Dr S CHAPLIN ROGERS BDS (Honours) LDS RCS (Eng) MBBS
Qualified: Dental Surgeon 1978; Doctor: London 1985
Joined the practice in 1990 after training in London Hospitals and completing a registrar’s post
in Putney. He is a general list doctor within the practice and spends one day per week in an NHS
dental practice. He is involved in Local Medical and Dental Committee work and enjoys time with
his young family, sailing and tennis.
Dr M RICKENBACH MBBS DCH DFFP FRCP FRCGP PHD ILTM
Qualified: Royal London Hospital 1984
Joined the practice in 1993 after training in medicine at Portsmouth. He has a particular interest in
cardiology, chest, paediatrics and eye conditions. Within the practice he undertakes minor surgery
and IUCD (coil) insertion. He is a GP trainer and an Associate Dean in Postgraduate Medical
Education and Research for Wessex, and visiting Professor to Winchester University. He has three
children and enjoys sailing and badminton.
Dr K FOWLER MBChB MRCP MRCGP
Qualified: Manchester Medical School 1981
After further experience in medicine and paediatrics Dr Fowler joined the practice in 1994. She has
a special interest in general medicine, respiratory medicine, paediatrics and gynaecology. She is a
qualified GP trainer. She is married with three children and enjoys skiing, swimming and running.
Dr C ARDEN MBBS BSc MRCGP
Qualified: London 1992
Dr Arden has a BSc in Physiology (1988), subsequently undertaking hospital training in General
Medicine, Paediatrics and Obstetrics, and General Practice training in West Sussex. Within the
practice he undertakes minor surgery and has an interest in cardiology, including working two days
a week for Southampton Community Healthcare in a community based cardiac clinic. He enjoys
sailing, football and music.

Qualified: Royal London Hospital 1991
Joined the practice in 2001 having previously worked in Chichester. Prior to training as a GP she
had worked and trained in general dermatological surgery. She is married with a young family.
Dr J O’CONNOR MBChB
Qualified: Leeds University 1984
Joined the practice in 2002 after having served in the Royal Army Medical Corps and as a general
practitioner in Leeds and Romsey. She moved to Hampshire with her husband and two children in
1999. Her special interests include women’s medicine, particularly the management of PMS and
the menopause, depression and rheumatological disease.

Qualified: Southampton Medical School 1998
He joined the practice in 2005 after gaining entry into the Royal College of General Practice. He has
a BSc (Honours) in physiology and biochemistry and spent a year in research. His hospital training
included paediatrics, general medicine and intensive care. He has a keen interest in respiratory
medicine. He is married with a young family and his interests include cycling, running and the
obligatory gardening.
Dr J BIRMINGHAM MBBS MRCGP DCH DFFP
Qualified: Newcastle 1990
She has been working in the practice as a salaried doctor since 2001 and became a partner in April
2006. Previously she worked in the North East and Birmingham. She is married with four children.
The Practice
Our two sites are centrally located in Chandlers Ford and provide a full range of services for over
15,000 patients. We have 8 GPs which enables us to offer our patients a greater breadth of
experience as well as a wide range of skills and areas of interest. We are a teaching practice and
often have trainee doctors on site as well as participating in various research projects.
Park Surgery was built in the 1970s and St Francis Surgery was purpose-built in 2001 to expand
the services available to the community, including the St Francis Dental Practice. Patients can be
seen at both sites and all doctors and staff work at both sites.
The current practice area is shown on the back cover and we are part of the NHS local Clinical

The Practice Staff
Busines and General Practice Manager
Our Business and General Practice Manager, Sue Levesque, oversees the day-to-day administration
and operation of the practice.
Nursing Team
Our nursing team comprises of Jane Tutton, Kate Lockhart, Clare Sharland, Paula Say, Michelle
Bonsey, Sara Payne and Emma Billett and are assisted by our health care assistants Ann Davis
and Maureen Webb. Our nurses see patients by appointment. They are involved in all screening
programmes as well as taking blood samples, ear syringing for medical concerns, routine blood
pressure checks, undertaking cardiograms and cervical smears. They can offer advice on numerous
health topics including travel advice. All are competent and fully qualified for their roles.
AdministrationTeam
In addition to the Business and General Practice Manager, we have two medical secretaries, and
an administration team which looks after the office functions of the practice.

