The Aveda Institute Des Moines is dedicated to providing students with a quality learning
environment, which will prepare students to obtain their license after graduation and to obtain a
career post-graduation in Cosmetology, Esthiology, Massage Therapy, or other business and
management related fields such as retailing and management.

PROGRAM OPTIONS6
Cosmetology: Explore the latest styles and techniques in hair cutting, coloring, styling, skin care, nail
care and makeup, as you express your creativity. Upon graduation, the student will receive a diploma in
Cosmetology and will have the skills needed to find employment in a salon, spa or freelance business,
management, or retailing position. 12
Esthiology: Learn specialized techniques to purify balance and renew the skin, while using pure flower
and plant essences. Explore facial and body treatments, hair removal and makeup. Upon graduation,
the student will receive a diploma in Esthiology and will have the skills needed to find employment in a
spa, freelance business, management, retail or medical setting. 12
Massage Therapy: Explore balance and well-being. Learn the fundamentals of Swedish massage,
anatomy, physiology, reflexology and state safety requirements. Upon graduation, the student will
receive a diploma in massage therapy and will have the skills needed to find employment in a spa,
health club, or chiropractic clinic. 12

ACADEMIC COMPLETION INFORMATION8
Policy: It is the policy of the Aveda Institute Des Moines to prepare students for a career in
Cosmetology, Esthiology, Massage Therapy, and other business and management related fields,
through training in theoretical knowledge, practical experience and professional business-building
skills vital for success.
Procedure: In order to ensure the best possible education for the students, the following course
phases and curriculum will be followed.

The emphasis of these phases is to introduce the fundamentals of haircutting, hair styling, hair coloring,
chemical hair restructuring, skin and nail care, as well as the related sciences. Students learn state
safety requirements, and receive basic theoretical knowledge and practical application through lectures,
demonstrations and workshops. Students also learn product knowledge and positioning, makeup
artistry, guest servicing and personal development skills to further their professional development.
Upon the completion of these phases, students have the primary skills and experience to meet the
Aveda Institute’s hair and skin care retail servicing concepts.

The purpose of the Beta phases is to introduce students to advanced spa and nail services to
compliment their clinic floor experience. In addition, the Beta phases will strengthen students’ skills
necessary for gaining and maintaining employment. During the Beta phases, students are introduced
to contemporary techniques in haircutting, hair styling, hair coloring, permanent waving, chemical
restructuring hair, and nail care. This unit expands upon the student’s practical skills that will now
become standard behavior.
Through the review of theoretical and practical experience in hair and nail care, students improve in
confidence, dexterity and technical performance.
Gamma and Salon Lifestyle phases – 90 Days (approx. 3 day) 120 Days (approx. 5 day)

In the Gamma phase there is in-depth training in guest servicing, time management, self-promotion,
retailing, communication, goal-setting, merchandising, management and successful salon
entrepreneurship. In addition, students receive instruction in job requirements, interviewing techniques,
resume writing and employee benefits and wages.

Students are directed to fine-tune their coordination, speed, accuracy and concentration to meet entrylevel salon requirements. Students demonstrate competency in all tasks required for the skill
certification examination, as well as the theoretical knowledge necessary to pass the written
examination required by the Aveda Institute and the State of Iowa for licensure.

The main focus of these phases is the refinement of students’ practical skills, as well as introduction into
industry practices and equipment. Strong emphasis is placed on makeup, aromaology, the art of
retailing, business management and guest servicing.
Upon the completion of this phase, students have the skills and experience to meet the Aveda Institute’s
skin care, makeup, service standards and state safety requirements. The students improve dexterity,
confidence, timing, and technical performance.

The Massage Therapy curriculum is divided into two phases. In the Intro phase, Massage students learn
History of Massage, Clinical Applications, Anatomy, Joint Movement, Body Areas, Reflexology,
Hydrotherapy, Spa Treatments, Chakra Balancing Massage, and Pregnancy Massage.
In the Alpha phase, Massage students are taught Deep Tissue, Trigger Points, Acupressure, Lymphatic
Massage and are on the clinic floor.

GRADING10
Students are allowed to continue onto their next phase with a grade point average of 80% or above. A
student’s GPA is based on a written and practical exam, both weighted at 50%. A student must also
complete all quotas, models and quizzes within the allotted hour checkpoint, in order to graduate.
The Institute provides the following as grades given to students by %.

ADMISSION POLICY AND PROCEDURE
Policy:
Admission into Massage Therapy, Esthiology, and Cosmetology is open to any applicant who possesses a
high school education or equivalent (e.g. GED), is at least 16 years of age, and if it is determined by the
Admissions committee that the student is capable of successfully completing and benefiting from the
training provided. All proof of education may be verified. Aveda Institute Des Moines will not accept
online diplomas. No qualified person may be excluded from enrollment in Aveda Institute based on
age, race, color, sex, creed, religion, disability, marital status, or national or ethnic origin. A written
notice of acceptance or rejection must be sent to all applicants. The Institute does not take ability to
benefit.
Students currently in default on previous student loans will not be considered a candidate for admission
to the Institute until the default situation is resolved by the student.
Procedure:
1. Interview and tour with an Admissions Coordinator and receive the following information
a. Catalog
b. The pre-enrollment information packet including financing available, steps in applying
for financing, school’s current rates, physical demands, safety requirements, school
licensing requirements, compensation, campus security and drug policies of the school
c. Course Outline
2. Meet with financing person OR set up an appointment to discuss financing options
To enroll all applicants must:
1. Pay a non-refundable $100.00 application fee.
2. Have a signed permission from a parent or guardian if less than 18 years old.
3. Take aptitude test to determine capability of successful completion of program.
4. Approval by the Admissions department based on documents received and ability to complete
and benefit from the program.
5. Sign the enrollment agreement and pay a $100.00 registration fee (refundable only if the
student cancels enrollment within 3 business days after signing the enrollment agreement.)
6. Submit all documentation for enrollment to the Admissions department.
7. Submit all documentation to apply for financing
If all requested documents have been successfully submitted and reviewed and all admission
requirements have been met, a written notice of acceptance or rejection will be sent to the applicant.

TRANSFER STUDENTS15
Policy:
At the Aveda Institute, we treat all transfers as another prospective student. Applicants who wish to
transfer hours are considered on an individual basis. The Aveda Institute will accept transfer hours
based on the student’s ability to test out in various areas and on the student’s current hours of
completion. All admissions requirements and tuition payments must be arranged prior to the
acceptance of any transfer student.
Transfer of program work to another school is based on the policies of the transferee school.
Procedure:
All transfer students receive the same information that a new potential student would receive prior to
enrollment.
Transfer students must furnish all admission documentation required of any student enrolling at
Aveda, plus transcripts of hours, credits and grades from the school they are transferring from.
Transfer student must have a personal interview with Admissions. At the interview, the student will
discuss why they wish to transfer. Admissions will meet with the Director to determine if the student
will be allowed to transfer to the Aveda Institute.
The transfer student may need to take a placement test to determine at which phase they will enter
the Aveda Institute. The student will be placed in the appropriate level of training in accordance with
the number of hours and progress of their training at this time.
The transfer student will follow the same procedures for enrolling as any other student entering into
the Aveda Institute.

NON-DISCRIMINATION POLICY16
Policy:
No qualified person may be excluded from participation in, be denied the benefits of, or be subjected to
discrimination under any education program or activity at the Aveda Institute Des Moines based on age,
race, sex, color, creed, religion, disability, marital status, or national or ethnic origin. Title IX of the
Education Amendments of 1972 (“Title IX”), 20 U.S.C. § 1681 et. seq., and its implementing regulations,
34 C.F.R. Part 106, prohibit discrimination on the basis of sex in education programs or activities
operated by recipients of Federal financial assistance. Sex discrimination covers sexual harassment,
including sexual violence. See Sex Discrimination & Sex Offense Grievance, page 60-62.
Procedure:
If a potential student feels they have been discriminated against, they may request an appointment to
meet with an Admissions representative to discuss reasoning behind not being able to attend the Aveda
Institute Des Moines. Inquiries concerning the application of Title IX may be referred to the Title IX
Coordinator or to the Department of Education’s office for Civil Rights.

CAREER PLACEMENT14, 21
Policy:
The Aveda Institute maintains close lines of communication with many salons, spas and Aveda
Experience Centers nationwide. Although we do not guarantee job placement, we do provide career
placement assistance to help students seek out employment opportunities, not only in their licensed
field but also in areas of business management and retailing.
Procedure:
Student should express interest in employment opportunities with the Career Placement Coordinator
Career Placement Coordinator can assist student in updating cover letters, resumes, and other forms
needed for interviewing.
Career Placement Coordinator will work with students and salon owners and other businesses to set
up interviews for students.
Career Placement Coordinator signs all students up for Aveda Pure Pro. Students are asked to
become friends of Aveda Institute Des Moines facebook to learn about new job opportunities.

