listed product model(s) between the purchase dates listed on this form. This offer valid in
USA only for purchases of specified product(s) at BrandsMart USA. Fill out this form completely, and submit it along with
1) a copy of a valid invoice, receipt or order confirmation with the qualifying purchase

2) the original UPC barcode and serial number label from the product box. The UPC

Purchase a qualifying Daewoo full size refrigerator at BrandsMart USA between January 1st, 2015 and March 31st, 2015.
Attach a photocopy or original of your store receipt that shows the date the item was purchased and the items that were purchased.
If your item was delivered using the delivery service and the UPC is not available, please submit a copy of the delivery receipt.
Provide a valid email address to receive processing updates regarding this rebate submission.
Review your rebate submission and verify that it includes:
* store receipt that clearly shows purchase date and items purchased
* a copy of your delivery receipt
* this completed rebate form
Make a photocopy of your rebate submission and keep it for your records.
