Instructions for filling up the form

CLASS 1 – ADMISSION 2015-16
1. Documents to be produced along with the completed
registration form.
a. Attested copy of date of birth certificate or Extract from
service record in case of uniform defence personnel in the
prescribed format.
b. Residential proof : Attested copy of
First page of bank pass book currently operated/Original
telephone bill latest month/Original gas bill latest month/lease
deed if residing in rented house/Ration card/Driving
license/Voter’s ID/Passport/Aadhaar card.
2. Attested copy of certificate wherever applicable for the
following cases issued by Competent Authority.
SC/ST/OBC (Non creamy layer)/EWS/BPL/Disabled
3. A Self declaration from the parent for the distance from the
Vidyalaya to residence with supporting document.
4. In case of single girl child an affidavit sworn jointly by both the
parents duly attested by a first class judicial magistrate in Rs.20
non - judicial stamp paper should be produced.
5. Attestation must be obtained from a Gazetted officer.
Incomplete registration forms will be summarily rejected.