SCHOLARSHIPS20
Policy
The AVEDA Institute Des Moines does accept scholarship programs on an individual student basis.
Scholarships and/or awards may be provided by private, civic, high school, church and other
foundations, both inside and outside of the beauty & wellness industry. The Institute accepts
scholarships based upon academic merit or talent, and not solely on financial need. Whether a
scholarship program is accepted is at the discretion of the Institute Director.
Furthermore, the AVEDA Institute Des Moines may offers scholarships to incoming and current students
who have been accepted into a specific program of study. Funds are awarded in recognition of a
student’s accomplishments, and potential for future growth and development, based upon criteria set
by the Institute. When a scholarship is provided by the Aveda Institute Des Moines, any eligible
candidate meeting the set criteria may apply for the scholarship.
Procedure
In order to be considered for scholarships and awards, you must first be accepted into an AVEDA
Institute program and meet all admissions requirements. Talent scholarships provided by the Institute
may require an audition, portfolio and/or additional criteria as specified. All scholarships awarded by the
Institute and accepted by the Institute will be credited directly to the student’s account. If the student
account is credited with more funds than what is owed to the Institute, the student will receive a check
for the amount of the credit balance. It is the responsibility of the student receiving the scholarship to
notify the Business Manager of all the details pertaining to the scholarship. At that time, the appropriate
adjustments will be made in the student’s award and account.
*If a student’s enrollment status changes after they receive a scholarship, it is per the discretion of the
Aveda Institute Des Moines if the funds are applied towards tuition or if they are returned to the
scholarship provider.
The student is responsible for the following:
• Meeting educational costs.

• For supplying accurate information on all forms submitted. Funds obtained on the basis of false
information must be repaid and could result in criminal prosecution.
• For completing all necessary paperwork.
• For responding to the Office of Admissions on a timely basis. If you are applying for a scholarship with
a deadline, you must meet that deadline.
• For reporting any change in your circumstances which might affect eligibility for financial assistance.
These changes include residency, enrollment status, academic and attendance standing, and increase or
decrease of financial resources.
Students applying for, and using scholarship funds, can expect confidentiality. All financial information
submitted to the Office of Admissions will remain confidential and will not be released without your
written consent (unless you are a dependent student, at which time a parent/guardian may be granted
access to student records).

ADDITIONAL HOURS CHARGES23
Policy: It is the policy of the Aveda Institute Des Moines to allow students below 100% attendance, to
avoid overtime charges, to make-up any lost hours once they have completed 750 hours (Cosmetology
only) of the program. If a student does not complete the required number of Clock Hours for a program
prior to their contracted graduation date, then the student must pay a fee (as specified in the
enrollment contract) for each Clock Hour of instruction after his/her anticipated graduation date.
Procedure: At 700 hours, (Cosmetology only), a student will test-out with the education department, to
ensure they are at a skill level to make-up clinic floor hours once they reach 750 hours. Due to amount
of educators on the clinic floor, the amount of students allowed to makeup hours each day, may be
limited. If required Clock Hours are not met by the anticipated graduation date, a student will not be
allowed to clock any hours until they have paid the fee in full. A student will not be allowed to complete
the course without full payment of fees. Arrangements may be made with the Financing Department to
accommodate payments.

Student’s Right to Cancel; Notice of Cancellation and Refund. The student’s enrollment can be
terminated by written notice from one party to the other. If a student enrollment is terminated or
canceled for any reason, all refunds will be made according to the following refund schedule.
Refund Policy (In Accordance with Iowa Code 714.23)
If a student’s enrollment is terminated or canceled for any reason, all refunds will be made
according to the following refund schedule:
a. Cancellation must be made in writing. In order for a student (or in the case of a student
under legal age, his/her parent or guardian) to cancel his/her enrollment and withdraw from
school, the student must notify the Institute in writing.
b. All monies will be refunded within 45 days, if the applicant is not accepted by the Institute
or if the student cancels within three (3) business days (as determined by the postmark on
written notification) after signing the enrollment agreement and making an initial deposit
except for the $100 application fee. The postmark will determine the cancellation date on
written notification, or the date written notification is delivered to the school
administrator/owner in person. This policy applies regardless of whether or not the student
has actually started training. *
c. Cancellation after the third (3rd) business day, but before beginning the first class, will result
in a refund of all monies paid, with the exception of the $100.00 registration fee.
d. Student’s kit is non-refundable after the student has possession of their kit.
e. For students who enroll in and begin classes, and later withdraw or are terminated, the
Institute shall make a refund through the following calculation:

The Institute shall charge at least 10% of the tuition of the terminating student. After 60% of
the scheduled hours, no refund is due to the student and 100% of the tuition is owed by the

student. If a student terminates a program due to the student’s physical incapacity, or due
to the transfer of the student’s spouse’s employment to another city, the terminating
student shall receive a refund of tuition charges in the amount that equals the amount of
tuition charged to the student multplied by the ratio of the remaining clock hours in the
school period to the total number of clock hours in the school period.
f. Enrollment time is defined as the time elapsed between the actual starting date and the
date of the student’s last day of attendance in school.
g. The termination date, for refunds computation purposes, will be determined by the
postmark on written notification, or the date said information is delivered to the school
administrator/owner in person. The refund must be calculated within 30 days of the
withdrawal date, and all refunds will be made within 45 days of termination or receipt of
written cancellation.
h. For purposes of calculating the date of withdrawal, attendance is monitored on monthly
basis to determine unofficial withdrawal. If a Student is absent 30 consecutive calendar
days and was not granted an approved leave of absence, the student will be considered to
have withdrawn. The determination date will be the date that attendance was reviewed.
i. A student on an approved leave of absence notifies the school that he or she will not be
returning. The date of withdrawal shall be the earlier of the date of expiration of the leave
of absence or the date the student notifies the Institute that the student will not be
returning.
j. If the Institute is permanently closed and no longer offers instruction after a student has
enrolled, the student is entitled to a pro-rata refund of tuition based on the tuition
adjustment schedule.
k. If a program is cancelled subsequent to a student’s enrollment, but prior to beginning
instruction, the school shall, at its option: 1) provide a full refund of all monies paid;
or 2) provide for completion of the program.

EXCEPTIONS TO REFUND POLICY
Policy: Per Iowa Law: If a terminating student has completed sixty percent or more of a school term or
course that is more than four months in length, the person offering the course of instruction is not
required to refund tuition for the student. However, if, at any time, a student terminates a school term
or course that is more than four months in length due to the student's physical incapacity or due to the
transfer of the student's spouse's employment to another city, the terminating student shall receive a
refund of tuition in an amount which equals the amount of tuition multiplied by the ratio of
the remaining number of scheduled clock hours to the total clocked hours of the school term or course.
**If a student terminates within 30 calendar days (Cosmetology) or 15 calendar days (Massage Therapy,
Esthiology), the Institute will charge tuition by multiplying the scheduled hours as of LDA by the current
hourly rate of overtime charges as specified in the student’s contract.
Procedure: Once a terminating student provides proof that they are dropping due to physical incapacity
or the transferring of the student’s spouse’s employment to another city (not within driving capability of
remaining a student), the terminating student will receive a refund of tuition, equal to the amount of
tuition, multiplied by the ratio of the remaining number of scheduled clock hours to the total clocked
hours of the school term or course.
The termination date, for refunds computation purposes, will be determined by the postmark on
written notification, or the date said information is delivered to the school administrator/owner in
person. The refund must be calculated within 30 days of the withdrawal date, and all refunds will be
made within 45 days of termination or receipt of written cancellation.
**If a student terminates within 30 calendar days (Cosmetology) or 15 calendar days (Massage Therapy,
Esthiology), the Office of Financial Aid will use the above calculation to issue a refund to the student. At
this point in the student’s education, the above refund is more beneficial to the student than the listed
contract refund policy.

PERMANENT SCHOOL CLOSURE POLICY
Policy:
If the Aveda Institute closes permanently and ceases to offer instruction after students have enrolled, or
if a program is cancelled after students have enrolled and instruction has begun, the Institute will make
arrangements for students or implement any applicable teach-out plan.
Procedure:

The Institute will offer the students a reasonable opportunity to promptly resume and complete
the canceled program or Course(s) of study OR transfer to a substantially similar program or
course at an institution which offers similar educational programs.

The teach-out is to be performed by an institution in the same geographic area as the Aveda
Institute Des Moines

The school at which the students continue their education and training shall not charge the
students an amount greater than that to which the original school would have been entitled
under its contract with the student and for which the student has not yet paid.

The Aveda Institute Des Moines shall notify affected students individually of the availability of
the arrangement or teach-out plan, and diligently advertise such availability. The agreements
among institutions may provide that these notices may be sent by the school(s) that is accepting
students from the original school.

The Aveda Institute must submit to NAACAS a list of all students enrolled at the time of closure,
and will indicate on it the arrangements made for each student to complete his or her
education.

The Aveda Institute shall dispose of school records in accordance with state laws.

If unable to provide a teach-out plan, the Aveda Institute will refund students pro-rata tuition.

Salon, spa, and medical atmospheres are generally clean and pleasant environments with good lighting
and ventilation. Good health and stamina are important as most salon professionals work on their feet
for the majority of their shifts. Full-time in the beauty industry usually consists of 40 hour work weeks
which may include evening and weekends. Many salons offer part-time hours or have variable
schedules. Risks of working in Cosmetology, Massage or Esthiology may include vein and back problems
from standing, or carpal tunnel from consistent hand, and arm, movement.
Successful professionals must have hand and finger dexterity, a sense of form and artistry, and strong
listening skills with the ability to create what the customer desires. Strong relationship skills are integral
to job success and client building.

