Through these programs, associates can learn valuable problem solving and project management skills while earning VIP points throughout their career. As VIP points accumulate, milestone awards can be earned. In addition, the VIP points have a dollar value for which the associate will be compensated upon promotion to an exempt level, retirement or death (minus any milestone awards achieved). NH Circle Program NH Circle activity is a small group of associates (three to five) working voluntarily together to solve problems or implement improvements related to their work areas or other areas within Honda. Utilizing the step by step NH Circle process, associates will learn valuable problem solving and project management skills. Matching Gift Program The Matching Gift Program is designed to supplement associate-directed funding to cultural and educational organizations. Honda augments the associate’s contribution to eligible organizations with a 50% match, with a minimum associate donation of $25 and a maximum associate donation of $500 per year. Organizations that may be eligible, which must be non-profit and tax-exempt, include: Graduate and professional schools, four-year colleges, two-year junior and community colleges and technical/vocational institutes and Arts and cultural organizations that benefit the community To be eligible for the Matching Gift Program, the donor must be a regular, full-time or part-time associate of Honda and must have worked at Honda for a minimum of three months. To apply for the Matching Gift Program, you must submit a Matching Gift application completed by both you and the recipient institution no later than 90 days after the date of the donation. Honda Hero Volunteer Program The Honda Hero Volunteer Program provides an opportunity for Honda to encourage community involvement and demonstrate support for associates in their volunteer efforts. Through the program, fulltime and part-time associates, retirees and their spouses/Same Gender Domestic Partners (SGDP) can earn “Dollars for Doers” grants for their eligible non-profit organizations by volunteering a minimum of 50 hours to one organization within a 12-month period. Each family may earn one grant per 12-month period. (Grants cannot be used to fulfill personal pledge obligations.) To apply for a $250 “Dollars for Doers” grant after a minimum of 50 hours of eligible volunteer service, both the volunteer and the non-profit organization should complete the applicable sections of the “Dollars for Doers” Grant Request Form and return it to Honda. ADRs You receive ADR certificates for the One-Year Service Award and through the Stock Purchase Plan. ADRs, or American Depository Receipts, are receipts for shares of a foreign company, which are deposited in the foreign branch of an American bank. Honda Motor Company Ltd. ADRs are traded on the New York Stock Exchange under the ticker symbol HMC. Computershare will be the recordkeeper for your ADRs. This order is processed at the next available sale price as soon as market conditions allow. You have the ability to place two types of Limit Orders: Day Limit Order. This order allows you to specify a price at which you would like your sale to be processed Good Until Canceled Order (GTC). This order allows you to indicate a price at which you would like your sale transaction to be placed.
