! Employees must elect and/or renew benefits through BannerWeb from November 3rd14th.

! Health Insurance rates will increase by 4%
• High Deductible Health Plan (HDHP) – Employee Only coverage will continue to
be offered with no premium.
! High Deductible Health Plan (HDHP)
• The HDHP is an underutilized plan that might suit the needs of you and your
family. Feel free to take the survey, found at
http://www.clearviewlogix.com/richmond/, to help you determine if the HDHP
Plan might be best for you.

• Deductibles are changing for 2015 - $2,600 Individual/$5,200 Family
• Open Enrollment Informational Sessions will also focus on HDHP education:

! Health Savings Account (HSA)
• The University contributes $500 (individual) or $1,000 (family) annually into the
employee’s HSA. For plan year 2015 ONLY, the University will deposit the full
amount into the account on January 1, 2015.
! In response to service concerns encountered by employees, we have moved away from
Express Scripts and back to Anthem for prescription (Rx) coverage.
• Employees will have one Anthem card for both health and Rx.
! Dental Insurance
• Rates have increased by 2% for the first time in 3 years.
! Benefit Fairs - November 6th, 12th & 14th – 9:00a.m.-4:00p.m.; Weinstein Recreation and
Wellness Center
• FREE Flu Shots – All active employees and their spouses; see below for
information on how to schedule a time.
• FREE Health Screenings and Assessments - Human Resources, Recreation and
Wellness and HCA invite all full-time employees to participate in a confidential
health screening at the Benefit Fairs. Those who participate will receive two FREE
movie tickets.

Renew your current benefits and/or make benefit elections online through BannerWeb
beginning November 3, 2014 and ending November 14, 2014. REMINDER: Step Four will ask
you to confirm that the dependents you are covering are indeed your dependents based on the
definition of dependents found in the plan document on the HR website
(http://hr.richmond.edu/benefits/insurance/medical/Wrap%20Document%2011.19.13.pdf).

Complete all five steps of the online enrollment process through BannerWeb. If you do not
complete all five steps, we cannot guarantee that your benefits will renew for 2015.
"

The benefits you elect or renew will be effective January 1 – December 31, 2015. You may not
change your elections during the year unless you have a status change. Even then, only certain
changes are allowed.

HEALTH INSURANCE

We are continuing to offer the High Deductible Health Plan (HDHP), Healthkeepers (HMO),
and KeyCare (PPO), for 2015. Please see the Summary of Benefits Coverage for all three plans at

Reminder: You may cover children until the end of the year they turn 26.
o This includes:
o Adopted
o Step Children
o Foster Children
o Children of Same Sex Domestic Partners (tax implications may apply)
PRESCRIPTION DRUG COVERAGE

INSURANCE WAIVER

If you are currently waiving enrollment in the University's health insurance program and plan to
waive again for 2015 on the same basis as in 2014 (i.e. your selection of money or vacation days
will also be the same), you need to confirm your selections in BannerWeb. You may make a
change to this benefit by following the directions in BannerWeb.

If you are currently enrolled in a University health insurance plan and wish to waive coverage for

2015, follow the directions in BannerWeb (http://bannerweb.richmond.edu). You will be asked to
complete a form and provide proof that you have other medical insurance coverage. This proof
may consist of a medical insurance card listing you as a covered participant or a letter from
another employer or insurance company specifying your coverage. Human Resources cannot
verify coverage for you. You must provide the information required.
DENTAL INSURANCE

" We are continuing to offer Anthem Dental Insurance for 2015 with no general plan design

FLEXIBLE SPENDING ACCOUNTS (Health & Dependent Care through Flores)

Flores & Associates will continue to provide flexible spending accounts for UR employees.
"
Reminder: If you are going to enroll in the High Deductible Health Plan (HDHP) and have a
Health Savings Account (HSA), you are not eligible to enroll in the Medical Flexible Spending
Account.
"
Expenses for non-prescription drugs (i.e., over the counter drugs) are not eligible for
reimbursement from your flexible savings account.
"
The Health FSA allows you to pay for out-of-pocket health expenses on a pre-tax basis. You
may want to consider using it to cover out-of-pocket expenses like your deductibles and
coinsurance from the medical, prescription drug, dental, and vision plans. You may contribute
between $100 and $2,550 annually.

Dependent FSA is for daycare expenses of dependent children between the ages of 0-13 years
old. You may contribute between $100 and $5,000 annually.
"
Flores cuts reimbursement checks on a daily basis. Claims may be submitted by mail or fax.
Participants may request direct deposit into their bank accounts. Log on to https://www.floresassociates.com/sflex/default.asp to view the medical reimbursement schedule.

If you currently have a medical or dependent care reimbursement account for 2014, you may
continue to submit claims for expenses through March 31, 2015 to Flores. Please remember
that all claims for 2014 must be incurred by December 31, 2014.

If you wish to participate in either the Health FSA or the Dependent Care FSA in 2014, YOU
WILL NEED TO ENROLL IN BANNERWEB. If you are currently participating in these
plans, you must enroll for 2015. Failure to enroll by 5 p.m. on Friday, November 14, 2014 will
negate your opportunity to participate in the medical/dependent spending accounts for the year
2015.
HEALTH SAVINGS ACCOUNT (HSA)

An HSA is an account that functions much like an IRA and a health flexible spending account.
Employees may invest money, reduce taxable income for the amount invested, and use the money

without penalty for eligible medical expenses. The interest gained over the duration of the account
is also tax-free.

HDHP participants may participate in the HSA
o You cannot contribute to the HSA if money is still in your FSA after 1/1/15.
o REMINDER:
o The University’s FSA allows for up to $500 unused money to roll to the
following year.
o If there is a $0 balance on 1/1/15, then you can open the HSA and contribute.
o If a spouse is enrolled in an FSA and they have a different plan year than the
University, employees can only contribute to the HSA once the spouse’s plan
year is over.