STUDENT DISCOUNTS AND INCENTIVES POLICY24f
Policy:
It is the policy of the Aveda Institute Des Moines that students must be using Aveda products, aromas,
and environmental practices on themselves and all guests. The Institute will provide a 30% discount
on all Aveda retail products, as well as additional incentives for students to be able to always use
Aveda products.
Retail Discounts: Students will receive a 30% discount on all Aveda retail products for personal use
and legitimate gifts. The student must be present, in person, to purchase products. Active students
may be asked to show current badge identification in order to receive a discount. Students may not
purchase more than 2 of any item at one time. Students are eligible for the discount ONLY at the
Aveda Institute Des Moines. Resale of Aveda product is prohibited. Students will not be eligible for
any discounts while on leave of absence.
Retail Incentive Program: Students are rewarded for their weekly retail sales with “Aveda Bucks”.
“Aveda Bucks” can be used to purchase retail products.
“Aveda Bucks” will be earned as follows: A weekly RPGT of $13.00 or more earns 10% back in “Aveda
Bucks”. A weekly RPGT of $20.00 or more earns 15% back in “Aveda Bucks”. When redeeming
“Aveda Bucks”, discounts do not apply. “Aveda Bucks” cannot be used to purchase gift certificates.
The Aveda Institute is not responsible for lost or stolen Aveda Bucks. Aveda bucks expire after
graduation. Please direct any questions regarding Aveda Bucks to the Experience Center Manager.
Student Back bar Order: Students may periodically be able to order Aveda products at Back bar
pricing. No more than 10 items or no more than one gallon per item can be placed per order.
Payment is due at the time the order is placed. It is per the discretion of the Inventory Specialist
when Back bar orders may be placed.
Student Service Pricing: All student service pricing must cover the cost of materials used (i.e. color
cost of a highlight). Students may book appointments on days they are not scheduled to be at the
Institute. However, these appointments are subject to be cancelled prior to 30 minutes before the
service time, if the time is needed to accommodate an external guest. Students must pay for services
prior to getting the service done if they are in attendance that day. If it is the student’s day off, the
student must pay for their service prior to leaving after the service.

Policy: It is the policy of the Aveda Institute Des Moines to ensure all students are given the opportunity
to be successful in the beauty, spa and wellness industry. We strive to make our programs accessible to
all individuals, in compliance with Section 504 of the Rehabilitation Act of 1973, Title III of the Americans
with Disabilities Act of 1990, and all applicable state laws.
Procedure:

Student must verify and file documentation with Student Services at the time of enrollment of
any disabilities that might affect their education with Aveda.

Student services will provide reasonable accommodations if possible.
If a student needs an accommodation for a disability, the disability must be disclosed at the time of
enrollment.
General Guidelines:
1. Eligibility and Documentation
To be eligible, a disability must be verified in writing by a doctor. Eligibility for accommodations
is dependent on the nature of the disability and its impact on learning. Therefore, as part of
their request for an accommodation, students generally must provide disability-related
documents from an appropriate licensed professional(s) to verify that a student has a disability
and to determine the need for reasonable accommodations. Documentation serves two
purposes. It establishes existence of a disability (a physical or mental impairment that
substantially limits a major life activity) that affords protection under the law, and it
demonstrates a need for accommodations to insure equal access to courses, programs, services,
activities, and facilities.
Documentation from a licensed professional must include the following information:
 A clear statement of the diagnosed disability
 A description of the functional limitations resulting from the disability
 A list of the accommodations recommended
 A statement of why the disability qualifies the applicant for accommodations
requested

The document should be current (ordinarily within the last three years), must appear on official
letterhead of the licensed professional, and must be signed by a qualified professional not
related to the student.
All costs associated with obtaining documentation are borne by the student. If the initial
documentation is incomplete or inadequate, Aveda has the discretion to require additional
documentation.
2. Reasonable Accommodations
A reasonable accommodation is an accommodation that would allow a student to be able to
perform responsibilities as assigned tasks and skills to all students.
3. When to Request an Accommodation
No accommodations will be awarded to students not disclosing a disability upon enrollment.
4. Accommodation Process
If Aveda determines that an accommodation is required and appropriate, educators will be
notified by Student Services. Educators will be expected to assist with the provision of
accommodations when reasonable and necessary. They are not expected to compromise
essential elements of the course or evaluation standards.
5. Grievances
Students with disabilities will be responsible for contacting the Student Services Coordinator if
reasonable accommodations are not implemented in an effective or timely way, or if they
believe they have been discriminated against on the basis of disability. Educators who wish for
reconsideration of an accommodation should also contact the Student Services Coordinator,
and should do so no later than one week after the accommodation is scheduled to be
implemented.
If an agreement cannot be reached informally, an individual may file a written complaint with
the Institute Director, who will conduct a thorough investigation that allows interested persons
an opportunity to submit evidence relevant to the complaint.
The Director will review the gathered evidence, meet with all parties in the dispute with their
permission, and decide upon an appropriate plan of action. A written determination as to the
validity of the complaint and a description of the resolution, if any, will be issued by the Director
to the complainant and other concerned parties no later than 10 workdays after the filing of the
grievance.

The school realizes that not all student’s progress at the same rate; however, all students are expected
to show a measurable rate of progress and development relative to their individual abilities. The school
has therefore, adopted the following policy statement to uniformly apply to all students. 2,4
The standards set forth by the school as minimum requirements for maintaining satisfactory progress
are (1) for all students to maintain a passing grade average of 80% or better in theory sciences and
practical work throughout the course. (2) For each student to proceed through the program at a pace
leading to completion of the course within the maximum time frame of 150% or 67% attendance 5,6
EXAMPLE:
The minimum number of hours to complete the Cosmetology 3 day program is 2100 clock hours, in 70
weeks the maximum number of hours scheduled to complete is 3150 and 105 weeks.
TO DETERMINE SATISFACTORY PROGRESS, All STUDENTS ATTENDING THE AVEDA INSTITUTE
COSMETOLOGY PROGRAM, WILL BE EVALUATED AT 450, 900, 1350, 1800 actual hours. 12 THE 600
HOUR ESTHIOLOGY ARE EVAUATED AT 300 AND 550 actual hours. THE MASSAGE THERAPY COURSE
WILL EVALUATE STUDENTS AT 350 AND 550 HOURS actual hours.10,11 At these intervals, students
receive written Satisfactory Progress Reports placed in their file from the Office of Student Services,
where when unsatisfactory, they are signed as received by the student. 25
Transfer hours from another school that are accepted towards a student’s program are counted as both
attempted and completed hours for the purpose of determining when the allowable maximum timeframe has been exhausted. SAP evaluations are based on actual contracted hours at the Institute. 24

If a student is making satisfactory progress at evaluation time, the said student is considered making
satisfactory progress until the next evaluation period.13
If a student is NOT making satisfactory progress at evaluation time, the said student will be placed on
warning status.14
PROBATION
ACADEMIC/ATTENDANCE:
Students who do not achieve satisfactory progress at an evaluation period in academic and attendance
will be placed on warning status until the next scheduled evaluation period. While on warning status,
the student will be eligible for Financial Aid. At the end of the warning period, if student has not
regained satisfactory progress, the student will be placed on probation and financial aid will be
terminated.16a
A student on probation is advised concerning his/her need to improve and is reminded of the school’s
policy on probations and suspensions. If advising is followed, the student will meet satisfactory
progress within the maximum timeframe established. 16d
PROBATION16
Students who fail to meet minimum requirements for attendance or academic progress after the
warning period will be placed on probation and considered to be making satisfactory academic progress
while during the probationary period, if the student appeals the decision, and prevails upon appeal.
Additionally, only students who have the ability to meet the Satisfactory Academic Progress Policy
standards by the end of the evaluation period may be placed on probation. Students placed on an
academic plan must be able to meet requirements set forth in the academic plan by the end of the next
evaluation period. Students who are progressing according to their specific academic plan will be
considered making Satisfactory Academic Progress. The student will be advised in writing of the actions
required to attain satisfactory academic progress by the next evaluation. If at the end of the
probationary period, the student has still not met both the attendance and academic requirements
required for satisfactory academic progress or by the academic plan, he/she will be determined as NOT
making satisfactory academic progress and, if applicable, students will not be deemed eligible to receive
Title IV funds.
STUDENT’S RECEIVING TITLE IV FUNDS
A student who does not achieve the minimum standards is no longer eligible for Title IV, HEA program
funds, if applicable, unless the student is on warning or has prevailed upon appeal of the
determination that has resulted in the status of probation. 18
RE-ESTABLISHMENT OF SATISFACTORY ACADEMIC PROGRESS
Students may re-establish satisfactory academic progress and Title IV aid, as applicable, by meeting the
minimum attendance and academic requirements by the end of the probationary period. 15

APPEALS17
Students with special circumstances (such as death of a relative, illness or injury of the student, or other
special circumstances) 17b may appeal the satisfactory progress determination by filing a written appeal
to the director within three (3) business days of the determination. The appeal should include the
reason(s) why the student believes the decision should be reversed and provide any supporting
documentation that will show what has changed in order for the student to make Satisfactory Progress
at the next evaluation.14, 17c An appeal hearing will take place within five (5) days of the receipt of the
written appeal. The Institution Director will make a decision within three (3) business days of the
hearing. The decision will be final and will be communicated to the student in writing. A copy of the
final decision will also be documented in the student’s file.15, 17d If a student prevails on appeal, the
student will be placed on the status of probation and will be re-entered into the program. At this time,
the student will work with the Office of Financial Aid to re-establish eligibility for Financial Aid. 17a
STUDENTS WHO RE-ENTER SCHOOL AFTER A PERIOD OF WITHDRAWAL WILL RE-ENTER IN THE SAME
PROGRESS STATUS AS WHEN THEY LEFT, REGARDLESS OF THE AMOUNT OF TIME THAT HAS
ELAPSED.20, 21
If a student is granted a leave of absence the student’s contract period and maximum time frame will be
extended the same number of days as in the leave of absent.7, 20

