How to enter Functional Limitation (G-Code) data into Casamba SMART
There are three ways to get to the Functional Limitation data input screen. First, and easiest, is at the bottom of the Tx
Input screen (where there is also a ‘warning’ on the left with information about when the assessment is due).
At the bottom of the Tx Input
screen is the easiest link to the
Functional Limitations Module
On the Tx Resident tab, look for the Functional Limitations button in the lower right corner
IF using the Tx Resident tab to
enter Therapy Admissions, the
button to the Functional
Limitations Module is found here.
Under the “Documentation/POC” tab, one will find the Functional Limitations button on the POC, under the Treatment
Plan tab, in the lower right corner.
On the POC (Doc/POC module)
go to the last tab (Treatment
Plan).
Look in the lower right corner to
find the Functional Limitations
button.
After clicking on the Functional Limitations button, a new screen opens within the window.
After clicking on the
button, the
Functional
Limitations screen
opens within the
software.
For ease of viewing, the screen shots below are enlarged.
To enter G-Code data for the first time, one simply begins at the top (date) and completes the information in the drop
down boxes. This is demonstrated in the next few screen shots:
Start by using the drop down and
entering the date.
One note: Be sure the date
chosen is one in which treatment
time was entered.
Next, select the appropriate G
Code from the drop down list.
Choose the correct current level
modifier for this G-Code.
Choose the correct goal level
modifier for this G-Code.
If we try to ‘save’ without
entering documentation, we see
this error.
Click on “Set Note” to enter the
documentation.
This opens the Documentation screen, complete with a library of selections. Use one of these selections. Free text after
if you wish, but first use one of the drop down selections by clicking on “Select” to the left of the chosen item.
Click on the word “Select” to the
left of the menu item we choose.
Please do not free text until
AFTER choosing one of these
dropdown items.
Clicking on “Select” moves the item
to the ‘working zone’.
Once in the working zone, we click
anywhere in the red text to ‘fix’ this
by replacing with the desired text.
Attempting to save (“Set”) before one
replaces the red text results in this error.
So now we know we need to enter documentation. We know we click on the Select button to move this to the working
area. We click anywhere in the red text and the entire text is then selected. We type the text we want to replace this.
Then we click on “Set” in the lower right corner to ‘Save’ the text.
Click anywhere within the red text and
all red text is selected. Begin typing the
correct (replacement) text.
After using one of the drop down items,
one can free text as much additional
information as they want.
After the note is written, click on
“Set” to save.
Like the daily notes, once the
documentation is entered, the “Set
Note” becomes bold.
The only step left is to click on the “Save” button.
At any point, we can see what was entered by clicking on the “View History” button. The screen below is the one we see
the second time we return to the Functional Limitations area and is discussed in follow up screen shots.
When coming back into the Functional
Limitations module, one can click on the View
History button to see information that has been
entered.
This shows the screen that comes up when we click on the “View History” button.
When coming back into the Functional Limitations Module, the screen looks a little different. Several areas are grayed
out.
However, as soon as we enter a date, the grayed areas populate with the previous information so we only need to enter
“Today’s Level” for this G Code.
Start by entering the date…and the previously
entered information will pre populate.
The next step is to enter the modifier for the
currently ‘active’ G Code.
As in the past, attempting to save without the
supporting documentation results in an error.
Instead, click on the “Set Note” hyperlink to get
to the documentation screen.
As in the past, click on the “Select” to the left of the
chosen drop down item. This moves this item to the
‘working area’ where we can click inside the red text to
replace this. After using a drop down item, we can free
text as much as we want. The note we are working on
also displays on the right side of the screen.
For reference, the previously entered note
shows in the bottom of the screen.
In the bottom left side of the screen, there is a
question asking if we “want to DC the Functional
Limitation. This is preset to “No”. However, this
can be switched to “Yes” when appropriate.
When we choose yes, another question appears
asking if this patient will remain on Therapy.
If we say “Yes”, then the Functional Limitation
options return and we complete this just like we
did previously with the chosen G Code, Current
Modifier, Goal Modifier, and the Documentation
supporting our reason for choosing this modifier.
Every tenth visit, or on the day of a planned discharge (last visit), we return to the Functional Limitation Module to again
enter G Code information. There is an alert on the bottom of the TxResident tab, the Tx Input tab, and on the daily
schedule to remind us when it is time to complete another G-Code entry.
When due, return to the Functional Limitations module as we did previously by clicking on the “Functional Limitation”
Button on the Tx Input screen (easiest), the Tx Resident Screen (Therapy Admission), or on the POC (under
Documentation/POC module).
The easiest way to get back to the
Functional Limitations Module is
from the Tx Input screen.
Click on the Functional Limitations
button in the lower right corner.
