GUIDELINES FOR COMPLETION
To apply for a trade account with D­ZERO s.r.o. please follow the steps below. If you
do not follow these guidelines or omit to send information to D­ZERO, we regret your
application may be delayed.

Once fully complete, please print and sign the trade application.
Then fax the following to D­ZERO on +42 0378880023:

1. Opening an Account
We welcome companies who would like to open a trade account with D­ZERO. To do this please
contact sales on +42 037888029 or fill out the trade application form.

We will contact you regarding your application within 2 working days of receipt.
2. Ordering
D­ZERO currently offer a number of methods for placing an order,
1. Orders can be processed via SKYPE
2. Telephone orders between the hours of 8.30 am and 5.00pm.
3. Orders can be processed via fax and email
Please note that to ensure that we continue to provide the best possible service to all our customers and
ensure we are not dealing with end users, we require accounts to maintain an average minimum monthly

We endeavour to ensure all orders processed by 4.00pm are delivered next day, however, please place
your order as early as possible. For an extra cost, Same day, AM and Saturday deliveries are also
available.
Order Cancellation/Amendment
We regret that once orders have been received by D­ZERO, they cannot be changed or added to. Any
amendments or additions will be treated as additional orders and will therefore incur any standard
minimum order and delivery charges.