Reception Team
Our reception team of 14 is led by Julie Richardson and undertakes the day-to-day work of
reception including the front desk, booking appointments, dealing with general enquiries and
providing support for all other areas of the surgery. All of our staff will respect patient confidentiality
in the course of their duties.
Health Visitors
The local health visitor team are based at Park Surgery and undertake their regular baby clinics at
St Francis Surgery.
District Nurses
The local district nursing team are based at Brendon Care and care for patients in the Chandlers
Ford area who need nursing at home.
Midwives
If you cannot keep your appointment, please let us know so that we can offer it to another patient.
A single appointment is for 10 minutes. Should you wish to have a longer consultation, please
ask the receptionist when making the appointment as this will help avoid delays for other patients.
You may be accompanied by a relative or a friend at any consultation, but please introduce them
to your doctor at the beginning of the consultation. If you would like a chaperone to be present
during any intimate examination, please ask the clinician before the examination commences.
We do not offer a walk-in service.
Our extended opening hours: Every Saturday at Park Surgery, from 8.30 to 11.00am, we offer
routine appointments with the GP and Chronic Disease appointments with the Practice Nurse.
Please note these are for PRE-BOOKED appointments only and unfortunately we are unable to
give out prescriptions or deal with emergencies during these extended opening times.
The practice is supported by three midwives based at St Francis Surgery.
Emergencies
Registration
If there is a life-threatening medical emergency, please call an ambulance by telephoning 999.
For other emergencies please telephone the surgery option 2 and speak to a receptionist. A doctor
or nurse will contact you to assess the problem and the best way forward.
Patients register with the practice and are able to see the doctor of their choice. In the interest of
continuity, we assign each new patient to a “usual” doctor and would suggest that you consult
with this doctor for any ongoing problems.
When you register with us you will be asked to complete the GMS1 form and a supplementary
information form. It is important to complete all forms in full so that your medical record can be
brought up to date immediately. (It can take several weeks for your record to arrive from your
previous doctor).
APPOINTMENT SYSTEM
The practice can be contacted from 8.00am to 6.30pm Monday to Friday for emergencies and
general enquiries. The appointment lines open from 8.15am to 12 noon, then 2.00 to 6.00pm.
We offer a range of appointments throughout the day including urgent appointments for the same
day, telephone consultations and routine appointments which can be booked up to six weeks in
advance.
Before making an appointment, please consider whether you need to see a doctor, whether an
appointment with a nurse may be suitable, or whether your problem may be sorted via a telephone
consultation. To make an appointment, please telephone 023 8025 2131 and select one of the
following options:
Option 1 - for routine appointments with a doctor or nurse. For routine appointments you will be
offered the first available appointment with the doctor/nurse of your choice, or another doctor/nurse
if this is not convenient. Please try, when possible, to see the same doctor for a particular problem.
Option 2 - Medical Emergencies. This connects you directly to the EMERGENCY desk where
urgent advice or assistance will be arranged.
Option 3 - For an urgent appointment today or medical advice. The trained duty team will assess
your situation and offer advice, arrange a prescription or a same day appointment as appropriate.
Option 4 - For general enquiries.

Requests for Home Visits
For housebound patients, please try telephoning the surgery before 10.00am. We ask that requests
for non-urgent emergency home visits are also made before 10.00am.
Please call 023 8025 2131 to talk to a receptionist. You will be asked for some medical information
at this time, which will help us to prioritise the visit.
Telephone Consultations
If you feel your problem might be resolved by a telephone call, an appointment will be given and
the doctor will call you.
Out-of-Hours Arrangements
NHS England has responsibility for the provision of all out-of-hours services. The current
arrangements are provided when the surgery is closed (ie from 6.30pm to 8.00am, throughout
weekends and bank holidays). In the event of an urgent medical problem outside surgery opening
hours, please telephone the out-of-hours service on 111. Please do not use this number for any
other matter.
Medical Advice
Medical advice can be obtained at any time by telephoning 111 or by visiting the local chemists.