Policy: Aveda Institute complies with the Family Educational rights and Privacy Act of the 1974 Buckley
Amendment. Public Law 93-380, section 438. ALL STUDENT RECORDS ARE CONFIDENTIAL, UNLESS
OTHER WISE REQUIRED BY LAW or FOR ANY ACCREDITATION PROCESS. AVEDA INSTITUTE REQUIRES
WRITTEN AUTHORIZATION FROM A STUDENTOR PARENT/GUARDIAN (IN CASE OF A MINOR) TO RELEASE
INFORMATION TO ANY THIRD PARTY. STUDENTS OR PARENT/GUARDIAN (IN CASE OF A MINOR) HAVE
THE RIGHT TO REVIEW AND INSPECT A STUDENT'S EDUCATIONAL, FINANCIAL AND ATTENDANCE
RECORDS. STUDENTS AND PARENTS/GUARDIAN OF MINOR STUDENT, MAY DENY AUTHORITY TO

PROCEDURE: Student’s records are retained at the Aveda Institute Des Moines. All financial files are
maintained in the Business Office.
A student may request a transcript in writing. After 30 days, a fee of $20.00 will be charged to a student
for each transcript requested.
Students or parents/guardian of minor student must sign a release statement each
and every time anyone other than NACCAS, or by law, requests information. This includes prospective
employees or any other relative of the student, including a spouse and parents of students over 18.19
Students or parents/guardian of minor student must make an appointment with the Director of the
Institution to review the student's individual records. Students must allow reasonable time to assemble
records (no more than 45 days). 18

The Aveda Institute Des Moines is committed to providing students with the best education, from
classroom to clinic, to the facilities and equipment provided for use in education. The spacious 22,000
square foot building provides ample space for students to learn, study and interact with guests.
Aveda Experience Center:
A retail center for Aveda hair, skin, flower and plant Pure-Fume, body care, makeup and lifestyle
products. The retail store and its Aveda trained personnel give you the opportunity to practice your
client service and retailing skills.
Guest Service Areas:
A diverse array of clients comes to the Aveda Institute for beauty and wellness services. As a student,
you will receive training in a variety of salon and spa settings under the direct supervision of your
licensed instructors. The Institute has 122 Cosmetology stations, 8 manicure and 10 pedicure stations, a
Vichy Shower, 8 fully equipped spa rooms, and a fully equipped wax room for students to perform
services on guests.
Student Classrooms:
Individual classrooms have been designed to provide the proper environment for different types of
learning and activities. All of the classrooms are equipped with ample seating and learning space, as
well as state-of-the-art audio and visual learning devises. Classrooms can be opened to provide
additional space for continuing education classes sponsored by the Institute.
Resource Library and Administrative Offices:
A resource library has books on styling, motivation, health, wellness and environmental consciousness
for your reference. The clinic floor is surrounded by administrative offices and the leads are available
throughout the day to answer student questions. Educator offices are located near the classrooms.
Laptop and copy machines are available for student use.

Common Areas:
The Aveda Institute offers water fountains in the Experience Center, restrooms located in both the
Cosmetology and Spa areas, and a student break room equipped with refrigerators, microwaves and
vending machines for student use. The common areas also include a dispensary with state-of-the-art
equipment, including washers and dryers.
Lockers/Workstations:
Students are responsible for all of their belongings. Students will be provided with a locked locker and
workstation. If the student leaves the Aveda Institute by transfer, withdrawal or extended leave of
absence, they need to take all of their belongings with them. Items left in the locker/workstation will be
disposed of after 5 school days in order to provide space for incoming students.
Parking/Entrance:
The Aveda Institute offers student parking directly north of the Institute (across the street). All spaces
located on the west side of the Institute should be reserved for our guests. Students must enter the
building through the doors marked “student entrance” at the front of the building.

STUDENT KITS23
Policy: Students are responsible for any item in their student kit. Students need to inventory and
label their kit. It is the student’s responsibility to ensure all kit items are in proper working order
within 48 hours of receiving their kit. The student kit is to be used on guests, and students may be
required to purchase additional products. The Aveda Institute is not responsible for missing or stolen
items.
Procedure: If products are missing or damaged in original student kit, given at orientation, the
Inventory Manager must be notified within 48 hours and will replace missing or damaged item.

PAYMENTS ON STUDENT KITS
Policy: The student’s kit fee is due at the time a student signs his/her enrollment agreement and must
be paid in full by one of the following options: cash, debit card, credit card, personal check, cashier’s
check, personal loan or Sallie Mae loan, unless other arrangements are made. In the case of Federal
Financial Aid, kit payments may be dependent on the student’s financial plan.
Procedure: The Office of Financial Aid shall help each student determine a suitable plan to cover the kit
fee.

Policy:
The Aveda Institute uses a hand scanner to record attendance to school. The Aveda Institute is a clock
hour institution, therefore clocking in and out is mandatory. Aveda Institute can only issue credit for
hours that are properly documented through the time clock. Students are allowed to miss up to 10%
of their scheduled hours without incurring over-contract charges. Any student in Cosmetology Intro
who misses more than (3 days) 30 classroom hours will be required to re-take the Intro program
before progressing on to the next phase. This means the student will have to terminate or take a
Leave of Absence until the next Intro phase they are eligible to attend. If a student drops below 90%
attendance in Cosmetology Alpha-Gamma II phases, it is per the discretion of the Educator and
Student Services Coordinator to require the student to re-take the phase. Students who do not
comply with the attendance policy will be charged for overtime hours, at the rate specified in their
contract. Students will be allowed one edit per phase to their time records. Cosmetology students
are not allowed to miss more than (6 days) 60 Friday or Saturday hours during their course duration.
Esthiology students are not allowed to miss more than (3 days) 30 Friday or Saturday hours during
their course duration. Massage Therapy students are not allowed to miss more than (3 days) 30
Friday or Saturday hours. Missing more than allowed amount of Fridays or Saturdays will lead to
disciplinary action which may include additional charges or termination.
Procedure:
Students must scan in/out upon arrival to and departure from the Institute.
Students should report any malfunction with the scanner immediately to Administrator
NOTE: All students are expected to be in assigned area for roll call (see tardy policy).
If a student falls below 90%, they will be counseled by Student Services at the end of the phase. If a
student completes a second phase below 90%, the student must make-up hours to avoid overtime
charges. If a student finishes a third phase with below 90% attendance, they will be subject to
disciplinary action up to and including termination. Students are allowed to apply for make-up hours
once they reach the hour requirement of 750 hours (Cosmetology only).

MAKE-UP POLICY
Make-up Hours:
The Aveda Institute wants students to be successful after graduation. A student will be allowed to
make-up hours missed during school time, to avoid overtime charges. If a student fails to show up for
a scheduled makeup day, further disciplinary action will be taken. Any student who continually
misses their regularly scheduled class or clinic days may temporarily lose the opportunity to make-up
hours, per the discretion of Student Services.
Procedures:
Students are allowed to make-up hours on non-scheduled days, once they have hit the hour
requirement of 750 hours (Cosmetology only). At 700 hours, (Cosmetology only), a student will testout with the education department, to ensure they are at a skill level to make-up clinic floor hours
once they reach 750 hours. Due to amount of educators on the clinic floor, the amount of students
allowed to makeup hours each day, may be limited.
An administrator must sign a form approving all make-up hours, so that the student may be put on
the client book. All approved make-up hours must be placed in student file.
A student failing to show up at school for makeup time, therefore missing appointments, will be
subject to disciplinary action as follows:
1st offense, the student will not be eligible for make-up hours for 7 days
2nd offense, the student will not be eligible for make-up hours for 15 days
3rd offense, the student will not be eligible for make-up hours for 30 days
NOTE: Cosmetology students are not allowed to miss more than 60 Friday or Saturday hours (3 day) or
7 Saturdays (5 day) during their course duration. Esthiology students are not allowed to miss more
than 30 Friday or Saturday hours during their course duration. Massage Therapy students are not
allowed to miss more than 30 Friday or Saturday hours during their course duration. Missing more
than allowed amount of Fridays or Saturdays will lead to disciplinary action which may include
additional charges or termination.

Make-up Work:
Policy:
The Aveda Institute Des Moines allows students to make-up missed work.
Procedures:
All written and practical exams, quizzes, quota sheets must be completed and passed by the end of each
phase. All model days must be made up within 1 week of scheduled model day to avoid receiving a
violation. All make up work will be completed during the student scheduled hours. If at end of the
phase, the student has not made up missed work or projects he/she will be allowed to move into the
next phase of the program. However, if the student has not made up all practical work, projects and
written exams by the end of that phase, the student will be subjected to termination or repeating the
curriculum. At this time, the student must complete all work OFF THE TIMECLOCK prior to advancing to

TARDY, LATE ARRIVALS AND EARLY RELEASES24a
Policy:
Tardy: Students who do not arrive to class promptly at 8:00 am (3 day) or 10:00 am or 9:00 am on
Saturdays (5 day) will be marked as tardy. A 15 minute grace period for extenuating circumstances will
be given to each student. However, time missed will be counted against the student’s total % of
attendance. After three tardies, the student will be sent home for the day and further disciplinary
action may follow.
Lunch and Breaks: All students are allowed a lunch at a specified time. Additional breaks may be
allowed if time permits. Students who return late from lunch will be marked as tardy.
Early Release: All students must have permission to leave class early. In the event a student is late
arriving or leaves early and is able to provide a doctor’s note, they will be allowed to clock back in for
the day. If this falls on a Friday (3 day) or Saturday (3 or 5 day), and a doctor’s note is provided, only the
actual hours missed (not a total Friday or Saturday) are counted towards the student’s pool of
Friday/Saturday hours. If a student leaves during make-up hours, this will result in a write up. Please see
Makeup Hours policy.
Procedure:
Tardy: Any student arriving between 1 and 15 minutes late (EXAMPLE: 8:01-8:15) or returning late from
lunch or break will be marked as tardy, and a warning must be issued to the student. Each student NOT
in class or clinic at 8:05 am (3 day) or 9:05 or 10:05 (5 day) will be considered tardy and the warning will
be documented. After 3 documented warnings, the student will be sent home. After 8:15 am (3 day) or
10:15 or 9:15 am (5 day) a student NOT in class or clinic will be sent home. All tardies are unexcused
hours and are subject to overtime charges. A student may be allowed to arrive to school for the day
after lunch if the tardy is pre-approved. All late start days will be approved per situation.
Early Release Procedure:
Students leaving early need to:




Obtain and complete an “Expected Absence Form” to be signed by the educator.
Proceed to retail area and deliver Expected Absence Form to a retail team member, at least
1 hour in advance of time leaving for the day.
Scan-out for the day.
Hours missed will count as absent hours towards the student’s total % of attendance.

MODEL POLICY23
Policy:
It is the policy of Aveda Institute Des Moines that students must provide models to practice services on
for each phase. If a student fails to provide a model at the appropriate time, the student will then need
to clock out for the duration of the model period, and may be sent home for the day.
Procedure:

Student must secure a model for each appointment time by confirming the time with
the guest.

The student must give the Experience Center front desk the name, phone number,
service being performed, and service time by business close of the day before the
appointment is booked for.

If a model is changed, the student must let the front desk know as soon as possible.

Any student without a model may be required to clock out and will receive an
incomplete for the model day.

All missed model days will need to be made up per make-up work policy

Missed model days will be made up on the student’s day off for hours, under the
supervision of the clinic floor educator within 1 week of the scheduled model day. The
day and time must be pre-approved by that educator.

Failure to make up a model day within 1 week will result in the student receiving a
violation.

Students are not allowed to work on more than one model/guest at a time.

TEMPORARY INSTITUTE CLOSING
Institute Closing: Institute closing will mirror DMACC (Des Moines Area Community College). Saturday
closing will be at the school’s discretion. Hours missed due to school closings will be considered an
excused absence but hours must be made up.
Procedures: Students can go to http://www.whotv.com/weather/closings/who-weather-relatedclosing-alert,0,2272435.framedurl and register to receive emails or text messages on school closings.
The Experience Center will be contacting all guests to reschedule appointments cancelled due to the
school closing.
Evacuations: If students evacuate prior to school closing, any time missed will be considered an
unexcused absence. Students unable to return due to safety issues (i.e. airport closures) must provide
proof of travel. The Institute Director will look at circumstances on a case-by-case basis.
The safety of each student is the school’s top priority. The Aveda Institute maintains the safety of their
students, staff and clientele while also committing to excellent customer service and a quality
educational environment.
Holidays: The Aveda Institute will be closed for the following holidays:

LEAVE OF ABSENCE24f
Policy: All students requesting a leave of absence must do so in writing. Leaves may be approved for
medical reasons for a minimum/maximum of 30-180 calendar days in a 12 month period. All other
leaves must be approved by an Administrator of the Institute. Students will be notified of approval or
denial for the written request of a leave of absence. A student who takes an unapproved leave of
absence, or fails to return to the school at the end of an approved leave of absence, is terminated. In
addition, the Institute can require a student below satisfactory performance to take a leave of absence.
Procedure: The student submits a written, signed and dated request within 72 hours of the intended
leave of absence. Emergency leaves of absence, without prior written request, may be granted provided
the student completes and submits the leave of absence request form, along with any necessary
medical documentation, to the Institute via mail or in person.

The leave must be a written request to the Director for a leave of absence.
2. A written request is then documented with date of beginning of leave, end date of leave, and
new calculation of graduation date.
3. A student must call or come in to see the Director to have a leave extended.
4. If student does not return on the specified date of return, he/she will automatically be
terminated and a refund will be calculated.
(Refer to refund policy).
5. Students who return from a leave will rejoin their program where they left off. The date of reentry will be determined by school Administrator.
6. Student will return making the same satisfactory progress as when the leave began.

RE-ENTRY24f
POLICY: Students who do not return from a leave of absence, are expelled or withdraw from the
Institute may apply for re-entry into a program with the Institute Director. Students re-entering will
return to the program making the same status as when they left the Institute.
PROCEDURE: A student must make an appointment to meet with the Institute Director. If approved for
re-entry, the student needs to initiate the re-entry process with Admissions as if they are a new student.
A student may need to re-apply for financing and complete the entire financing process as well as the
Admissions process. Cash paying students will be required to pay the entire cash balance owed prior to
returning. A $100 registration fee and $100 application fee must be paid before re-entering the
program.

PROFESSIONAL STANDARDS AND DRESS CODE23, 24d
Policy: It is the policy of the Aveda Institute Des Moines that all students must conduct themselves in a
professional manner and maintain a professional appearance at all times during school hours or when
representing the Institute. Failure to do so may result in disciplinary action.
Procedures:
 Educator will write up offense on a Violation form and student may be sent home for the day,
losing the hours they are sent home for.
 Write up form will be filed in student’s file.
The following guidelines are examples of what constitutes unprofessional behavior and dress code. This
is not an exclusive list and students will be written up per-basis.
Professional Standards and Dress Code






The uniform supplied in the student’s kit is to be clean, neat, not torn, unstained, unaltered and
worn at all times when in the building. If it does not meet these standards, the student has 24
hours to replace it from Aveda at the student’s expense.
The uniforms supplied are worn with full length solid black pants and a solid black shirt.
NOTE: Official Aveda shirts are allowed if they are black – colored shirts may not be worn.
Identification badges, supplied by the Aveda Institute, are to be worn as issued during all
clocked hours to identify students and staff to guests. If lost, the student has 24 hours to
replace it at a cost of $10.00
Apparel considered unprofessional includes jeans, sweatpants, athletic wear, hats, revealing or
torn clothing (determined by staff).
Black closed-toe footwear must be professional in appearance and worn at all times.
Aveda Institute reserves the right to maintain an aesthetic standard for all students, including
personal hygiene and grooming, makeup, and groomed facial hair
Students are expected to conduct themselves in a professional manner at all times and be aware of the
following behaviors:


Anyone who is disruptive in the classroom or clinic floor (rudeness, foul language or other
unprofessional behavior).
Food, candy and gum are allowed in the lunchroom area only.

Beverages can be consumed in the lunchroom and classroom areas only, with the exception of
closed, eco-friendly water bottles on the clinic floor.
Aveda Institute is a smoke-free facility, and smoking is not allowed in or around any part of the
school.
Personal electronic devices should be left in the student’s locker and may only be used during
break times in the lunch room. This includes cell phones.
To ensure that each student receives consistent and comprehensive instruction in the classroom or
clinical environment they need to:




Remain in their assigned areas or receive their educator’s permission to be in unassigned areas.
Fully participate in all classroom and clinic activities utilizing Aveda’s products and
treatment/service protocols.
Maintain a mentally alert and sober state of mind.
All services or work done by students will be assigned by an educator – students who refuse an
assigned service may be dismissed for the remainder of the day.

It is the policy of the Aveda Institute Des Moines to promote health and sustainability of the earth
through education, commitment, action and accountability.
Procedure:

Glass should be recycled in the bin located in the break room.

Aluminum cans should be recycled in the bins located in the break room or outside of the
Director’s office. Unclean foils may not be recycled as aluminum.

Paper should be recycled in bins located outside of classrooms, the break room, or outside of
the Director’s office.

Plastic should be recycled in the dispense area and the bin outside of the Director’s office.
Plastic silverware and bowls may be recycled.

EMERGENCY PROCEDURES24f
POLICY: It is the goal of the Aveda Institute to provide and maintain a safe and non-violent academic
and working environment. During an emergency, students should remain calm and take an active role
in maintaining a safe environment.
Medical Emergencies and Accidents: Preventative measures should be taken to avoid accidents and
injuries.
Procedures:
1. Notify Administrator immediately in case of a medical emergency.
2. Administrator must collect information regarding the nature of the problem and the location of
the emergency in the building, stay with the injured person, send someone to meet with
emergency personnel, and keep the area clean of bystanders.
3. Administrator must call 911 if necessary.
4. Administrator must document incident and forward paperwork to the Business Office.
5. Security personnel must be called to the scene to gather information and submit a written
report to school Administration.
Fire/ Fire Drills: The Aveda Institute has an alarm-monitored system that is directed to the West Des
Moines Fire Department. Whenever a fire is detected, a continuous siren will sound. Floor plans are
posted throughout the Institute that shows a direct evacuation route. From time to time, fire drills will
be conducted to prepare for an emergency. During drills, everyone must follow normal procedures for
evacuating the building.
Procedures:
1. If alarm sounds, instruct all students, guests and facility to evacuate building.
2. Assist guests who need help evacuating.
3. All students and staff meet at the far northwest parking lot outside of front doors of building.

4. Instructors take role call to assure all students have evacuated.
5. Re-enter the building only when the Fire Department has given clearance (or Administrative
personnel in the case of a drill).
Tornado/ Tornado Drills: Whenever there is danger of tornado touchdown, the tornado warning sirens
for the West Des Moines area will sound. When sirens sound, building occupants must retreat to the
safest area of the Institute. From time to time, tornado drills will be conducted to prepare for an
emergency. During drills, everyone must follow normal procedures for tornado emergencies.
Procedures:
1. If alarm sounds, instruct all students, guests and facility to take cover in spa area of Institute
where there are no windows.
2. Assist guests who need help relocating to the spa area.
3. All students and staff meet in area outside of Vichy room.
4. Instructors take role call to assure all students are in spa area.
5. Re-enter the rest of the building only when the sirens in West Des Moines have stopped
sounding and there is no longer a Tornado Warning in effect.
Bomb Threat: In case of a bomb threat at the Aveda Institute, an Administrator should be notified
immediately.
Procedures:
1. Administrator should call 911 for assistance.
2. Students, guests and staff should be evacuated to the far North West parking lot.
3. Instructors will take role call to assure all students have evacuated.
4. Re-enter the building only when Emergency Personnel have given clearance.
Act of Terrorism: In the case of a terrorist act, or an act of violence at the Aveda Institute Des Moines, it
is imperative that all students and faculty are able to immediately leave the building and consider their
safety first.
Procedures:
1. All students, guests and staff should evacuate calmly and immediately through the nearest exit.
No one should grab personal items or wait for another individual to evacuate.
2. Anyone aware of the situation and who is able to call 911, should do so immediately after
evacuation.

