 For video instructions on how to knit hats with a loom and what supplies
to use, go to www.youtube.com/evangehats.

3. Complete
 Knitter form – Go to www.JFHP.org/resources/evangehat and print one
form per knitter.
4. Pack five (5) EvangeHats in a clear 1 gallon Ziploc bag and seal.
5. Obtain a shipping box and place packs in box to be shipped. (We recommend
sending five (5) Ziploc bags per box.)
6. Send a check(s) – made payable to JESUS Film Harvest Partners – in the amount
of $2.00 per pack of five hats (for handling and shipping cost during distribution
process).
7. Mark in the memo line of the check(s), EvangeHat.
8. Insert check(s) along with participant forms into envelope(s), seal and place in the
box; seal box and ship to:
