EXHIBITION APPLICATION
The Coyotes Skating Club of Arizona invites you to take part in our
“2015 Spring Fever” Exhibition at the Ice Den Scottsdale.
EXHIBITION DATE: Sunday, March 22, 2015
TIME: 8:15 am – 10:15 am (Ice Den Surface)
REGISTRATION DEADLINE: March 18, 2015 at 5:30 pm
As a benefit to our Home Club and Associate members, this Exhibition is FREE. The exhibition is for performance
experience only and will not be critiqued. Applications will be accepted on a first come/first serve basis. Space is limited
therefore only one program per skater may be performed.
Non-CSCA members or Ice Den Scottsdale Basic Skills Learn to Skate members may sign up for a fee of $15. Nonmembers and Basic Skills applications will be held for approval until all Members’ applications have been processed. You
will be notified via e-mail by March 20th if your application has been approved.

connection with participating and voluntarily assume and accept those risks. I unconditionally release, waive and covenant not to sue U.S. Figure Skating, the Coyotes
Skating Club of Arizona, Coyotes Ice, LLC and the Ice Den Scottsdale and any of their affiliates and subsidiaries, their promotional sponsors and advertisers and all their
agents, servants and employees from any and all suits, claims and demands of any kind for personal injuries, property damage, including but not limited to, lost, stolen or
damaged goods, that I may sustain while participating in this exhibition.”
