participate in the Dunk Tank at the Cottingham Fun Fair. *

To be executed by all parents/guardians:
Vwe hereby release the Cottingham Jr. Public School and its respective Parent Council members,
officers, employees, stuclents, volunteers, and their successors and assigns (collectively referred
to as the "Council") from any and all liability for damages sustained in consequence of loss,
injury or damage to the person or property of the student referred to above, and from all other
actions, causes of action, claims, or damage of any kind with respect to death, injury, loss or
damage to the person or property of the student (the "Claims"), arising from the student's
participation in the dunk tank. I agree to indemnify the Council against any and all Claims
which may occur as a result of the student participating in the dunk tank.
