In order for your team members to receive their gift, we require that the representative return the
Team Gear Order Form by the April 27 deadline.

If the player chooses to upgrade to another item, a $5.00 discount will be applied to that item.

Acceptance and distribution of your free gifts are contingent to the order form being submitted by the deadline. NO EXCEPTIONS!