Practice Nurses
Care Quality Commissions
Our practice nurses are available for routine appointments throughout the day and these can be
booked by telephoning the appointment line. They also specialise in asthma, diabetes, COPD,
CHD and travel.
As from April 1st 2013 we are registered with the Care Quality Commissions.
Registrars/Trainees
Being a proactive training practice, we often have registrars and foundation doctors working with
our team. Sometimes registered doctors who have had extensive hospital experience and who
hope to enter general practice will be invited to join the practice for six months or a year to gain
insight and experience of general practice. The receptionist will tell you who they are when you
book an appointment with them. They are fully trained doctors and under our supervision. Please
accept them as one of us.
We often have students with us, both medical and nursing. A student may be present when you
book and/or when you attend for your appointment (if you are in agreement).
Training and Research
We are a research active practice and we occasionally take part in research projects which you
may be invited to participate in. You do not have to participate in any medical research or training
- please tell us if you do not wish to do so.
Repeat Prescriptions
In the interest of safety and accuracy, prescription requests are not taken over the
telephone. Our receptionists are instructed that they are not to take such requests.
If you need regular medication we provide a repeat prescription system, which allows you to have
repeats of your medication, up to certain limits in numbers and time, without you having to see the
doctor every time. Repeats can also be requested via our website.
Please allow two working days’ notice for the production of a repeat prescription.
If you are unable to collect your repeat prescription in person, provided you supply a stamped
addressed envelope, we will forward the prescription to your registered address when processed.
Alternatively, your pharmacist may have a system of prescription collection from the surgery - please
ask them for details.
Please make arrangements to collect repeat prescriptions during normal opening hours, ie Park
Surgery Monday to Friday 8.00am to 12.30pm and St Francis Monday to Friday 8.00am to 6.30pm.
Complaints
If you are not happy with any of our services please let us know. We aim to provide the best possible
service in pleasant surroundings. Our staff are trained to help you. If you have any suggestions for
improving our services, please let us know, either in person or by writing to Sue Levesque, Business
and General Practice Manager.
Most cases can be resolved quite easily but if you feel we have not dealt with the issues you have
raised as you would have liked, you can contact the Clinical Commissioning Group.

Test Results
You may phone for the results of tests after 2.00pm on 023 8025 2131 option 4, but please
remember the following points:
1. Results will only be given to the specific patient to whom they apply. In the interest of
confidentiality, please be prepared to identify yourself. Please do not phone for results
for friends/relatives without prior arrangement (written consent may be required).
2. The surgery tends to be at its quietest after 2.00pm and the receptionist will have
more time then to deal with your enquiry. Checking results is time-consuming and
requests at other times may result in you being asked to call back later.
3. You will need to know the specific test results that you require. Some tests take longer
than others to process. If you have had several tests the receptionist will not necessarily
know whether all the results are back unless you can identify them.
4. Please do not expect the receptionists to have any medical knowledge. They are only
instructed to tell you if your test is normal. If any test is abnormal, or you require
further discussion or interpretation, then please refer to your doctor.
We do hundreds of tests and we cannot contact you all with your results. We do, however, ensure
that all test results are seen by a doctor.

stroking, pressing and kneading different areas of
the body, which can relieve pain, relax, stimulate,

Disabled Access
Both surgeries have been designed and built to accommodate the needs of disabled patients. The
main entrances are appropriately ramped and doorways are easily negotiable by normal wheelchairs.
Counters have lowered sections where possible to provide reasonable access to people unable to
use the higher main counters. There are disabled toilet facilities on every floor and where appropriate
a lift is available if required.
These are the local standards set within this practice for the benefit of our patients. It is our job to
give you treatment and advice. Following discussion with you, you will receive the most appropriate
care, given by suitably qualified people. No care or treatment will be given without your informed
consent. In the interest of your health it is important for you to understand all the information given

Patient Confidentiality And Data Protection
Our Responsibility To You
We ask you for personal information so that you can receive appropriate care and treatment. This
information is recorded on computer and we are registered under the Data Protection Act. The
practice will ensure that patient confidentiality is maintained at all times by all members of the
practice team. However, for the effective functioning of a multi-disciplinary team it is sometimes
necessary that medical information about you is shared between members of the practice team.
We are committed to giving you the best possible service.
Names: People involved in your care will give you their names and ensure that you know how to
contact them. The surgery should be well signposted and the doctors’ names are indicated on
their surgery rooms.
Waiting Time: We run an appointment system in this practice. You will be given a time at which
the doctor or nurse hopes to be able to see you. You should not wait more than 30 minutes in the
waiting room without receiving an explanation for the delay.
Access: You will have access to a doctor rapidly in cases of emergency.
Please indicate to the receptionist if you consider your problem to be urgent.
Telephone: We will try to answer the phone promptly and to ensure that there are sufficient staff
available to do this. You should be able to speak to a doctor by telephone; this may be at the end
of their current surgery.
Test Results: If you have undergone tests or x-rays ordered by the practice, we will inform you
of the results at your next appointment. If no further appointment needs to be arranged, we will
advise you when and how to obtain the results.
Respect: Patients will be treated as individuals and partners in their healthcare needs, irrespective
of their ethnic origin, cultural or religious beliefs.
Information: We will give you full information about the services we offer. Every effort will be made to
ensure that you receive that information which directly affects your health and the care being offered.
Health Promotion: The practice will offer patients advice and information on:
• Steps they can take to promote good health and avoid illness.
• Self-help which can be undertaken without reference to a doctor in the case of minor ailments.
Health Records: You have the right to see your health records, subject to limitations in the law.
These will be kept confidential at all times.
Temporary Visitors
If you are away from your registered address, you can see a doctor anywhere in the United Kingdom
by requesting to be seen as a Temporary Visitor. You will need to give details about this practice.
Information on your attendance will be forwarded to us after a set time. Please let us know of your
problem when you return home.
If you have British visitors staying with you, the same applies. We aim to treat new problems and
also provide repeat medication. Remember that for repeat medication a visitor may have to wait
two or three working days for an appointment with a doctor, so plan early.
Foreign Visitors
All patients, regardless of nationality or residence are entitled to Immediate Necessary Treatment.
Patients who are usually resident in EEA member countries are entitled to necessary treatment for
pre-existing conditions as well as those arising during their visit. This entitlement does not continue
beyond the end of their visit and does not include elective care. For patients who are not entitled
to treatment, private services are available and the charges for these are available on application.
Freedom Of Information – Publication Scheme
The Freedom of Information Act 2000 obliges the practice to produce a Publication Scheme. A
Publication Scheme is a guide to the ‘classes’ of information the practice intends to routinely make
available. This scheme is available from the practice manager or reception.
Your Responsibility To Us
We strongly support the NHS policy on zero tolerance. Anyone attending the surgery who abuses
the GPs, staff or other patients be it verbally, physically or in any threatening manner whatsoever,
will risk removal from the practice list. In extreme cases we may summon the police to remove
offenders from the practice premises.

