To be recognized as the leading provider of care in the region and a national model for quality, service excellence, and fiscal stewardship. 1 0 0 Y E A R S Dear Neighbors, he past three years have been very busy and exciting for our home care and hospice agency—there has been constant change! In September of 2012, Neighborhood Health celebrated 100 years of service to our community. Reflecting on our beginnings, I find it comforting to know that although technology, medicine and transportation have changed greatly over these 100 years, our mission and caring have not. We were founded to serve those in the community with no access to health care. From newborns after delivery, to school nursing in the Radnor school district, to post-injury care, through death—all of our services were paid for by charitable donations and in-kind payment. Our mission has always remained firm and steady—to provide care to the members of our community, to provide the answers and be the educators, comforters, and friends who help our neighbors stay home where they want to be to heal and recover or to die in comfort surrounded by their loved ones. On September 1, 2013, as a member of Chester County Hospital and Health System, we became a part of Penn Medicine. This has already opened doors for us to not only contain costs, but to expand the flow of patients. We are so excited for this partnership to continue and flourish! In September 2013, as a member of The Chester County Hospital and Health System, Neighborhood Health became a part of Staff in home care have, for three years in a row, achieved the HomeCare Elite™, indicating our high quality care provision. Our hospice department has assimilated a Palliative Care Program—designed to improve quality of life by focusing on pain and symptom management, combined with counseling and support for patients and families. In the past year, our home care department has grown by 25% 4 of CARING Our home care department has grown by 25% over the past year and Senior HealthLink has expanded the number of visits and programs it provides in the community, while hospice volume has dropped by 15%. We think this is due to the many treatment options available to patients previously considered “terminal.” We continue to look for ways to rebalance our care provision to the community. Technology continues to drive much of the change we see every day. Over the past year, we have been converting our home care staff to a new computer documentation program, which will aid the staff in their assessments and patient goal planning. Hospice staff will be upgraded next, most likely by summer 2015. Donations support Senior HealthLink’s free community outreach as well as offsetting the costs of caring for hospice patients who are un- or under-insured. Agency and Chester County Hospital staff, volunteers, patients, families, and community members enjoyed an evening of food and drink, dancing, a silent auction and agency history. The event raised over $8,000 for our Palliative Care Program! A nationally-recognized falls prevention program developed at Boston University, Matter of Balance helps participants learn to view falls and fear of falling as controllable, set realistic goals to increase activity, change their environment to reduce fall risk factors, and exercise to increase strength and balance. The program is presented at various senior centers, senior housing facilities and senior living communities throughout the county.
