Customer Eligibility: You must be eligible for and participate in
the Mass Save Home Energy Services Program. You must be a
residential Eversource customer installing equipment between
January 1, 2015 and December 31, 2015. Equipment must have
been installed in a 1–4 family home with an active residential
Eversource Electric account. Conservation Services Group (the
“Vendor”) must receive application and required documents
listed within 30 days of installation.
Post-Installation Verification: Prior to offering any rebate,
Eversource (the “Company”), reserves the right to conduct an
onsite verification that the equipment was installed according to
the guidelines of the program. It does not include any kind of safety
review and should not be relied upon as one. If the Company
determines that the equipment installed does not meet the program
specifications as described in the application, the Company reserves
the right to refuse to pay the rebate.
Rebates: Subject to these Terms and Conditions, the Company,
through its contractual Vendor, will pay rebates to eligible
Massachusetts Eversource customers for the installation of
equipment in Eversource’s service territory.
Rebate Application: The Customer is responsible for completing all
required information on the Rebate Application. Rebate applications
will not be processed if they are not properly filled out.

No Warranties: The Company and the Vendor do not make any
representation of any kind regarding the results to be achieved by
the equipment or the adequacy of such equipment. Eversource
expressly disclaims any and all warranties or representations of
any kind, whether oral, statutory, expressed or implied, including,
without limitation, warranties of merchantability, usage, suitability or
fitness for a particular purpose. Contact your contractor for details
regarding equipment performance and manufacturer warranties.
Changes in the Rebate Program: The program and these Terms
and Conditions may be changed by the Company and the Vendor
at any time without notice.
Contractor Insurance: The Company and the Vendor are not
responsible for any damage that may be caused by or arise out
of an installation of any equipment. The Customer should select
Contractors who carry appropriate insurance coverage and licenses.
Liability & Release: The Company is not liable for any Customer
damages that may occur as a result of the termination of this
program beyond the amount of the rebate. As part of the
consideration for participating in the program, the participant
hereby releases and shall indemnify, hold harmless and defend
Eversource, its member utilities, and the Vendor from any and all
claims, losses, harm costs, liabilities, damages and expenses
(including attorneys’ fees) of any nature whatsoever arising
directly or indirectly out of or in connection with the installation
of the thermostat(s) at the premises or any material and labor
required for such installation. By your signature on the reverse
side of this form and acceptance of energy efficiency incentive(s)
you acknowledge that the data collected through the use of the
wireless enabled thermostat may be shared with your electric
and/or gas distribution company.
No Tax Liability to Eversource and Rebate Administrator:
Eversource and the Rebate Administrator are not responsible for
any tax liability which may be imposed as a result of receipt of
the rebates by the electric utility to the customer.
Eversource recommends that customers seek multiple contractor
estimates for the installation of the rebate eligible, energy efficient
equipment, and services for the products listed on this form.

All replacement heating systems must have an Annual Fuel Utilization Efficiency
(AFUE) equal to or greater than the value noted in the following table. Existing
system being replaced must be fueled by oil or propane and the new system
must be fueled by oil or propane.
