When you go to make a deposit at an ANZ bank you will need to fill out a deposit form to take to the bank teller. Below is an example
of how to fill out one of these forms.
This is your deposit slip, you
will be given this as proof of
your payment. Please keep!

You will need to write and sign your
name, and also leave a contact
phone number. (Use your host’s
number if you don’t have one.)
The name of the account is

Account.
Make sure you correctly enter
today’s date, and how you are
making your payment.