3. All students and staff should meet in the Kohl’s far parking lot located at 6515 Mills Civic
Parkway, West Des Moines, IA 50265.
4. Instructors take role call to assure all students are in the safety zone (Kohl’s parking lot).
5. An emergency number will be given to all students to call in the event they are unable to find
the safety zone.
6. After police release all safe staff members and students, the student will be able to return to the
school or home as applies.
Student Responsibility: Students are reminded that they are not employees of the Aveda Institute Des
Moines, and therefore are not covered by Workers Compensation insurance. The Aveda Institute Des
Moines is not responsible for injuries or damages suffered by students during their education. Students
are encouraged to obtain medical and other insurance to protect from injuries or damages.

DRUG AND ALCOHOL PREVENTION PROGRAM24f
AVEDA INSTITUTE DRUG POLICY
It is the policy of the Aveda Institute to prohibit the unlawful manufacture, distribution, dispensing,
possession or use of a controlled substance in or on the Aveda Institute owned or controlled property.
The Aveda Institute is committed to providing a campus environment free of abuse of alcohol and the
illegal use of alcohol and drugs. To strengthen that commitment, the Aveda Institute has adopted and
implemented a program that seeks to prevent the abuse of alcohol and drugs by the Institute, which
includes its employees and students. Outlined in this statement are the Institute polices concerning
the use of alcohol and drugs. This statement is provided in response to the federal DRUG-FREE
SCHOOLS AND COMMUNITIES ACT AMENDMENT OF 1989 (Public Law 101-226).
STANDARDS OF CONDUCT
The unlawful manufacture, distribution, dispensation, possession or use of a controlled substance
(drugs), and the unlawful possession, use, or both, of alcohol, is prohibited in and on property owned or
controlled by the Aveda Institute.
No employee or student is to report to work, attend class, or participate in an Aveda Institute activity
while under the influence of one or more illegal drugs or alcohol.
The possession and use of alcoholic beverages by members of the Aveda Institute is at all times subject
to the alcoholic beverage laws of the State of Iowa, as well as city ordinances within our service area and
the Institute’s policy.
AVEDA INSTITUTE SANCTIONS
Violation of the policies and laws referenced above by an employee or student will be grounds for
disciplinary action up to, and including, termination or expulsion in accordance with applicable Aveda
Institute policies. Violators may be required to undergo evaluation, treatment, or both, for a drug or
alcohol-use disorder or referral for prosecution consistent with local, state and federal criminal laws.
Disciplinary action taken against a student or employee by the Institute does not preclude the possibility
of criminal charges being filed against that individual. The filing of criminal charges similarly does not
preclude action by the Institute.

OTHER SANCTIONS
State law prohibits the possession of alcoholic beverage by persons under age 21. No person may sell,
give, serve, or be served, alcoholic beverages to a person under age 21, and it is unlawful for a person
under age to misrepresent his or her age in order to obtain alcohol.
Under State law, it is a crime for any person to possess or distribute a controlled substance (drug).
HEALTH RISKS ASSOCIATED WITH THE USE AND ABUSE OF ALCOHOL AND ILLICIT DRUGS
Alcohol consumption causes a number of marked changes in behavior. Even low doses of alcohol
significantly impair the judgment and coordination required to drive a car safely, increasing the
likelihood that the driver will be involved in an accident. The use of even small amounts of alcohol by a
pregnant woman can damage the fetus. Low to moderate doses of alcohol also increases the likelihood
of a variety of aggressive acts.
Moderate to high doses of alcohol cause marked impairment in higher mental functions, severely
altering a person’s ability to learn and remember information. Heavy use may result in chronic
depression and suicide, and may also be associated with the abuse of other drugs. Very high doses can
cause respiratory depression and death. If combined with other depressants of the central nervous
system, much lower doses of alcohol will produce effects described above for very high doses. Longterm, heavy alcohol use can cause digestive disorders, cirrhosis of the liver, circulatory system disorders
and impairment of the central nervous system, all of which may lead to early death.
Repeated use of alcohol can lead to dependence, and at least 15-20 percent of heavy users eventually
will become problem drinker or alcoholics if they continue drinking. Sudden cessation of alcohol intake
is likely to produce withdrawal symptoms, including severe anxiety, tremors, hallucinations and
convulsions, which can be life-threatening. The use of illegal drugs, and the misuse of prescription and
other drugs, poses a serious threat to health. For example, the use of marijuana (cannabis) may cause
impairment of short-term memory, comprehension, and ability to perform tasks requiring
concentration.
The use of marijuana also may cause lung damage, paranoia and possible psychosis. The use of
narcotics, depressants, stimulants and hallucinogens may cause nervous system disorders and possible
death as the result of an overdose. Illicit inhalants can cause liver damage.
There are significant risks associated with the use of alcohol and drugs. These risks include impaired
academic or work performance; lost potential; financial problems; poor concentration; blackouts;
conflicts with friends and others; vandalism, theft and murder, sexual assault and other unplanned
sexual relationships; spouse and child abuse; sexually transmitted diseases; and unusual or
inappropriate risk-taking which may result in physical or emotional injury or death.
The school maintains drug and alcohol education information which can be obtained from the Student

AVAILABLE THROUGH drugfreeinfo.org 24/7 help line in Iowa, CALL 1-866-242-4111.

FEDERAL DRUG-FREE WORKPLACE ACT REQUIREMENTS
The following are required of the Aveda Institute and its employees and students:
1. An employee or student shall notify his or her supervisor, or other appropriate management
representative, of any criminal drug statute conviction for a violation occurring in the work place
no later than (5) days after such conviction.
2. The Institute shall notify any federal contacting agency within ten days of having received notice
that an employee or student engaged in the performance of such contract or grant has had a
criminal drug statute conviction for a violation occurring in the work place.
3. The Institute will take appropriate personnel action against any employee or student who is
convicted for a violation occurring in the work place and will require the employee‘s or student’s
satisfactory participation in a drug abuse assistance or rehabilitation program.

SEXUAL HARASSMENT AND SEXUAL VIOLENCE24f
Policy:
The Aveda Institute Des Moines is committed to ensuring an education environment free of sexual
harassment, sexual violence or harassment based on sexual orientation or gender identity.
Procedure:
1. Student must file a complaint with an Administrator.
2. The Administrator will document the complaint and review it in a timely manner (not to
exceed (7) days).
3. In cases of harassment, the Director will first attempt to resolve the challenge through a
mutual agreement of the complainant and the person complained against.
4. The Director will maintain periodic communications with the complainant until the
complaint is resolved.
5. The Director will resolve complaints quickly. Barring extenuating circumstance, the Director
will complete the investigation and make a recommendation within 30 days from the time
the formal investigation is initiated.
This policy pertains to all incidents of sexual harassment, criminal sexual violence, and assault that occur
on property owned by the Aveda Institute or sponsored activities held off Aveda property.
1. Definition of Sexual Harassment: Unwelcome sexual advances, requests for sexual favors, and
other verbal or physical conduct of a sexual nature may constitute sexual harassment when:
a. submission to such conduct is made either explicitly or implicitly, as a term or condition of an
evaluation of a student’s academic performance, term or condition of participation in student
activities, or in other events or activities sanctioned by the Institute;

2. Sexual Harassment is a violation of Section 703 of Title VII of the Civil Rights Act of 1964 as amended
in 1972, (42 U.S.C. S2000e, et, Sequa.) the Iowa Civil Rights Act of 1965, Iowa Code Chapter 216, and
Title IX of the Education Amendments of 1972(20 U.S.C.1691, et.seq.) and is punishable under both
federal and state laws.
3. Definition of Sexual Violence or Assault: acts of sexual violence, such as rape, sodomy, sexual assault
with an object, acquaintance rape, or other forms of nonconsensual sexual activity, or violence or
harassment based on sexual orientation or gender identity. These acts will not be tolerated at the
Institute as such acts are inappropriate and create an environment contrary to the goals and mission
of the Institute. Any such acts will be thoroughly investigated and will subject an individual to
appropriate disciplinary sanctions and/or possible action by appropriate law enforcement agencies.
It is the responsibility of all persons within the Aveda Institute to ensure an educational environment
free from sexually violent and/or harassing behavior. All members of the Institute (student and staff)
are expected to report incidents of sexual harassment, sexual violence or assault, and harassment based
on sexual orientation or gender identity.
The Institute Administrator is responsible for investigating complaints of sexual harassment, sexual
violence, harassment based on sexual orientation or gender identity, and alleged sexual harassment.
The Institute Administrator is contacted by using the following information:
Efforts shall be made to protect the privacy of the complainants within the constraints of the law.
Retaliation against persons complaining of sexual harassment, sexual violence, or violence based on
sexual orientation or gender identity is prohibited. Appropriate and immediate attention will be given to
complaints. Students may also contact the Iowa Department of Human Rights, the Federal Equal
Opportunity Commission, or the Criminal Justice System.
For all formal complaints of sexual harassment, sexual violence, or violence based on sexual orientation
or gender identity, the Director shall determine the action to be taken, implement the action, and notify
both parties of the action. A memorandum of such action will be sent to the Aveda Institute’s Human
Resources Department. Individuals found in violation of these policies will be subject to appropriate
disciplinary sanctions, including possible expulsion from the Aveda Institute.
If the perpetrator of sexual violence/assault is a student, he/she will be subject to disciplinary measures
by the Institute. In the course of any sexual violence/assault proceedings, the victim, the victim’s
support person of choice, or attorney may be present.
Aveda Institute Management, in cooperation with the appropriate law enforcement authorities, and at
the victim’s request, shall shield the victim from unwanted contact with the alleged assailant, including
transfer of the victim to alternate classes, (if alternative classes are available and feasible).
Consensual Romantic/Sexual Relations
Consensual romantic/sexual relations between staff and student are strictly prohibited. Any violation of
this policy will result in disciplinary action.

