Admission are open in Doctor of Pharmacy (Pharm-D)
2014-2015.
 The application form / prospectus can be downloaded from
JSMU website www.jsmu.edu.pk from 21-12-2014 to
04-01-2015.
 The application form along with required documents and
paid voucher of Rs. 2000/- to be submitted to admission cell
via courier service.
 Voucher of processing and entrance test fee will be auto
generated on submission of online form to be paid at any
branch of HBL Account # 065-79007294-03.
Admission Rules & Regulations






The decision of the selection board shall be final in all admission matters.
The selection board may investigate any documents and may call any candidate for that
purpose.
The selection boar shall have the right to refuse admission to any candidate, who indulges in
violence, use abusive language, becomes insolent or misbehaves with the members of the
selection board.
The selection board shall not consider any candidates for admission if he/ she is found to be
involved in any form of unfair means/fraud at any stage of admission/selection.
Any candidate found using Domicile /PRC of more than one district /place in Pakistan will be
disqualified for admission at JSMU, in any other medical institution of Sindh province and the
case will be referred to the concerned authorities for investigations.
In case of any controversy, the decision of the Selection Board shall be final.
Eligibility Criteria
 Inter Science Pre-Medical or Equivalent examination from Karachi.
 Minimum marks of 60% in Inter Science (B.Sc students may also apply)

Admission letter will not be issued without submission of pay orders * 10% increase in

Entrance Test:
It will be held at Karachi for categories of, Open Merit and Self Finance.
 The entrance test will be conducted for the eligible candidates on 04-01-2015. The venue is
Sindh Medical College, JSMU, Rafique Shaheed Road, 11:00 AM.
 Candidates who have acquired less than 60% marks in the Inter science or A level exam will
not be allowed to appear in the Entrance Test.
 No candidate will be allowed to appear in the entrance test without the admit card /
Original CNIC & paid processing fee voucher.
 All male and female students will only be allowed to appear in the entry test after being
identified with complete face exposure by authorized persons. The Entrance test will be
based on the following subjects.
a) Biology………………………30 questions
b) Chemistry………………….30 questions
c) Physics………………………30 questions
d) English……………………….10 questions
All the questions will be from the current syllabus of the Board of Intermediate and
Secondary Education Karachi.
Documents to be sent:

Matric Certificate/ ‘O’ level or equivalent examination certificate.
Mark sheet of Matric /transcript ’O’ level or Equivalent examination.
Inter science / A- level mark sheet or transcript.
Provisional certificate from college.
Candidate’s domicile certificate of district Karachi / interior of Sindh.
Candidate’s permanent resident certificate (PRC, or Form- C) district Karachi/
interior of Sindh.
 Candidate NADRA National Identity card (CNIC) OR ‘B’ form, if he/ she is
below age of eighteen years)
 Father’s Domicile of Sindh province on form P- 1.
 Two recent passport size photographs
NOTE:

No mark sheet whether original or revised will be accepted after entry test.

Candidates are also advised to fill online forms available on JSMU website www.jsmu.edu.pk.

On submission of online form fee voucher will be generated along with the receipt of the form.

Send the hard copy of application form, receipt of online form and paid processing fee voucher along
with above required documents upto 02-01-2015