Please let us know if you change your name, address or telephone number.
Please do everything you can to keep appointments. Tell us as soon as possible if you cannot,
otherwise other patients may have to wait longer.
We need help too. Please only ask for home visits by the doctor when the person is too ill to
visit the surgery.

Zero Tolerance
Please keep your phone call brief and avoid calling during the peak morning time for
non-urgent matters.
Test results take time to reach us, so please do not ring before you have been asked to do so.
Enquiries about tests ordered by the hospital should be directed to the hospital, not the practice.
We ask that you treat doctors and practice staff with courtesy and respect.
Remember, you are responsible for your own health and the health of your children. We will give
you our professional help and advice. Please act upon it.

When Death Occurs At Home...

Three midwives are based and work at St Francis Surgery. If you telephone their 01962 863535 number,
please ask the operator to “bleep” the duty Chandlers Ford Midwife. Please give the operator your
name, address and contact telephone number to enable the midwife to call you back.
Expectant mothers will have a named midwife who will be mainly responsible for your antenatal
and postnatal care.
The midwives also work within a team of midwives from other surgeries in Chandlers Ford. They
provide a range of services including antenatal classes, community midwife deliveries at home and
in Winchester Maternity Unit, and also 24-hour “on-call” availability for care and advice.

3. Collect the death certificate from the surgery.
4. Take the certificate to the registrar for the area in which the death took place, together
with the birth certificate, if available. An appointment is necessary.

funeral.

At least two months before departure, discuss any vaccination requirements with the practice nurse.
For travel in EU countries, obtain a European Health Card. Please visit the Post Office for an
application form.
When Abroad
Check on the quality of drinking water. If in doubt either drink only bottled water or use water
purification tablets. Avoid ice in drinks as this may very well have been made from suspect water.
Raw vegetables, salads and fresh fruit should be carefully washed in clean water. If in doubt, stick
to freshly cooked food.
Beware of the sun! Use a high factor sun screen particularly in the first few days of exposure.
Children in particular should be monitored closely in this respect.
In hot climates drink plenty of non-alcoholic drinks. If you are not passing water regularly, you are
not drinking enough.
On Your Return
If you fall ill, don’t forget to tell your doctor that you have travelled abroad.
If you have received treatment abroad, inform your own doctor on your return.
When donating blood, tell the transfusion staff which countries you have visited.
Alcohol
A Little Of What You Fancy...
Alcohol is a depressant which has the effect of dulling the brain and nervous system.
In small quantities, alcohol may actually be beneficial to health. In large quantities, on a regular
basis, it can have a very serious effect on health. At one extreme it does kill.
CIRRHOSIS of the liver, for instance, is killing an increasing number of people, as are drivers who
are over the limit. At the other end of the scale, excess consumption of alcohol can lead to poor
co-ordination, impaired vision, a weakening of the immune system, impotence and obesity.
What Is A Safe Limit?
An accepted limit is 21 units a week for men and 14 units a week for women. (A unit is approximately
a glass of wine, half a pint of beer or a single measure of spirit.) This recommended maximum
presumes that the consumption is spread throughout the week and not consumed all at once in
a “binge”.
A man’s liver can process only eight units of alcohol within a 24 hour period and can take 72 hours
to recover fully. It is easy to see how easy it is to damage this vital organ.
