I verify that I have read and understand the new state solo and ensemble rules, and that all of the students listed have met the following requirements: Students
must be in grade 10, 11, or 12*, must have made a 1st division at region solo and ensemble contest, must have had a piano accompaniment at region if the music
specified such, music was chosen from the required state list, and the specified instrumentation was used.
I hereby certify that my students have met all the requirements in the rules and that all information submitted by me is correct.

* A junior high student who is a regular member of an ensemble only may participate in an ensemble group, provided the majority of the ensemble is composed of
students in grades 10-12. No junior high students (grades7-9) are allowed to perform a solo at state.
