What will I need to register?
1. Proof of Address: You will need to provide “proof of residence” when registering your child
for school. This should be two bills or other official documents that contain the name of the
parent/guardian as well as the actual address of the residence. In order to verify residency within
the District, you will need to present two or more of the following items at the time of
registration:
A. Home Title (Warranty Deed, Quit Claim Deed, Security Deed)
B. Lease or rental agreement
C. Current utility (gas, telephone, power, water, and garbage) monthly statement
(must be dated within the last 30 days)
2. Proof of Birth: To enter school your child will need an original birth certificate, doctor’s
records, or hospital records. Birth records in Georgia are available from 1919 to the present and
may be obtained through the Georgia Department of Human Resources by contacting:

3. Certificate of Immunization: Georgia Law requires a certificate of immunization for a
student to be enrolled in a public school. All students must have the Georgia Department of
Human Resources Immunization Certificate (DHR form #3231) acknowledging that the child has
been immunized. A waiver may be granted when there is a medical exemption authorized by a
medical doctor or a conflict with religious beliefs verified by parents’ sworn affidavit. A waiver of
30 calendar days may be granted provided that documentation is on file from the county health
department or a medical doctor stating that an immunization sequence has been started and can
be completed within the 30-day waiver period.
4. Ear, Eye, and Dental Record: A student who has never been in a Georgia public school must
also provide certification of eye, ear, and dental examinations. Any child admitted to school
without a certificate must present one within 30 calendar days. The form for this examination is
DHR Form #3300. All DHR forms may be obtained at the local public health departments (not at
school).
5. Social Security Card: Students enrolling in Georgia public schools for the first time will be
asked by the school to furnish a Social Security number. However, the Social Security number is
not required for enrollment in public schools. If they prefer, parents or guardians may sign a
statement declining to provide the number without giving a reason. Students for whom a Social
Security number is not available will be assigned other student identification numbers.
6. Picture ID: The parent or guardian enrolling the student will need to provide a picture ID.
How old should my child be to attend Kindergarten?
A child must be five years old on or before September 1 to enter a public kindergarten in the
state of Georgia, pursuant to O.C.G.A. 160-5-1-.28. A child who was a legal resident of one or
more states for a period of two years immediately prior to moving to Georgia and who was legally
enrolled in a public or private kindergarten accredited by a state or regional association, would be
eligible to enroll in a Georgia kindergarten program provided that the kindergartener is five years
old by December 31.
