Name Choice: The person submitting this application has the right to choose a name for
the dog. BY A reserves the right to add a numeral suffix or change the name as
circumstances demand. BYA furthermore reserves the right to refuse any name that they
feel is not an appropriate name.
Send to: Completed form is to be sent to the address on the front with appropriate fees.
Incomplete forms, undated, no signature and/or no fees will delay processing or result in
the form being returned unprocessed.
Ownership: The dog may only be registered in ONE person's name. Forms submitted
with more than one person listed as the owner will be returned unprocessed. The person
submitting this form for registration 'attest that all of the information and facts included
on this form are true and correct to the best of their knowledge and the animal is a good
specimen of the breed and does not exhibit any of the breed standard disqualifications ~.
Liability: This application, upon receipt, becomes the property of said registry.
Liability shall be limited to a refund of registration fees only. No other warranties, or

Dog Description: BREEDERS please be sure that the form is filled out completely in
the area of Coat Color, Pattern, Coat Type, Eye color and Nose color as Registration
Certificates can not be issued without this information.
Transfers: Submit a completed Transfer Form, for each person who has owned the dog,
when a dog has been transferred more than once at the time of applying for registration in
the name of the current owner. Only the current owner is allowed registration privileges
unless individual fees are paid for each transfer.
Members: There are no registry members. You are granted the privilege of registration
as outlined in our Policies Manual.