HSA participants may not participate in the medical flexible spending account (FSA).

If an employee is eligible for Medicare, they may not be eligible for a HSA.

2015 Federal HSA Contribution Limits
o Employee only - $3,350
o Employee plus dependents - $6,650
o Catch-up (age 55 by 12/31/14) – additional $1,000

The annual University Contribution to HDHP participants (For plan year 2015 ONLY, the
University will deposit the full amount into the account on January 1, 2015.)

Investing in an HSA – Contributions to the HSA may be made via payroll deduction and/or
directly to the account. Participants are immediately vested in their HSA, and there is no “use it or
lose it” stipulation. It is the participant’s account—it is portable and can be accumulated over time.

VOLUNTARY VISION (Coverage Provided by UniView Vision)

The University of Richmond offers a Voluntary Vision plan through UniView Vision, one of
the largest vision care networks in the industry. When you choose to receive care from a
UniView Vision participating provider, you receive full in-network benefits and discounts.

This coverage is for routine eye care only; if you need medical treatment for your eyes, you
must visit a participating eye care physician from your medical network.
"
UniView Vision providers cover a wide range of ophthalmologists, optometrists, and opticians,
including retail locations such as 1-800-CONTACTS, LensCrafters, Target Optical, Sears
Optical, JCPenney Optical, and Pearle Vision.

The University offers employees the ability to purchase additional life insurance for themselves
in increments of $10,000 up to five times their salary or $500,000, whichever is less.
Employees will need to complete evidence of insurability.

Employees may purchase life insurance for their spouse/same sex domestic partner in
increments of $10,000, up to $50,000. Those applying for coverage will need to complete
evidence of insurability. The employee must be enrolled in order to purchase coverage for
a spouse/same sex domestic partner.

Employees may purchase life insurance for their dependent children in increments of $2,000
with no medical questions asked, up to $10,000. The employee must be enrolled in order to
purchase coverage for a dependent.

The cost of coverage is based on the applicant’s age and the amount of insurance desired.

A Cigna representative will be available at all three Benefit Fairs if you have questions.

If you are currently enrolled in the plan, and do not wish to make a change to your Voluntary Life
Insurance, YOU MUST CONFIRM YOUR 2015 ELECTIONS THROUGH BANNERWEB.
Please remember if your age changed during 2014 to an increment of five (25, 30, 35 and 40 etc.),
your premiums will change in January 2015. If you wish to terminate coverage, you may do so on
BannerWeb.

FREE flu shots will be administered by Health Advocate to all active employees and their
spouses at the Benefit Fairs.
You can pre-register by calling 800.970.1263 or online at
https://members.healthadvocate.com/. Walk-ins are also welcome at the Benefit Fairs.

Health Screenings and Assessments
Human Resources, Recreation and Wellness, and HCA Virginia invite full-time employees to
participate in a FREE and CONFIDENTIAL health screening at the Benefit Fairs.

Employees will be asked to complete a brief health profile and consent form with general
information (name, address, DOB) and basic health questions

Health screenings (Height/Weight/BMI, Blood Pressure, Cholesterol, HDL Cholesterol and
Blood Glucose) are administered at the Benefit Fairs
Employees may schedule their screening and reserve space by visiting
https://hcavaworkplacewellness.acuityscheduling.com/schedule.php?calendarID=100027 and
choosing a preferred date and time. Walk-ins are also welcome at the Benefit Fairs.
Employees who participate will receive a one-on-one consultation with a healthcare
professional to review and discuss their screening results
Full time employees who participate will receive two-free movie tickets and a print out of the
results
Employees may take the RealAge test to calculate your body’s health age based on your
nutrition, fitness, and sleep habits at the Benefit Fairs
HCA Virginia’s Wellness Portal will open approximately two weeks prior to the screening
event. Additionally, the wellness portal allows employees to have access to great wellness
resources including fitness/nutrition/sleep tracking, along with health content and videos from
national and local sources.
LEGAL RESOURCES (Prepaid Legal Care)

The employee cost for this benefit remains the same for 2015 at $18.00 per month. Once enrolled,
the participant must remain in the plan for 12 months.

If you are currently enrolled in this plan, and do not wish to make a change in your enrollment in
the prepaid Legal Care benefit, YOU MUST CONFIRM YOUR 2015 ELECTIONS THROUGH
BANNERWEB. Enrollment and/or termination may be completed through BannerWeb.

If you are enrolling for the first time, you must enroll through BannerWeb, then also go to the
Legal Resources website at https://legalresources.com/enroll_now (Company ID: 264; Password:
uorlegal) to choose your attorney and list your dependents. If you do not choose an attorney, one
will be assigned to you.

A Legal Resources representative will be available at all three Benefit Fairs if you have
questions. In addition, there will be a Legal Resource Information Session held on Friday,
October 31st in Weinstein room 221 from 10:00 – 11:00am.
HEALTH ADVOCATE PROGRAM

Health Advocate is the benefit provided by the University to help full-time employees and
their spouse/same sex domestic partner, dependent children, parents, and parents-in-law
navigate the healthcare system and maximize their healthcare benefits.
"
Health Advocate will assist you in solving healthcare-related issues. To get help sorting
out billing concerns, scheduling specialized treatments, transferring medical records,
finding eldercare, and more, call the toll-free number at 1-866-695-8622. A Personal
Health Advocate (PHA) will work with you one-on-one to find solutions to these timeconsuming issues.

FREE & CLEAR QUIT FOR LIFE PROGRAM (provided by Anthem)
