Covered Illnesses: heart attack, stroke, coronary artery bypass surgery, life-threatening cancer, kidney failure, blindness,
major organ transplant, Alzheimer’s disease, Parkinson’s disease, paralysis, multiple sclerosis, deafness, loss of speech,
coma, severe burns, aortic surgery, benign brain tumour, heart valve replacement, loss of independent existence, loss of
limbs, motor neuron disease, and occupational HIV infection.
LONG TERM DISABILITY (LTD) BENEFITS:
Waiting period 120 days
Amount – 66.67% of your monthly earnings to a maximum of $6,000 or 85% of your pre-disability take-home pay,
whichever is less. Any amount of LTD Insurance over $4,800 is subject to approval of evidence of insurability.
Benefit period – to age 65
100 % (when provided in Canada) to your plan maximum as determined by Emergis Inc.

Eye Examinations. 1 every 24 months
Glasses, Contact Lenses, & Laser Eye Surgery $200 every 24 months