SEX DISCRIMINATION & SEX OFFENSE GRIEVANCE
Policy:
The Aveda Institute Des Moines prohibits crimes of dating violence, domestic violence, sexual assault,
stalking, rape, sodomy, sexual assault with an object, sexual harassment, including sexual violence by an
Institute employee, student, or third party.
Procedure:
1. Complainant must notify the Title IX Coordinator of the offense. The Title IX Coordinator
will stress the importance of preserving any evidence as may be necessary to the proof of
criminal sexual assault. Complaints must be made within six months of the issue.
2. The Title IX Coordinator will document the complaint and review it in a prompt timeframe
not to exceed (7) days. At the time of review, the Title IX Coordinator will review the Title IX
rights and available resources, such as counseling, health and mental health services and the
right to file a complaint with local law enforcement. At the request of the victim, the Aveda
Institute shall make any reasonable accommodations to shield the victim from unwanted
contact with the alleged, including transfer of the victim to alternate classes, (if alternative
classes are available and feasible), regardless of whether the victim chooses to report the
crime to local law enforcement.
3. The Title IX Coordinator will first attempt to resolve the challenge through a mutual
agreement of the complainant and the person complained against. Mediation will not be
used to resolve sexual assault complaints. Complainants have the right to end the informal
process at any time and begin the formal stage of the complaint process (see 4).
4. During the formal complaint process, the Title IX Coordinator will research the issue,
providing adequate, reliable and impartial investigation of the complaint. Both the
complainant and the person complained against will be allowed equal opportunity to
present relevant witness and other evidence including an advisor of their choice. Both
parties are given timely notice of meetings at which one or the other or both may be
present. The accuser, the accused, and appropriate officials are given timely access to
information that will be used after the fact-finding investigation but during informal and
formal disciplinary meetings and hearings. The Aveda Institute Des Moines uses a
preponderance of the evidence standard to evaluate complaints.

5. The Title IX Coordinator will resolve complaints quickly. The Title IX Coordinator will
complete the investigation and make a recommendation within 60 days from the time the
formal investigation is initiated. The accuser and accused will receive simultaneous
notification, in writing, of the result of the proceeding and any available appeal procedures.
6. Possible sanctions that the Institute may impose following the results of any institutional
disciplinary proceedings for an allegation of dating violence, domestic violence, sexual
assault, stalking, rape, sodomy, sexual assault with an object, sexual harassment, and sexual
violence include suspensions of 3-14 days, a forced leave of absence, and termination
without consideration for re-entry into any program.
A. Definition of Sexual Harassment: Unwelcome sexual advances, requests for sexual favors, and
other verbal or physical conduct of a sexual nature may constitute sexual harassment when:
1. submission to such conduct is made either explicitly or implicitly, as a term or condition of an
evaluation of a student’s academic performance, term or condition of participation in student
activities, or in other events or activities sanctioned by the Institute;
2. submission to, or rejection of such conduct by an individual, is used as the basis for academic
decisions or other decisions about participation in student activities or other events/activities
sanctioned by the Institute;
3. such conduct has the purpose or effect of threatening an individual’s academic performance; or
creating an intimidating, hostile or offensive educational environment.
B. Definition of Sexual Violence or Assault: acts of sexual violence, such as rape, sodomy, sexual assault
with an object, acquaintance rape, or other forms of nonconsensual sexual activity, or violence or

C. Definition of Domestic Violence: a felony or misdemeanor crime of violence committed by a current
or former spouse or intimate partner of the victim, a person with whom the victim shares a child in
common, a person who is cohabitating with or has cohabitated with the victim as a spouse or
intimate partner, a person similarly situated to a spouse of the victim under the domestic or family
violence laws of the jurisdiction receiving grant monies, or any other person against an adult or
youth victim who is protected from that person’s acts under the domestic or family violence laws of
the jurisdiction.
D. Definition of Dating Violence: a violence committed per a person who is or has been in a social
relationship of a romantic or intimate nature with the victim and where the existence of such a
relationship shall be determined based on a consideration of the following factors: the length of the
relationship, the type of relationship and the frequency of interaction between the persons involved
in the relationship.
E. Definition of Stalking: engaging in a course of conduct directly at a specific person that would cause
a reasonable person to fear for his or her safety or the safety of others, or suffer substantial
emotional distress.

F. Definition of Consent in reference to sexual activity: A voluntary, positive agreement between the
participants to engage in specific sexual activity. Consent must be clear and unambiguous for each
participant at every stage of sexual encounter.
G. Definition of “Proceeding”: The event or series of activities involving a formal or set procedure so
that a decision can be made about a claim.
H. Definition of “Result”: Something that happens as a consequence; outcome.
Efforts shall be made to protect the privacy of the complainants within the constraints of the law.
Retaliation against persons complaining of dating violence, domestic violence, sexual assault, stalking,
rape, sodomy, sexual assault with an object, sexual harassment, including sexual violence or violence
based on sexual orientation or gender identity is prohibited.

VIOLATION STATEMENTS
Policy: It is the policy of the Aveda Institute Des Moines that any behavior that is considered
inappropriate or disruptive is viewed as a violation. Violations will result in disciplinary action which
may lead to termination based upon the severity of the violation. 24e
Procedures for Minor Standard Violations:
Minor violations include, but are not limited to, assigned area violations, property misuses, guest service
violations, tardiness (separate violation), dress code violations, misuse of electronic devices, and
unprofessional behavior.
 1st offense results in a verbal (documented) warning by Educators.
 2nd offense results in a written warning by Educators.
 3rd offense may result in the student being sent home for the day, and the student may be
required to have a conversation with the Director.
 4th offense will be considered a major violation, and may result in a 3 day school suspension,
an 8th minor violation may result in a 5 day school suspension, and a 12th minor violation
may result in a 10 day school suspension. Further violations may result in termination.
Procedures for Major Standard Violations:
If at any time during the student’s program, the violation of a major standard will result in termination
with the exception of the 4th minor violation. If a student is terminated from a program for a major
violation, they will not be considered for re-entry into any program.
Major violations include, but are not limited to, using controlled substances, defacing or destroying
property, stealing personal or company property, falsifying documents or timekeeping, threatening
words or behaviors to other students or staff members, committing fraud, abusing or causing physical
harm towards others, possession or use of weapons, and violation of local or state laws.
If a student feels a violation was enforced unfairly, a student is encouraged to file a written grievance as
stated in the Aveda Institute’s grievance policy, to be submitted to Administrators for approval.

Unresolved Disputes
Grievance procedures are provided for students who believe that they have been unlawfully
discriminated against, unfairly treated, or harassed in any way. Academic grievances relate to a
complaint about a course, program of study, or grade.
Students are expected to address any disagreements or conflict directly with the individual involved, in
person, with a written document outlining the complaint and communication. After this, if there is no
satisfactory resolution, the student may set an appointment to see the Institute Director.
The grievance policy is as follows:
1. Complaints against students or Institute employees shall first be directed to the individual.
Complaints must be made within six months of the issue.
2. If the complaint cannot be resolved informally, then students shall write up the details and
submit to the Institute Director who will research the issue and respond with a resolution within
10 working days.
3. If there has been no satisfactory resolution, then the student may send to the Grievance
Committee a written statement of the grievance. The Grievance Committee will respond back
to the student within 30 days of the committee’s resolution.
4. All communications must be in writing and on file.

Policy: Lockers and stations furnished for student use belong to the Aveda Institute and therefore are
subject to search by school or police officials at any time for any reason. By entering onto the premises
of the school, students agree that they and any parcels they bring with them are subject to reasonable
search by school personnel at any time for any reason. Students are responsible for all of their
belongings.
Procedures:

If a student leaves the Aveda Institute by transfer, withdrawal, or extended leave of absence,
they need to take all belongings with them.

Items left in the locker and/or workstations will be disposed of after 5 school days in order to
provide space for incoming students.

Policy: The commission for Independent Education requires all Educators to have and retain their Iowa
occupational license in order to provide instructional services at any educational institution in the State
of Iowa. The Aveda Institute Des Moines requires educators to have a minimum of 12 continuing
education hours per year, with 4 of these hours falling under the discipline of Teaching Methodology,
and 2 falling under Sanitation/Iowa Law.
Procedure: Each year all of the Aveda Institute Instructors must attend Aveda training in addition to
Law required classes. Documentation supporting the continuing education will be placed in each
Instructor’s file. Instructors must provide copies of any continuing education documentation to the
Institute Director in an on-going basis as hours are earned.

evaluations. All administrative employees will be given a review 1 time per year, after the 1st twelve

VISITOR’S USE OF FACILITIES
Policy:
The Aveda Institute Des Moines loves offering guests within the Aveda network to use the space of our
facilities for training purposes. Any guest requesting to use the facility, must follow the correct
procedure and turn in a signed off checklist to the Director of Education following the class. Any guest
not following procedure may be asked not to use the facility space in the future.
Procedure:
Any guests requesting to use facility space must turn in an event request sheet NO LATER than one
month prior to the date of the class. Once the Director of Education has approved the event, the
contact person will be notified. The Director of Education will provide the contact person with a
cleaning checklist to be completed and turned in after clean-up of each event is executed.

