(STATE, INSTITUTIONAL, LOCAL GOVERNMENT AREA)

Letter of Application to the National Secretariat for Registration.

List of members (to include following)
i.
Membership Application Form No.
ii.
Contact Address
iii.
Two Passport size photographs
iv.
Evidence of Payment of Membership Forms Fees
List of Executives (It could be Caretaker Committee or Protem Executives).
Letter of Introduction by State or Zonal Coordinator or a member of the
National Executive (where necessary)
Opening of Corporate Account in line the Association’s Constitution.

p.s.: Note that a minimum of twenty five (25) members are required to form a
Unit Chapter (Institutional and Local Government Areas) and thirty five (35)
for State Chapter(s).
