Harmony Parents don’t miss the opportunity to pre-order your child’s school supplies. As a
convenience to our families, the Harmony PTA has partnered with School Box Express to offer affordable
school supply packs. A portion of the proceeds goes to support the PTA. The supplies requested
for each course are listed on the back. Pre-pay now and your child’s supplies will be delivered to
your school in time for the first day. No lines, no running from store to store, no sales tax…just
convenience!

it along with your payment.
• Make checks payable to Harmony Middle School PTA
• Please write your child/children’s name on the memo line of the check
• If you have multiple students attending Harmony please be sure to use a separate order form for each
child. You may however write one check and include it along with your order form/forms.
• Remember to mark Next Year’s Grade level
• All order forms and payment may be turned into student’s 1st Block or 5th Block teachers or House
Secretaries.