FINANCIAL AID PACKAGING
Policy:
The Aveda Institute Des Moines understands the importance of being able to afford the best education.
The Financial Aid department determines the best combination of aid to meet individual student’s
needs.
Procedure:
When packaging financial aid, the department of Financial Aid considers types and availability of funds,
specific program eligibility criteria, and student variables. Once these considerations are made, aid will
be packaged in the order of grants, subsidized loans, unsubsidized loans, parent plus loans, and outside
financing options. All aid is borrower based and awarded by academic year, determined by the
student’s start date.

RETURN TO TITLE IV (R2T4)
Policy: The Aveda Institute Des Moines provides to all enrolled and prospective students information
about the institute’s refund policy, requirements of Return to Title IV funds, and requirements for
officially withdrawing from the school. The Consumer Information Packet explains the procedures for
the return of unearned Title IV funds. After the school has determined the amount to be returned, the
school will return all sums according to the following distribution: Federal Direct Loans, Federal Pell
Grants and the Student.
Procedure: The school will determine the amount of Title IV Aid to be returned in accordance with the
Department of Education Guidelines. Eligible Title IV Aid recipients who fail to complete over 60% of a
payment period are considered to have not earned all the Federal Aid that may have been previously
awarded. A required calculation will be performed to determine the portion of the unearned Federal
Student Aid that must be returned to the U.S. Department of Education. In many cases, the Return of
Unearned Title IV Funds calculation will result in the student owing tuition and fees to the Institute that
would otherwise have been paid with Federal Aid funds. This policy may also result in the student owing
a refund to the Department of Education.

VERIFICATION
Policy: Verification for information reported by students may be necessary. The Department of
Education requires a portion of FAFSA filers to be verified as selected by the Central Processing System.
In addition, the Aveda Institute Des Moines may choose or be required to verify additional students. All
verification should take place 30 days in advance of the students start date, and must be resolved within
30 days of enrollment.
Procedures: The school will provide a written statement via email to students selected for verification
within 30 days of the Financial Aid department being notified, with the following information:
documents required for verification, student responsibilities, and notification methods. Once notified,
the student will have 7 days to make corrections to FAFSA/DOE either on their own, or in the Office of
Financial Aid. Verification not completed 30 days prior to a student’s start date, may result in the
student not being able to start on the original decided date. If a student starts and verification is not
resolved within 30 days of enrollment, student will be withdrawn, put on official leave of absence, or
converted to cash pay. The Office of Financial Aid will refer overpayment cases to the Department of
Education through NSLDS.

ENTRANCE AND EXIT COUNSELING
Policy: All first time Federal Financial Aid loan borrowers are required to complete entrance counseling.
In addition, upon termination of enrollment through graduation or withdrawal, borrowers are required
to complete exit counseling.
Procedure: The Office of Financial Aid will not request money for students until entrance counseling is
completed. Entrance counseling may be completed on www.studentloans.gov either on the student’s
own time or in the Office of Financial Aid. The Office of Financial Aid will verify through the database
that all entrance counseling has been completed prior to requesting money for the student. Exit
counseling will be completed through www.NSLDS.gov and all students will be notified by the Office of
Financial Aid via email that this needs to be completed. The Office of Financial Aid will include in the
email, the PDF brochure of exit counseling procedure.

Policy: Student Records for Financial Aid are retained for no less than 3 years from the end of the last

during the student’s enrollment and for the three years following. Student records that exceed the
retention policy guidelines may be stored in a secure location either on or off-site or destroyed as the

Policy: The Aveda Institute maintains high standards and follows Federal Copyright Laws. Any
unauthorized distribution of copyrighted material, including peer-to-peer file sharing, may subject a
student to civil and criminal liabilities. Each year, students will be given a list of the Institute procedures
and penalties for violation of Federal copyright laws.
Procedures: Any work submitted to the Aveda Institute may be subject to be searched via the internet ,
other students work, or text book work, to verify legitimacy. If a student is found to have used or
distributed copyrighted material or to have shared data via file sharing, they may be subject to the
below listed civil and criminal liabilities as set by the Federal Government. In addition, the Institute
reserves the right to take internal disciplinary action up to and including student termination if
copyrighted or file sharing occurs.
Copyright infringement is the act of exercising, without permission or legal authority, one or more of the
exclusive rights granted to the copyright owner under section 106 of the Copyright Act (Title 17 of the
United States Code). These rights include the right to reproduce or distribute a copyrighted work. In the
file-sharing context, downloading or uploading substantial parts of a copyrighted work without authority
constitutes an infringement.
Penalties for copyright infringement include civil and criminal penalties. In general, anyone found liable
for civil copyright infringement may be ordered to pay either actual damages or "statutory" damages
affixed at not less than $750 and not more than $30,000 per work infringed. For "willful" infringement, a
court may award up to $150,000 per work infringed. A court can, in its discretion, also assess costs and
attorneys' fees. For details, see Title 17, United States Code, Sections 504, 505.
Willful copyright infringement can also result in criminal penalties, including imprisonment of up to five

Policy: The Aveda Institute Des Moines complies with Federal regulations on all policies and

ISSUING CHECK POLICY
Policy: Students issued a check from the Aveda Institute Des Moines are required to cash the check
within 180 days of the date the original check was issued. Any student needing a check re-issued from
Aveda Institute Des Moines, due to the student misplacing or destroying the check after it is initially
issued, will be charged a $29.00 service fee. This fee covers the amount of cost incurred to the Aveda
Institute Des Moines to stop payment on the original check issued.
Procedure: Any student needing a check re-issued must pay the Aveda Institute Des Moines’ business
office, $29.00 in cash or personal check form. Once the $29.00 is received from the business office,
Aveda Institute Des Moines will stop payment on the initial check and will provide the student with a reissued check for the original amount. When a check remains uncashed at the 180 day mark of the
original check issue date, the business office along with the financial aid office, will determine if any
uncashed balance contains Title IV funds and therefore is to be returned to the Department of
Education. Any Title IV funds to be returned will follow the “Order of Return of Title IV Funds” outline in
the Federal Student Aid Handbook. If uncashed balances are not Title IV funds, the check recipient will
be notified by mail that funds are available for a minimum of 30 days, after which funds will be sent to
the State Treasurer of Iowa. Uncashed checks will be monitored on a monthly basis.

AVEDA INSTITUTE DES MOINES POLICY AND PROCEDURE
COLLECTIONS POLICY
Policy: It is the policy of the Aveda Institute Des Moines that all students are given ample notification
and opportunities to pay on their student loan debt, prior to the loans being sent to collections. If a
current student is behind on their payments, they may not be able to clock in until payments are made
current.
Procedure: All students are responsible for payment of their student loans, regardless of what lender
the loans are borrowed from.
If a currently enrolled student falls behind on their payments:
TFC follows up with the student
ASG follows up with the student
Aveda payments: The Office of Financial Aid informs students that they have fallen behind on their
payments.
If a student who has graduated or dropped from the program is in good standing with TFC or ASG, then
the payments continue on with that servicer. Once a former student falls behind, the servicer can
cancel the loan and give it back to the Aveda Institute. If a loan is returned to the Aveda Institute, the
Office of Financial Aid sends a letter requesting payment within 10 days of postmark date, or the former
student will be sent to collections. The same procedure remains for graduated or dropped students
who have made payment arrangement with the Aveda Institute for their balance.
For Federal Direct Student Loans, the Office of Financial Aid will pull a monthly list of students in default,
and follow up with all students over 30 days past due via phone and letter, as stated in the Default
Management Policy. Once a student is 180 days past due, they will be referred to Department of
Education Collections and may be flagged as ineligible for additional federal student aid, until their
defaults are resolved. The Department of Education may also garnish wages and income tax refunds.

DEFAULT MANAGEMENT & PREVENTION PLAN FOR FEDERAL DIRECT STUDENT LOANS
Policy: It is the policy of the Aveda Institute, that all students borrowing federal loans, are given
counseling prior to receiving loans, throughout school, and upon graduation, to ensure students
understand the importance of paying back federal loans.
Procedure: Each student receiving Federal Direct Student Loans will meet with the Office of Financial
Aid to accomplish the following:
1. All FSA loan students complete entrance counseling prior to initial loan disbursement.
2. Periodic financial literacy seminars are provided while in school.
3. Individual counseling is provided to all students who are taking additional loan funds for living
expenses, including assistance with planning a monthly budget.
4. Within 30 days of graduation, all loan recipients complete on-line exit counseling and meet with
a financial aid administrator for personal loan exit counseling to review their loan balance and
the repayment process. (Individual counseling is also provided with drop students if possible.)
The appointment includes verifying current address, phone and email.
5. A series of reminder letters are sent to all graduates and terminated students per the following
schedule:
 5 months grace remaining
 4 months grace remaining
 3 months grace remaining
 2 months grace remaining
 6 weeks grace remaining
 4 weeks grace remaining
 2 weeks grace remaining
6. At least one time per month, the Office of Financial Aid will pull the NSLDS delinquent borrower
report and follow up with all students over 30 days past due via phone and letter. At this time,
current contact information will also be obtained.
7. Notify direct loan servicers of all changes in student contact information.
