You are the heroes of Mary’s Center - the ones saving lives and creating stronger communities, one family at a time. Your contributions of time, expertise, and financial resources have allowed us to continue our strategy of integrating health care, social services, and educational programs to help families achieve physical and mental health, feel supported in their communities, and move up the economic ladder. Our 2010 outcomes speak to the effectiveness of our programs. For example, 97 percent of babies born to mothers enrolled in prenatal care at Mary’s Center had healthy birth weights; nearly 100 percent of our teen program participants prevented pregnancy; and more than 90 percent of the adolescents participating in our after school enrichment program proceeded to college or trade school. In 2009 we focused on implementing electronic medical records and other robust initiatives to measure and improve our quality of services. This set the groundwork for 2010, the theme of which was building capacity. Through the American Recovery and Reinvestment Act (ARRA) and DC Department of Health tobacco settlement funds that were distributed by the DC Primary Care Association, we were able to fully renovate one center and to add a new site at 3912 Georgia Avenue. This enabled us to increase participant capacity from 18,500 in 2010 to a projected 24,000 in 2011. At the same time, we are just as concerned about maintaining the hallmarks that distinguished Mary’s Center since our inception 23 years ago: quality care, personal touch, and creating a space for members of the community to come together. If at the end of every day we are able to continue treating each participant who walks through our doors with dignity, compassion, and skilled professional care, we will consider ourselves successful. However, this quality and impact does not come without an extraordinary amount of human and financial resources. Like many other organizations, Mary’s Center has not been left untouched by the financial crisis, and we are continually facing the risk of losing funding that sustains essential programs. As you review this report, I hope the pictures, stories, and impactful outcomes will inspire you to partner with us to carry through with our commitment to the community of maintaining state-of-the-art facilities, professional and capable staff, and the highest quality of care. Their work is guided by the principle model that treating participants for their health problems in isolation is insufficient to help individuals overcome the multiple life challenges they often face. We owe our success to a strategy of integrating primary health care, social, and educational services to help individuals of all ages achieve physical and mental health, feel supported in their communities, and move up the economic ladder to become more productive and contributing members of society. We believe that individuals who are healthy and feel supported in their communities become better learners and are motivated to achieve greater economic stability. We have observed that stable families are able to place more emphasis on the educational achievement of their children, thus strengthening the well-being—and health—of the next generation. Social Services Health Care Education Our pioneering model has been acknowledged and applauded by many, including First Lady Michelle Obama during her visit to Mary’s Center in the first three weeks of the administration. After 60 hours in the program, 91 percent of families owned a library card. (Library card ownership is one indication of parents’ interest in furthering their family’s education.) Reading with children More than 90 percent of parents who completed the family literacy program reported reading with their children. High school graduation rates Almost 90 percent of teens we regularly monitor graduated from high school; the remaining 10 percent earned a GED. College entry More than 90 percent of teens we regularly monitor entered college or trade school. Mary’s Center 2010 Annual Report Community Impact and Saving Each year, Mary’s Center spends an average of $825 per participant. Based on our track record, we know that this small investment in the health and well-being of our families results in tremendous community impact and annual savings. For example: • Mary’s Center’s Family Social Services help to reduce the tremendous cost of child abuse and neglect in the United States, which bears a burden on families and society in the amount of $103.8 Billion per year because of adverse outcomes throughout the life span of abused children. • Mary’s Center’s prenatal care reduces neonatal complications, and the savings increase dramatically when considering the long-term costs for newborns with physical and developmental disabilities. In fact, the Centers for Disease Control and Prevention (CDC) estimates a savings of nearly $15,000 per low birth weight births prevented by women who receive adequate prenatal care. • Our educational programs ensure that our adolescents have the opportunity to go to college and create a better future for themselves and their families. Over a work life, earnings for an individual with a bachelor’s degree compared with one who only has a high school diploma increase by about $1 million. Mary’s Center 2010 Annual Report 9 Quality: Our Priority Mary’s Center has a dynamic Continuous Quality Improvement Program (CQI) which provides direction for organization-wide quality management efforts. By identifying opportunities to improve administrative processes and service delivery systems, Mary’s Center is constantly improving the quality of care and assuring the best outcomes for all of our participants and the community at-large. The program is driven by the Continuous Quality Improvement Committee and guided by the Chief Medical Officer. The CQI program utilizes a multi-disciplinary team approach through the organization involving the teen program, health promotion, social services and nutrition among others, as well as through administration, supported by the Outcomes Manager. The CQI Committee meets monthly with activities addressing ongoing and future projects. There are quality HEDIS measures quarterly, reporting of patient and staff satisfaction and patient access measures in an environment encouraging active and open dialogue with the team. All quarterly reports are presented in a dashboard style to the Center’s Board of Directors. 10 Mary’s Center 2010 Annual Report A specialized 360 Peer Review Process was developed which not only includes quarterly peer chart reviews within our medical, social and educational programs but involves a supervisory review and one-on-one discussion in order to make improvements immediately. Mary’s Center has participated in a number of quality improvement projects with external organizations. We recently completed our second year of participation in the DC Primary Care Association sponsored by CareFirst BCBS, a, Quality Transformation Series sponsored by the Montgomery County Primary Care Association, and an Office Practice Redesign which was developed by Institute for Healthcare Improvement. All these projects provided techniques to improve and measure quality with benchmarks demonstrating quantitative successes. We were also recognized by the Delmarva Foundation for three consecutive years for “Excellence in Prevention through EHR Technology”. In addition to prenatal care, our mothers-to-be and their partners receive information on nutrition, exercise, labor, delivery, and lactation. Our doctors, nurses, health educators, and social service professionals use an interdisciplinary team approach to deliver the highest quality care. Mary’s Center is certified by the D.C. Department of Mental Health as a Core Service Provider. Our bilingual mental health services include diagnosis, assessment, counseling, case management, and prescription/monitoring of somatic medications. Substance abuse screening, referrals, and post detox counseling are also available. In 2010, 6,248 participants benefited from our Social and Mental Health Services. Our team works with our participants to support them on their smoking cessation goals, diabetes management, and breast cancer prevention, HIV testing and counseling in addition to sponsoring ongoing Fitness Activities to help instill the importance of fitness, relaxation and socialization. Mary’s Center 2010 Annual Report 13 Going Portable in 2010 Solutions that work When Dr. Gesker joined Mary’s Center in April 2010, our dental services were limited to a dental chair inside our mobile unit, the Mama & Baby Bus, and provided only basic services: screenings and cleanings. A few months later, Dr. Gesker added two innovative portable dental chairs and portable sinks inside our clinic increasing our productivity by 60%. Our array of dental services also increased: cavities, root canals, dentures and more. Today, the dental clinic has five permanent dental chairs. Mary’s Center allows access to dental services that have been so limited in the District due to the shortage of providers, especially for underserved communities on Medicaid or without health insurance.
